Replacement: #21459, based on main. This original is closed in favor of the replacement. GitHub refused its base change while it remained in its pre-existing stack.

A retried t3_thread_launch allocated another thread when the first response was lost. Optional clientRequestId now derives caller-scoped command, thread, and message identities so the existing launch receipt replays the original result. Omitting the key still creates a new launch.

The caller/key tuple is JSON-encoded before SHA-256 hashing, preserving lone UTF-16 surrogates without merging them with the replacement character. Bounded IDs avoid long-key failures when naming Scratch folders. Workspace preflight uses the accepted receipt so a retry survives removal of its original base branch or existing worktree. A changed worktree path still undergoes membership validation. Accepted retries use the stored model even after the project default changes; new launches still require a valid workspace and model. Agent initial-message dispatch also enforces the requested runtime and interaction limits under the command lock: a retry cannot start work on an existing thread with broader modes.

Maintainer replacement of juliusmarminge’s #16654. GitHub prevents retargeting that PR while it belongs to its existing 13-PR stack, and unstacking would affect unrelated PRs. This one-layer replacement is based on main and excludes the inherited mode-header feature. Its source tree is byte-for-byte identical to the reviewed and live-tested takeover head 42210fc516; only branch ancestry changed. The original PoC request-key encoding changed, so receipts created by that unreleased encoding are not replay-compatible.

Focused suites: 86 → 88 passing tests across the existing handler and launch-service files. Tests cover caller-scoped retry identity, lone-surrogate keys, long-key Scratch folders, narrower agent retries, and accepted workspace retries after the original branch/worktree disappears. Two mocked preflight tests were replaced by integrated receipt-replay cases, and an existing cross-project test now also covers caller-supplied IDs. Server, contracts, and provider-core typechecks, scoped lint and formatting pass (one pre-existing lint warning). Three adjacent MCP suites also pass: 97 tests.

Live proof on headless Linux used HTTP MCP, two isolated OAuth clients, and disposable state. Same-key retries returned the same thread; separate keys, callers, and omitted keys remained distinct. A lone surrogate key retried successfully and stayed distinct from the replacement character. The database contained thirteen unique resulting threads with one accepted creation receipt each and zero provider runs. Both 255-character Scratch keys with the same suffix succeeded. Accepted retries still returned the original thread after deleting a base branch or removing an existing worktree; new invalid launches were rejected. Clearing the project default preserved the original thread/model on retry while a fresh launch failed. Retrying with a changed path outside the project worktrees was rejected. No initial messages were sent.

Before: retry allocates a second thread · After: key replay and fresh launches · Caller and Unicode boundaries · Persisted receipts and zero runs

Scratch: before / after. Deleted base: before / after. Removed existing worktree.

Default model: before / after. Changed workspace: before / after.

Independent full-diff reviews: three paired rounds with Claude Opus 5.5 and GPT-6 Sol, followed by a fresh Sol clarification review. Confirmed findings fixed: malformed/long keys, workspace checks before accepted replay, the cleared model default, and changed-worktree validation. Final reviewers found no code blockers. The changed-payload objection was withdrawn because retries explicitly require the same payload; no persistent payload manifest was added.

Not verified in this pass: peer forwarding across two environments, relay/tunnel transport, actual provider turns, macOS, Windows, iOS, Android, or Electron desktop. Dispatch-mode protection is covered by the focused service regression, not a live provider run.

Implementation: GPT-6.1 Sol through the Codex harness in T3 Code; tests: GPT-6 Astra; reviews: Claude Opus 5.5 and GPT-6 Sol.

End-to-end run, two real servers

Two t3 serve processes from the top of this stack, each with its own data directory, on one machine. A laptop (port 3971) and a box (port 3972). Their projects are clones of one bare origin, so they share a repository. An outside agent (OAuth with a pairing code) drives the laptop's /mcp, and real Claude Sonnet 5.5 turns run on both sides. The last part repeats the run over Tailscale HTTPS (a *.ts.net HTTPS address).

A forwarded t3_thread_launch was sent twice with the same clientRequestId. Both calls returned the same thread id (mcp:launch:client%3A<link session>:fwd%3A<hash of the key>), and the box has exactly one thread with that title.

Opus 5.5 via Claude Code.

Approvability

Verdict: Would Approve

Macroscope's review found this PR approvable — This is a narrowly scoped, backwards-compatible idempotency addition: callers opt in with clientRequestId, while existing launches remain unchanged, and focused tests cover replay and related launch behavior. The unresolved high-severity findings concern long-key filesystem handling and workspace-change retries and are captured by the repository’s blocking correctness mechanism.

Test receipt replay through the real launch service.

This test says a retry replays the first launch, but clientLaunchHarness mocks ThreadLaunchService.launch and returns the supplied ID on every call. The assertions therefore cannot detect a failure in receipt replay. Keep the ID-derivation assertions, but add a service-level test that accepts a launch and retries its command ID against the receipt store.

As per coding guidelines, “Tests exercise behavior through the service, with test layers only for external dependencies. Don't mock the logic under test.” The matching path instructions require testing behavior through the service.

✅ Confirmed as addressed by @juliusmarminge

Fixed. Added a real launch-service test: an agent's launch with the same key, from a narrower caller, cannot start work on the broader thread the first launch created. It runs through ThreadLaunchService.launch and the real orchestrator receipts, and fails without the in-lock check.

@juliusmarminge Thanks for the fix. The reported test exercises ThreadLaunchService.launch with real orchestrator receipts instead of mocking the replay path. It also checks that a narrower caller cannot start work on the broader thread.

Move request-key ID derivation into the launch service.

The handler now defines how a request key maps to a launch command. Put that mapping in the domain service so other entrypoints use the same retry behavior. Keep the handler responsible for passing the decoded key and mapping typed errors.

As per coding guidelines, “A transport handler does three things: decode the request, call one service method, and map the service's typed errors to the transport's error. Nothing else.” The matching path instructions also place capability logic in the service.

Not changing. The handler is the only entry point that takes a client key; every other launch path already supplies its own command id. Moving the mapping would add a service parameter with one caller. The behavior the key protects (replay and the mode ceiling) does live in ThreadLaunchService.

@juliusmarminge Thanks for the clarification. The mapping adapts the MCP client key to the existing command-ID contract. It does not implement replay or the mode ceiling. Moving it would add an MCP-specific service parameter without a shared caller. I withdraw this finding.

5Learning: In t3code, apps/server/src/mcp/toolkits/project/handlers.ts is the only launch entrypoint that accepts clientRequestId. Other launch paths supply their own command IDs. The MCP handler maps the request namespace and client key to a command ID; ThreadLaunchService owns replay and the mode ceiling. Do not require moving this transport-specific mapping into the launch service solely to share retry behavior.

A clientRequestId containing an unpaired surrogate (for example, "key-\ud800") throws URIError here, aborting the launch instead of returning an invalid_request failure. encodeURIComponent rejects lone surrogates; validate the ID before encoding or use an encoding that accepts arbitrary UTF-16.

Fixed in the takeover head: the caller/key tuple is JSON-encoded and SHA-256 hashed, so lone UTF-16 surrogates neither throw nor collide with U+FFFD. Fixed-length IDs also avoid long-key Scratch folder failures. The focused regression covers both keys, and the isolated HTTP MCP proof in the updated PR body verifies successful distinct launches and receipt replay.

The current head still uses encodeURIComponent, which throws on lone surrogates. The surrogate regression was also removed, so the original issue remains open.

Taking over this one for sidebar-threads-08. Main still allocates a new launch on retry; I’ll reduce the tests, address the malformed request-key finding, and finish verification here.

Replaced by #21459.

The original proof of concept correctly identified caller-scoped command receipts as the way to prevent duplicate launches. The replacement credits juliusmarminge and keeps that behavior, with bounded Unicode-safe IDs, receipt-aware workspace/model replay, and the initial-message mode guard.

I compared the original diff against the replacement: the request-key schema, caller scoping, stable thread/message identities, mode guard, tests and agent guidance are carried over. The inherited mode-header feature is excluded because it belongs to the existing stack, outside this takeover. GitHub rejected changing this PR's base while it belongs to its 13-PR stack; the replacement targets main without restructuring those other PRs.

Three paired review rounds and a Sol clarification found and resolved the confirmed issues. The replacement has 88 focused and 97 adjacent MCP tests passing, live isolated HTTP MCP proof, passing code CI, and no unresolved bot findings. Macroscope's scope-based human approval remains required. Nothing has been merged.

An identical retry of an accepted launch returns invalid_request instead of the original thread identity if its workspace changes: deleting the local base branch makes checkWorktreeBase reject a worktree retry, and removing the path makes the existing_worktree membership check reject it. These preflight checks run before launchThread can replay the accepted command; consult the accepted receipt before revalidating the original workspace, while still validating changed paths.

Fixed the Macroscope finding on project/handlers.ts (a same-key retry failing after the project's default model is cleared): a keyed launch now reuses the model of the thread it already started before requiring project defaults. Regression test: a retried launch replays after the project loses its default model (1aa278ba28).
