fix(codex): stalled HTTP 429 retries end with an explicit failure instead of hanging#23842

Problem

The Codex CLI retries a rate-limited (HTTP 429) model request itself. The only thing the app-server sends is an error notification with willRetry: true. If the response carries a long Retry-After, Codex sleeps for that whole time and sends nothing else. The T3 Code turn then stays running with no further events. Any delegated task waiting on it also hangs, and the user gets no failure, reset time or recovery hint. In the case reported in #17503, a post-tool 429 retry left the turn running for hours.

Expected behaviour: the turn either makes progress or ends in a clear, retryable failure that includes the rate-limit reset time when it is known.

Refs #17503

Change

The change is limited to the Codex adapter (apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts) and a new pure helper module (apps/server/src/provider/codexRateLimitRetry.ts). There are no contract, client or settings changes.

Retry policy. Each willRetry: true notice classified as HTTP 429 arms a deadline:

  • At most 5 retry attempts.
  • With no Retry-After, the expected delay is exponential backoff: 5s, doubling each attempt, capped at 5 minutes.
  • If Retry-After is present (delta-seconds or HTTP-date), it is the expected delay. The HTTP-date form is measured from the notice time.
  • The per-attempt deadline is the expected delay plus 60s for the retried request to show progress.
  • A Retry-After above 5 minutes fails the turn immediately, reporting the reset time.
  • A 6th attempt, or a missed deadline, also fails the turn.
  • Any progress (deltas, items, compaction) or a newer retry notice clears or re-arms the deadline.

Worst case before giving up:

  • No Retry-After: about 455s (65 + 70 + 80 + 100 + 140).
  • Retry-After at the 5-minute cap on every attempt: 1800s.

Classification. The status comes from codexErrorInfo. When Codex sends no HTTP status (for example responseStreamDisconnected), a 429 / Too Many Requests in the error text also counts. An explicit non-429 status always takes precedence.

Ending the turn. When the bound is hit, the adapter:

  1. Marks the turn as rate-limit exhausted.
  2. Sends turn/interrupt to the native turn and to every active descendant (subagent) turn. Each request is bounded to 10s. Descendants are re-scanned under the existing terminalization permit, so a nested turn started during teardown is either stopped or handled by the existing late-subagent path.
  3. Fails the root turn with a retryable usage-limit failure, code rateLimitRetryExhausted, including resetAt when known.

Late turn/completed or error frames for that turn are ignored, so they cannot overwrite the failure. A subagent turn closes its retry item with the same failure.

Unconfirmed stop. If Codex does not acknowledge the interrupt (it times out or errors), the turn is not reported idle while Codex may still be retrying. It stays active until Codex sends turn/completed, or for at most 60s more. It is then failed and the thread disposition is set to broken, so the next turn runs thread/resume instead of reusing a loaded thread that may still be retrying. A subagent thread in this state is published as error. The same applies to a descendant whose own interrupt was not acknowledged (at the deadline or at the end of the 60s bound): it is marked abandoned, so its thread is published as error rather than idle. A later user Stop that Codex does acknowledge for such a descendant counts as a confirmed stop, so that thread returns to idle. If turn/completed arrives while the abandonment waits for the terminalization permit, the abandonment re-checks the containment state and leaves the confirmed end in place. A user Stop that Codex does not acknowledge keeps the same broken disposition.

The PR has these commits (plus a merge of main):

  1. The bound itself.
  2. Hardening of the bound: Retry-After HTTP-date, textual 429, compaction, interrupt-before-terminal, lineage, unconfirmed stop, and the subagent failure.
  3. Descendants whose 429 interrupt is unconfirmed are marked abandoned instead of idle.
  4. A user Stop acknowledged for a late descendant counts as a confirmed stop.
  5. The abandonment re-checks the containment state after acquiring the permit.

Scope and approval

This is a reliability fix for one problem: a Codex turn hanging silently after an HTTP 429. It links #17503, which has not been triaged by maintainers yet. I'm happy to wait for triage, or to narrow the scope (for example, to the first commit only) if you prefer.

Verification

How the problem was confirmed. The adapter tests replay Codex app-server transcripts. One has a tool call followed by an error notice with willRetry: true and HTTP 429, then silence. Against the unchanged adapter at 044f07257 (the Codex adapter and its tests are identical at 22840ab9, the main this branch now merges), I ran the new 429 tests (vp test run src/orchestration-v2/Adapters/CodexAdapterV2.test.ts -t 429):

  • The ends a stalled post-tool 429 retry with an explicit failure cases failed with expected [] to have a length of 1 but got +0: no terminal event was ever produced after the deadline.
  • The run never finished and was killed by a 600s timeout. This is the same silent hang as in the issue.

How it was tested. Each file was run separately from apps/server with vp test run <file> (Node 24.22.0):

FileResult
src/orchestration-v2/Adapters/CodexAdapterV2.test.ts97/97 passed
src/provider/codexRateLimitRetry.test.ts7/7 passed
src/orchestration-v2/testkit/CodexReplayFixtures.integration.test.ts24/24 passed
src/provider/Drivers/CodexDriver.test.ts11/11 passed
src/orchestration-v2/DelegatedCompletionDelivery.test.ts26/26 passed
src/provider/codexUsageLimits.test.ts15/15 passed
  • The 429 subset (-t 429) was run 3 times: 26 passed each time.
  • At the first commit alone: CodexAdapterV2.test.ts 79/79, codexRateLimitRetry.test.ts 5/5.
  • vp fmt --check on the 4 changed files was clean. vp lint gave 0 errors; its 2 no-unused-vars warnings are already present on main.

What the tests cover and what was observed:

  • Stalled 429 retries, covering backoff, short Retry-After, HTTP-date Retry-After, textual 429, and an over-cap Retry-After. The turn fails at exactly notice time + deadline on TestClock, with code rateLimitRetryExhausted and resetAt. turn/interrupt is sent before the terminal is published.
  • A retry that resumes into context compaction keeps running.
  • Retry-policy unit tests for backoff, the cap, the attempt limit, and Retry-After parsing (delta-seconds, HTTP-date, over the cap).
  • Lineage: the root and active descendants are interrupted, including a nested turn that starts during teardown or after the last scan. The root terminal does not report childTurnsContinue.
  • Unconfirmed stop:
    • The turn stays active when the interrupt times out or errors.
    • It is failed as broken after the 60s bound.
    • A late turn/completed is ignored.
    • A nested turn started during the wait is stopped.
    • An abandoned subagent thread becomes error, not idle.
    • A descendant whose interrupt fails at the deadline, or a late descendant that cannot be stopped within the bound, is published as error, not idle.
    • A late descendant whose unconfirmed 429 interrupt is later confirmed by a user Stop is published as idle.
    • A Stop Codex never acknowledged keeps broken.
  • A late final error during the interrupt does not replace the rate-limit failure.
  • A subagent retry that runs out of attempts closes with the usage-limit failure.

The tests use TestClock and wait on observable milestones (the retry notice, the interrupt request). There are no real-time waits.

Not verified:

  • Not run against a live Codex CLI / app-server, and no real upstream 429 was triggered. Coverage is replay transcripts and unit tests only.
  • Ran on Node 24.22.0, not the pinned Node version. The full typecheck (vp run typecheck, 0/32 cache hits) passed with 0 errors across 32 tasks; node scripts/release-smoke.ts passed (6/6) on the final head; the full test suite and the browser suite were not run.
  • Only contextCompaction counts as progress among the early-returning item handlers. subAgentActivity / userMessage items do not re-arm the deadline.
  • The containment re-check in commit 5 guards a race between turn/completed and the permit acquisition that the replay harness could not reproduce deterministically, so it has no dedicated failing-before test.
  • A turn in the unconfirmed-stop state has no timer beyond the 60s bound, and it does not send background-terminal termination requests.

Agent use: this change was written and tested locally with AI coding agents (Grok Bot). The commands and results above come from those local runs.

Fixed in a77e1c8f9. Fixed in ac46a1e62. turn/completed for an unconfirmed root now stops the active lineage first, rescanning under the terminalization permit like the bounded wait does, so a nested turn started during the wait is either stopped before the root fails or starts after the root is terminal. Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval โ€” This PR adds substantial production concurrency and terminalization logic that changes default Codex retry behavior, including stopping provider and descendant work and marking threads broken when containment is uncertain. The implementation also has an unresolved race involving descendant cleanup during root completion, so the lifecycle behavior warrants human review.

๐Ÿ“ Walkthrough

Codex rate-limit handling now parses HTTP 429 signals and retry delays, applies bounded retry decisions, and interrupts active turns when retries are exhausted. The adapter tracks stop confirmation and descendant turns, then finalizes failures and thread status based on containment.

Changes: Codex 429 Retry Handling

Layer / File(s)Summary
Retry policy and error parsing
apps/server/src/provider/codexRateLimitRetry.ts, apps/server/src/provider/codexRateLimitRetry.test.ts
Adds bounded retry decisions, retry-delay parsing, and HTTP 429 detection, with tests for retry limits, delay formats, and status extraction.
Adapter retry tracking
apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
The adapter detects rate-limited retry notices and tracks retry state. Context compaction completes that state. Replay tests cover retry timing and compaction.
Exhaustion and turn containment
apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
On retry exhaustion, the adapter interrupts root and descendant turns and tracks stop acknowledgements. Replay tests cover late descendants and unconfirmed stops.
Failure finalization and thread disposition
apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.test.ts
Finalization preserves exhaustion failures and reset times. Thread status reflects whether a stop is contained. Replay tests cover root and subagent outcomes.

๐ŸŽฏ Functional Correctness | ๐ŸŸก Minor | โšก Quick win

Read the time unit that follows a retry after N value.

The retry[-_ ]?after regex captures only the number and always multiplies it by 1,000. A prose message such as "Please retry after 2 minutes" therefore gives 2,000 ms, not 120,000 ms. The phrase fallback at Line 125-132 never runs, because the seconds match returns first.

The consequence: decideCodexRateLimitRetry gets a delay that is far too short. endStalledRateLimitedTurn then interrupts and fails the turn after 62 s, even though the provider asked for 2 minutes. A turn that would have recovered ends as rateLimitRetryExhausted, and resetAt is wrong.

To fix this, capture an optional unit on the same line and use UNIT_MS to scale the value. Seconds stay the default, so a bare header value still parses the same way.

Fixed in bf6a2fa2e. The retry after N match now takes an optional unit, as try again in already did, so "retry after 2 minutes" and "retry after 500ms" are read correctly; a retry-after-ms header is read as milliseconds, and a bare value stays delay-seconds. Added test cases that fail before the change and pass after.

@Hylouis233 Thanks for fixing this in bf6a2fa2e and adding regression tests. The changes you described address the unit-parsing issue and preserve seconds as the default for bare values.

P2 Badge Enforce the timeout across descendant teardown

When the root interrupt is unacknowledged and any descendant is still active at the 60-second deadline, abandonUncontainedRateLimitedTurn calls this loop after its sleep, and each interruptRateLimitedDescendant can spend another 10 seconds in interruptRateLimitedNativeTurn; the loop does so serially and rescans for newly spawned descendants. Thus one silent child makes the advertised 60-second bound at least 70 seconds and enough children can keep the root running arbitrarily longer, reproducing the hang for subagent-heavy turns. Use the same absolute deadline for lineage cleanup, or settle at the bound and make child interrupts concurrent or best-effort.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Addressed in c8156276a for the 60s unconfirmed bound. At that bound, interrupts for the nested turns are sent without waiting, and the lineage and root settle under one permit right at the bound, so one silent subagent no longer stretches this cap. A new adapter test fails before the change and passes after. Known limits: this is deliberately conservative, so a nested turn that confirms its interrupt only after the bound is still published as broken. The two other cleanup paths (the initial deadline path and the path after the root turn ends) still wait on descendants serially; they are outside this 60s bound and are unchanged in this commit.

P1 Badge Evict broken root threads from the live-session cache

When the 429 interrupt is unacknowledged and the same provider session stays live, returning only threadDisposition: "broken" updates the persisted projection but does not invalidate ProviderSessionManager's loaded-thread cache. Its event pump deletes loadedProviderThreadKeyByThread only for a provider_thread.updated event whose status is error (ProviderSessionManager.ts:1973-1988), and the resume wrapper skips the native thread/resume RPC on a cache hit (ProviderSessionManager.ts:1789-1792). Consequently an immediate follow-up can issue turn/start against the still-retrying native thread, defeating the containment this branch promises; emit the root thread error/update (as the subagent path does) or teach the manager to evict on a broken terminal.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Addressed in 386516b80. An uncontained 429 root now publishes its provider thread as error before the broken terminal, so the session drops the loaded thread and the next turn resumes it instead of reusing it. A new adapter test fails before the change and passes after. Scope note: this is verified at the adapter layer and with the existing session-manager tests only; I have not run an end-to-end check.

P1 Badge Wait for the authoritative interrupt completion

When a normal turn/interrupt response arrives before its later turn/completed notification, rootInterrupted is true and this branch finalizes the turn as reusable immediately. The probe-derived contract says the request returns first and only turn/completed is authoritative (docs/orchestration-v2/README.md:87-88); the ordinary Stop path accordingly waits on turnWaiters (CodexAdapterV2.ts:7684-7692). A queued follow-up can therefore issue turn/start during the gap while the native retry turn is still active, and descendants are finalized on the same premature criterion; wait boundedly for the completion event, then mark the thread broken if it never arrives.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Addressed in f0478d229. After the 429 deadline interrupts the root or a descendant, a turn/interrupt reply no longer counts as confirmation; the adapter waits up to 10s for that turn's turn/completed.

  • A root that does not end in time moves to the existing unconfirmed bound, then fails with the thread disposition broken.
  • A descendant that does not end in time is abandoned, and its thread is published as error.

New regression tests, both failing on the previous head and passing now:

  • "waits for turn/completed after a 429 interrupt reply before ending the turn"
  • "marks a descendant errored when Codex only replies to its 429 interrupt"

A third test, "ends a 429 turn once turn/completed follows its interrupt reply", covers the confirmed path (passes before and after). The existing 429 lineage test transcripts now include the turn/completed events Codex emits after an interrupt, since a bare reply no longer settles them.

Not verified:

  • The real ordering of the interrupt reply and turn/completed, especially for subagent threads, has only been exercised in replay transcripts, not against a live Codex.
  • The 10s confirmation budget is a design choice, not something from the Codex docs.
  • The user-initiated Stop path is unchanged and still treats the reply as confirmation.
  • A nested turn that starts while the root is waiting is interrupted on the rescan after the root confirms (or after 10s), not immediately.

Keep an unconfirmed rate-limited turn non-reusable until turn/completed.

A successful turn/interrupt reply only acknowledges the request. It does not confirm native completion. The Stop and late-descendant paths set rateLimitStopAcknowledged, then finalize while rateLimitContainment remains unconfirmed. The predicate therefore returns false, which marks the root thread reusable or the subagent thread idle. A later turn can reuse a native thread that is still running. The 60-second abandonment timer does not prevent this ordering because the acknowledgment path can finalize the turn before the timer marks it abandoned.

Keep unconfirmed classified as uncontained. Only the turn/completed path should change the containment to ended.

Synced from GitHub

Thanks @coderabbitai, the outside-diff Minor on CodexAdapterV2.ts:1319-1321 was valid. Addressed in 8ea79e49b: isUncontainedRateLimitedTurn now treats unconfirmed as uncontained regardless of interrupt replies, so only turn/completed moves the turn to ended. The now-unused rateLimitStopAcknowledged flag is removed. Added regression tests for a root Stop that only gets a reply (now broken) and a late descendant Stop that only gets a reply (now error), plus a control test where Stop followed by turn/completed stays reusable. The two regression tests fail on f0478d229 and pass now; adapter 111/111 and retry 8/8 pass locally.

Not verified: behavior against a real Codex app-server, CI, and server tests outside these files.

The fnUntraced tracing nitpick at 4796-4798 is not changed in this round: the helper only runs inside the named endStalledRateLimitedTurn / stopRateLimitedLineageThenSettle spans.

Bound descendant interruption across the whole stop.

If several active descendants do not send turn/completed, this loop waits up to 10 seconds for each descendant before it starts the root's 60-second unconfirmed-stop timer. Six silent descendants can add about a minute before that timer even begins, so a stalled turn remains active well beyond the intended bound. Use one deadline for descendant interruption, or start the root's abandonment timer before scanning the lineage. The later lineage-stop path has the same sequential-wait risk.

Keep retry metadata after exhaustion starts.

If contextCompaction starts while the exhausted turn is waiting for stop confirmation, completeProviderRetry deletes its tracked retry and marks the retry item completed. Root finalization then emits the rate-limit failure without its retry metadata. Guard retry completion once rateLimitRetryExhausted owns the turn; apply the same guard to compaction completion and other progress notifications.

P2 Badge Preserve live thread fields in the error update

If thread/goal/updated changes a root thread after the turn was registered, rootProviderThreads and the persisted projection contain the new goal while context.providerThread is still the turn-start snapshot. An uncontained 429 then spreads that stale snapshot here; provider-thread.updated replaces the whole object (ProjectionStore.ts:871-883), and RunExecutionService.ts:1411-1414 adopts it as the latest thread for terminalization, so goal status/token progress is silently rewound while marking the thread errored. Build this update from the current rootProviderThreads entry rather than the context snapshot.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Addressed in 837b02b50. The errored provider_thread.updated is now built from the live root thread rather than the turn-start snapshot, so the latest goal is kept. Regression test: "keeps the latest goal when an unconfirmed 429 marks the thread errored" (fails without the fix, passes with it).

Not verified: behavior against a real Codex app-server (coverage is replay transcripts under TestClock) and CI. Whether Codex emits goal updates after a deadline interrupt in practice, and end-to-end projection/UI handling, are also unverified.

P2 Badge Start the containment timer before waiting on descendants

When a root 429 interrupt is unconfirmed and one or more descendants never emit turn/completed, interruptRateLimitedDescendant can consume its full 10-second timeout and this loop awaits those descendants serially. The 60-second abandonment fiber is not created until line 4975, so even one silent child extends the advertised unconfirmed bound and N children add Nร—10 seconds; descendants spawned during the rescans can extend it further. Although the later abandonment loop now sends interrupts best-effort, this earlier pre-timer loop is fresh evidence that teardown is still unbounded; enter containment before scanning and share one absolute deadline or interrupt children concurrently.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Addressed in fc0ffbc26. Descendants are now interrupted concurrently under one shared interrupt deadline instead of being awaited one by one. Regression test: "starts the unconfirmed 429 bound one interrupt bound after silent descendants" (fails without the fix, passes with it).

Not verified: behavior against a real Codex app-server (coverage is replay transcripts under TestClock) and CI. Real wall-clock timing of the shared deadline is only asserted with TestClock.

P1 Badge Terminate background terminals before failing the retry turn

When the 429 occurs while a commandExecution with a processId is still running in the background, this path only sends turn/interrupt and then finalizes the turn as failed. Failed finalization marks the command item failed and drops its runningCommandItemsByTurn tracking, but Codex background terminals outlive a turnโ€”the ordinary Stop path separately invokes thread/backgroundTerminals/terminate at lines 7705-7759โ€”so the process can keep modifying files after the UI reports failure and the adapter can no longer observe or stop it. Terminate the tracked terminals, or retain their context, before publishing this terminal.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Not changing this in this PR. Background terminal cleanup is separate from the 429 retry bound: an ordinary failed turn/completed does not terminate background terminals either, and Stop is still the path that terminates them. A change here would reach beyond the retry adapter and helper, so it seems better handled separately.

P1 Badge Treat in-flight exhausted turns as uncontained

When Stop is already waiting for turn/completed as the retry deadline fires, rateLimitRetryExhausted is assigned at line 4883 but rateLimitContainment remains undefined until the automatic interrupt wait finishes at line 4925. If Stop's own 10-second wait expires first, it finalizes the context while this predicate returns false, so the terminal is reusable and no provider_thread.updated error evicts the loaded-session cache (ProviderSessionManager.ts:1973-1988), even though neither interrupt produced the authoritative completion required by docs/orchestration-v2/README.md:87-88; an immediate follow-up can therefore reuse the still-retrying native thread. The fresh current-code evidence is this pre-unconfirmed Stop window, so an exhausted native turn should be considered broken unless it has reached ended.

Useful? React with ๐Ÿ‘ย / ๐Ÿ‘Ž.

Addressed in 0a3f6703e. An exhausted 429 turn with a native thread now counts as uncontained until turn/completed confirms it ended, so if Stop's wait expires first the thread stays broken. Regression test: "keeps a 429 thread broken when Stop ends it before the deadline's stop is confirmed" (fails without the fix, passes with it).

Not verified: behavior against a real Codex app-server (coverage is replay transcripts under TestClock) and CI.

  • Major, CodexAdapterV2.ts:4953-4955 (each rescan restarts the descendant interrupt wait): addressed in fc0ffbc26. The deadline is set on the first descendant scan and shared across later rescans in both the stalled-turn and lineage settle paths. Regression test: "starts the unconfirmed 429 bound one interrupt bound after silent descendants".
  • Minor, CodexAdapterV2.ts:5174 (compaction start clears exhausted retry metadata): addressed in 3782d8adf. completeProviderRetry is a no-op once the retry is exhausted, so the failure keeps its retry metadata on every progress path, not only compaction. Regression test: "keeps the 429 retry metadata when compaction starts after the deadline".

Each new test fails with its fix reverted and passes with it. Adapter + retry tests 123/123, typecheck and formatter clean locally; the PR still only touches the same 4 files and the lockfile is unchanged.

Not verified: behavior against a real Codex app-server (coverage is replay transcripts under TestClock), whether Codex emits compaction starts after a deadline interrupt in practice, end-to-end projection/UI handling of the preserved retry metadata, real wall-clock timing of the shared deadline, and CI.

@Hylouis233 Thanks. I inspected head 3782d8ad and confirmed both fixes in the source.

  • Shared deadline: Both endStalledRateLimitedTurn and stopRateLimitedLineageThenSettle initialize the descendant deadline once. Later rescans use the remaining time, not a new interrupt bound.
  • Retry metadata: completeProviderRetry returns before changing retry state when rateLimitRetryExhausted is set. Compaction progress therefore cannot mark the exhausted retry as completed.

Both regression tests contain assertions for the reported failures. The deadline test checks bounded settlement and thread disposition. The compaction test checks the failure code, retained retry metadata, and absence of a completed retry item.

These changes address both findings. I did not run tests or independently verify the reported 123/123 result. CI, live Codex behavior, wall-clock timing, and projection/UI handling remain unverified. Before merging, confirm that the required CI checks pass on this head.
