After the 429 retry deadline stopped a turn, later progress such as a
context compaction start still completed the pending provider retry.
That removed the retry entry, so the turn's failure lost its retry
metadata and the retry item was reported as recovered. Once the retry
is exhausted it failed rather than recovered, so leave it in place for
the turn's failure.
