The provider_thread.updated sent when an unconfirmed 429 marks its
thread errored replaces the whole thread, but it was built from the
turn's snapshot taken at turn start. A goal Codex reported later in the
turn was rewound to the stale one. Build the update from the live root
thread instead, falling back to the turn's snapshot.
