Problem

A provider can report that an old native subagent ended after its process was replaced. Durable tasks and their child work then remain running because the replacement process cannot reconstruct the old task’s lifecycle rows.

Change

Add an internal terminal receipt that the session pump persists even between runs. Reconcile the matching active native task, parent node/items, and child roots/items while preserving the original owner and leaving unrelated work alone. Republish the receipt so an active run releases inherited background tracking without ingesting it twice. This is the foundation layer; #21725 emits these receipts from Claude.

Rebuilt from @luke2x’s original contribution, with author credit in both replacement PRs and Co-authored-by attribution in the carried commits. Please preserve Luke’s release-note credit when this lands.

Scope and approval

Addresses the accepted session-replacement gap in #18877, which prompted the maintainer to reopen the original PR. Stop without a terminal receipt remains outside scope. Live authenticated handoff and native-client verification remain outstanding; the evidence below is integrated SDK replay. Prepared for review, unmerged. The foundation layer still requires human approval for its orchestration lifecycle changes; previous bot approval is not a substitute for that decision.

Verification

Current-head CI results: latest complete validation passed on both heads; CodeRabbit completed, with all actionable threads fixed or explained and resolved. The upper layer has an older superseded run whose aggregate check failed solely because its dependencies were cancelled. Scoped optional jobs were skipped; cancelled duplicates are not reported as passing. Human lifecycle approval remains outstanding.

  • Bottom independently: 155 focused tests passed. Full stack: 448 tests passed; targeted lint, formatting, and server typecheck passed.
  • Before-fix service failures: durable task and idle-pump task remain running, and inherited tracking does not release; all are assertion failures without sleeps or intended timeouts.
  • Five added cases across the full stack replace eight original cases; an existing manager test was strengthened. Main’s tests remain. The original one-off integration file was replaced by cases in an existing suite. Baseline comparison: 451 cases across six executed files → 448 across five; the original baseline had one timeout in an unchanged main test, which passed in stable final runs.
  • CI follow-up: the unchanged attachment HTML browser test failed twice while its lazy source module finished outside React act. The existing test now awaits that module inside act, preserving all assertions. Three browser cases pass locally; the historical CI failure was not reproduced locally; fresh full Web CI and Check now pass on this head. No production web change.
  • Primary proof and limits: headless SDK replay through production services and SQLite. This is a backend state correction; screenshots do not prove it. The external SDK stream is stubbed. No live authenticated Claude handoff, macOS, Windows, Electron desktop, iOS, Android, or remote/tunnel verification was performed.

Models/harness: GPT-6 Astra for test reduction, GPT-6.1 Sol for implementation/integration (Codex); three independent full-stack review rounds by Claude Opus 5.5 (Claude Code) and GPT-6 Sol (Codex); both final reviewers found no code issues, including the replay and browser-test fixes.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The change adds durable native-task receipt handling that can settle parent and child orchestration records and release active-run background tracking. Because it changes cross-component lifecycle state and stream termination for existing work, the production behavior warrants human review.

📝 Walkthrough

The change adds a native task completion event to the provider event contract. The session manager ingests and publishes the event. The event ingestor settles matching subagent records, and the run service removes matching inherited items from active tracking. An attachment preview browser test also changes its rendering synchronization.

Changes: Native task completion

Layer / File(s)Summary
Native task event reconciliation
packages/provider-core/src/server/ProviderAdapter.ts, apps/server/src/orchestration-v2/ProviderEventIngestor.ts, apps/server/src/orchestration-v2/ProviderEventIngestor.test.ts
The provider event contract adds native task ID, status, and result fields. Ingestion settles matching active subagent records and related child-thread records. Tests verify that unrelated records remain unchanged and repeat events produce no updates.
Session event delivery
apps/server/src/orchestration-v2/ProviderSessionManager.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts
The session manager ingests native task completion events and publishes them to runtime subscribers. Tests check event order and the projected subagent status.
Inherited background item lifecycle
apps/server/src/orchestration-v2/RunExecutionService.ts, apps/server/src/orchestration-v2/RunExecutionService.test.ts
Inherited item routes carry native task IDs. The run service removes matching items from active tracking and skips duplicate ingestion. Tests cover receipt handling and ingestion closure.

Preview test synchronization
apps/web/src/components/files/AttachmentFilePreview.browser.test.tsx
The browser test imports ReadOnlySourcePreview inside act before checking that captured HTML appears in the source view.

Full details: Linked Issues check

Explanation

Issue [#18877] requires the replacement Claude session to consume a terminal notification and settle the persisted task, matching items, and child root. This PR implements the receipt contract, session-pump persistence, reconciliation, owner preservation, and inherited-tracking release. The tests cover those foundation behaviors. The reviewed changes do not add Claude adapter or transcript handling that emits subagent.native-task-ended for the issue's synthetic transcript and raw system/task_notification cases. The end-to-end issue behavior therefore remains incomplete in this PR.

Resolution

Add and test the Claude receipt-emission path for both terminal notification forms, or include that implementation in this PR. Keep the persisted-event barrier and projection assertions from [#18877].

Full details: Out of Scope Changes check

Explanation

The orchestration changes and their tests support [#18877]. The change to apps/web/src/components/files/AttachmentFilePreview.browser.test.tsx only changes lazy-module test synchronization. It has no connection to terminal receipts, task reconciliation, or inherited background tracking. It is an unrelated web-test change.

Resolution

Remove the attachment browser-test change from this PR, or move it to a separate web-test fix.

Full details: Description check

Explanation

The description includes all required sections and provides detailed scope, verification results, limitations, and test evidence. However, the required explicit maintainer approval of the lifecycle direction and scope is not provided; the description states that human approval remains outstanding.

Resolution

Obtain explicit maintainer approval for the orchestration lifecycle changes and link the approval comment in the Scope and approval section. Update the section to identify the approving maintainer and confirm the approved scope before merging.

Contain native-task receipt failures.

ProviderEventIngestor reads the receipt thread and, when present, the child thread. ProjectionStore.getThreadRecords fails when either projection is missing. The session pump maps that failure to ProviderAdapterEventStreamError without catching it. The failure then ends Stream.runForEach, releases the session with reason: "runtime_error", and fails its subscribers.

Catch and log receipt reconciliation failures so cleanup remains best effort.

Investigated against the production deletion and projection paths. Thread deletion emits a thread.deleted update with deletedAt; it does not remove the projection row. getThreadRecords reads that row without filtering deleted threads. A real-SQLite probe deleted both parent and child before this receipt and all existing settlement/ownership/idempotence assertions still passed. Native child creation also precedes the task reference, and projection rebuilds are transactional. No supported path to either missing projection was established. Declining blanket catchCause: it would hide storage/schema failures and could skip valid parent settlement. Resolving this lead without a code change; a concrete missing-state production trace would warrant narrow typed handling.

@juliusmarminge Thanks for the clarification. I withdraw this finding. My recommendation assumed a reachable missing projection, but I did not establish a supported production path to that state.

The deletion behavior and creation ordering address that premise. A blanket catchCause is not justified here because it would also suppress storage and schema failures. Narrow typed handling would need evidence of a concrete missing-state path.

5Learning: In pingdotgg/t3code, thread deletion emits `thread.deleted` with `deletedAt` and retains the projection row. `ProjectionStore.getThreadRecords` in apps/server/src/orchestration-v2/ProjectionStore.ts reads deleted thread rows without filtering them. Native child creation precedes the task reference, and projection rebuilds are transactional. Do not recommend blanket `catchCause` containment for native-task receipts in apps/server/src/orchestration-v2/ProviderSessionManager.ts based only on hypothetical missing parent or child projections; establish a supported missing-state path before recommending narrow typed handling.

Wait for the source preview to commit before asserting it.

await import("./ReadOnlySourcePreview") waits only for the lazy module. The preview renders <pre> only after the independent fetch and response parsing complete. The immediate assertion can observe the loading DOM while readFilePreviewResponse is still awaiting the response stream.

Validated the actual fixture rather than treating module resolution as a general fetch barrier. toggleMode already awaits async React act, and this fetch mock returns a buffered string-backed Response. A focused temporary probe wrapped the original reader cancellation and synchronous exact-HTML decoding, then asserted BOTH had finished immediately after toggleMode, before entering the import act. The probe and all original assertions passed. Installed React 19.3 drains these finite immediate continuations across its async-act task boundary. The explicit import waits for the remaining lazy-module/Suspense work, not response consumption. Replacing it with the suggested waitFor restores the synchronization that failed twice in CI. No delayed network/stream producer exists in this fixture. Resolving as a false positive; no permanent test growth or code change. Fresh full Web CI and Check also pass on this head.

@juliusmarminge Thanks for the detailed validation. My finding incorrectly treated response consumption as pending after toggleMode. Your probe shows that the buffered response is already consumed and decoded at that point. The explicit import handles the remaining lazy-module/Suspense work.

I withdraw the finding. No code change is needed.

5Learning: In apps/web/src/components/files/AttachmentFilePreview.browser.test.tsx, toggleMode awaits async React act, and the fetch mock returns a buffered, string-backed Response. The author verified that response consumption and exact-HTML decoding finish before toggleMode returns. The subsequent import inside act synchronizes the lazy ReadOnlySourcePreview/Suspense work. Do not flag that import as an insufficient fetch barrier without evidence of pending response work in this fixture.
