Contain native-task receipt failures.
ProviderEventIngestor reads the receipt thread and, when present, the child thread. ProjectionStore.getThreadRecords fails when either projection is missing. The session pump maps that failure to ProviderAdapterEventStreamError without catching it. The failure then ends Stream.runForEach, releases the session with reason: "runtime_error", and fails its subscribers.
Catch and log receipt reconciliation failures so cleanup remains best effort.

Investigated against the production deletion and projection paths. Thread deletion emits a thread.deleted update with deletedAt; it does not remove the projection row. getThreadRecords reads that row without filtering deleted threads. A real-SQLite probe deleted both parent and child before this receipt and all existing settlement/ownership/idempotence assertions still passed. Native child creation also precedes the task reference, and projection rebuilds are transactional. No supported path to either missing projection was established. Declining blanket catchCause: it would hide storage/schema failures and could skip valid parent settlement. Resolving this lead without a code change; a concrete missing-state production trace would warrant narrow typed handling.
@juliusmarminge Thanks for the clarification. I withdraw this finding. My recommendation assumed a reachable missing projection, but I did not establish a supported production path to that state.
The deletion behavior and creation ordering address that premise. A blanket catchCause is not justified here because it would also suppress storage and schema failures. Narrow typed handling would need evidence of a concrete missing-state path.

| 5 | Learning: In pingdotgg/t3code, thread deletion emits `thread.deleted` with `deletedAt` and retains the projection row. `ProjectionStore.getThreadRecords` in apps/server/src/orchestration-v2/ProjectionStore.ts reads deleted thread rows without filtering them. Native child creation precedes the task reference, and projection rebuilds are transactional. Do not recommend blanket `catchCause` containment for native-task receipts in apps/server/src/orchestration-v2/ProviderSessionManager.ts based only on hypothetical missing parent or child projections; establish a supported missing-state path before recommending narrow typed handling. |
