Provider CLI updates only happened when someone clicked Update on each environment. Settings → General → Update automatically (new, on by default, per environment) now has each server install them in the background when nobody would notice.

When it installs

  • No client interaction for 2 minutes, on the monotonic clock. An install disconnects nobody; a turn sent during one waits for it to finish. The web reporter now listens in the capture phase, so editors that stop key propagation still count.
  • No run preparing, starting, running, or waiting on an approval or question.
  • No provider installer running on this host, including a manual one or one started by another T3 server.
  • A pass runs every 10 minutes and installs one instance at a time. Each instance@version is tried at most once per 6 hours.
  • Update automatically needs Provider update checks on. While checks are off, the switch is disabled on web and mobile and says so. Dev servers skip it, since they share the host's CLIs. An unreadable settings.json keeps it off.

What it installs

  • The newest version the update check already vetted, pinned (npm install @openai/codex@0.162.1, not @latest), and never one the compatibility manifest marks broken or unsupported. Only native self-updaters and Homebrew run unpinned.
  • Under the lock it re-probes the CLI and skips if it already reaches the target, so it never redoes another server's update or downgrades a newer beta.
  • Background installs are silent: no update state goes to clients, so there is no toast, pill, or streamed output. Results go to the server log.

Install safety

  • One numbered lock file per install target in the user cache dir (t3/provider-install/<target>.lock.N), shared by every T3 server on the host. Exclusive creation is the only step that picks an owner: a server writes its record to a temp file and hard-links it as generation N+1, which fails if anyone else got there first, so two servers reclaiming a stale lock at once cannot both win and nothing is ever deleted out from under an owner. The record holds the server's pid, then the installer wrapper's, plus a per-run owner id; the server stays the owner through verification, and only a surviving installer keeps it after that. A lock whose pids are all dead, or a record older than an hour, counts as free. A successor that takes over an aged record whose pid was still alive re-reads that record after publishing its own generation and backs off if it changed, while a resumed owner refreshes its record before checking for a newer generation and sending "go": at least one side sees the other, so a server paused past the hour cannot install alongside its successor (at worst both back off and retry). Rewrites go through a temp file and rename, so a full disk never leaves an empty, held lock.
  • Turn starts wait while any install runs, so a CLI is never replaced as it launches. The run is persisted as starting first, so the updater's last check under the lock sees it and backs off.
  • The installer runs detached through a small Node wrapper with its output in a log file. The wrapper starts nothing until the server has durably recorded its pid and sends "go"; if the server dies first, or "go" comes more than 30 seconds late (a paused server resuming), it exits without touching the target. Once started, quitting or crashing the server no longer kills the install. The wrapper itself kills the installer tree after 30 minutes, so a hung install is bounded even with its server gone, which keeps the one-hour age cap safe. Main killed the installer on scope close and after 5 minutes, which could cut an npm extract in half.
  • A restarted server waits for a running installer, then refreshes the providers.
  • Manual updates and Update everything (#18053) use the same runner, now one per server instead of one per connection, and the same lock.

This replaces the earlier version of this PR (+8.6k lines). Its host-wide reader/writer lease, install supervisor, launch-permit admission across every probe, scheduled-task blockers, update origins in the protocol, and per-driver ownership changes are gone.

Verified

  • A non-dev server with no client connected, an isolated npm prefix, and Codex 0.160.0. The pass at +22 minutes found the window open and logged "Updating provider in the background". The lock file held the installer pid, ps showed npm install @openai/codex@0.162.1, Codex on disk became 0.162.1, the run logged success, and the lock file was removed. Earlier passes stopped at the window check.
  • SIGKILL of a second server 160 ms after its installer started: the installer was reparented, finished ("changed 2 packages in 7s"), and its lock kept the installer pid until it exited. A restarted server reported the provider behind while a lock's installer was alive, and current within 3 s of it exiting.
  • Lock protocol, on Linux with independent Node processes driving the real ProviderInstallLock and detached runner against an isolated prefix (fake installer that detects overlapping writers): 200 two-server install races with no overlap; 100 rounds of 8 simultaneous stale reclaimers with exactly one winner each; parent SIGSTOP+SIGKILL before and after recording the wrapper pid (wrapper exits, a second server installs); SIGKILL after "go" (lock held until the installer exits); a paused server resumed after the 30-second "go" deadline starts nothing; a lock held by an unrelated live pid frees after an hour; the wrapper's own backstop kills the installer and its descendants with the server dead. 200 forced rounds of a paused owner resuming while a successor takes over: 83 owner wins, 65 successor wins, 52 mutual backoffs, never two installers. The wrapper itself also ran under Electron 44.4.5's bundled runtime (ELECTRON_RUN_AS_NODE=1, on a Blacksmith Ubuntu runner): "go" runs the installer and forwards its exit code, the installer does not inherit ELECTRON_RUN_AS_NODE, closing stdin without "go" installs nothing, and the wrapper's backstop kills the installer and its descendant. Blacksmith: 85 lock/maintenance/turn-start/node-runtime tests, server and shared typechecks, lint, format.
  • After merging main (b72740852f): live with an isolated HOME, a pinned automatic install, deferral behind an active turn, waiting on the install lock, and a failed install followed by a successful retry. Blacksmith: 252 tests, then 68 lock/maintenance/turn-start tests after the lock ownership fix.
  • After merging main again (2893437f26, head 87aad2b47c): a real source server with an isolated home and Codex 0.162.0 in a temp npm prefix, latest 0.163.0. Client activity and a running thread each kept the install off. Once idle, one real npm install ran. A turn sent during it stayed starting until verification, then ran. The WebSocket snapshot moved to 0.163.0 / current. With Update automatically off, with Provider update checks off, and with a corrupt settings.json, nothing installed. An installer exiting 17 left a failed state, released the lock, and the next turn started. A hung installer was killed by the backstop (shortened at runtime to 7 s). With "go" withheld, the real 30 s deadline started nothing. Killing the server mid-install left the detached install running, and the restarted server saw exactly one install. Two servers sharing one prefix ran exactly one installer, and both reported 0.163.0. Blacksmith: 168 + 180 focused tests, server typecheck, lint, format.
  • Blacksmith: the touched server tests plus ws, runtimeLayer, and the mcp/ tests (214 and 404 tests), 57 web test files (602 tests), and server, web, mobile, and contracts typechecks. Lint and format are clean.

Not verified: Windows (.cmd shims through the wrapper, installer descendants, taskkill /T, turn starts during an install), the packaged desktop app end to end (the wrapper itself was run under Electron's runtime), and the mobile setting on a device. These need a Windows machine and a phone.

Known gaps: a server cannot see another server's already-running turn on the same host; that server's new turns do wait on the lock. A hung installer holds turn starts until the 30-minute backstop. Muse is not installed in the background; its own launcher updates it.

Before (main): Settings → General has no automatic update row

After: Update automatically sits below Provider update checks, on by default

Current head (8bb20296d1), seeded dev data:

Current head, desktop light: Update automatically on, directly below Provider update checks

Current head, desktop dark: the same two rows, both on

Current head, 390px phone width: both rows stack with their switches on

With Provider update checks off, Update automatically was still toggleable even though the server ignores it then (36021ba04f fixes this):

Before 36021ba04f: Provider update checks off, Update automatically still on and toggleable with no explanation

After 36021ba04f: Update automatically is disabled and reads "Turn on Provider update checks to update automatically."

Made with Opus 5.5 in Claude Code, with Sonnet 5.5 and GPT-6.1-Sol (Codex) subagents, running in T3 Code.

Keep the window closed when resume state cannot be read.

If getLimitRecoveryCandidates fails, orElseSucceed(() => []) treats an unknown resume schedule as no upcoming resume. A typed getSettings failure also bypasses the resume check. Unlike the active-thread and scheduled-task reads, these failures can allow an install immediately before an automatic resume. Return a closed window on either read failure. Effect.option maps typed failures to None. (effect.website)

✅ Confirmed as addressed by @maria-rcks

Note

Written by gpt-6.1-sol on behalf of Maria

Settings and recovery reads now fail closed. The window checks per-thread armed resumes even when the global default is off, and protects eligible near-due unarmed resumes when the global preference is on. Malformed persisted settings also disable automatic updates and restart continuation without replacing the file.

@maria-rcks Verified the fix in UpdateWindow.ts. Both getSettings and getLimitRecoveryCandidates failures now return a closed window. The recovery check also runs when autoResumeLimitedThreads is off.

The malformed-settings fallback disables automaticUpdates and continueThreadsAfterServerUpdate. This addresses the finding.

Clear Claude threads with no pending background tasks.

The Claude completion path emits status: "active" even when the roster is empty. Both active-work selectors then block updates although no run or background task remains. Set the thread to idle when the completed turn has no roster entries.

✅ Addressed in commits ce07c86 to d0fb53f

@maria-rcks Thanks for clarifying. The supplied PR context shows that Claude emits idle after a completed turn when its background roster is empty. The compact-turn test checks that lifecycle event. Pending background work keeps the provider thread active.

The concern does not apply to the current code. I withdraw this finding.

Include pending turn items in active-work selection.

When a command_execution, dynamic_tool, or subagent item remains pending after its run ends, this selector can report no active work. The SQL active-work selector at Line 3611 also omits those items. The existing runtime selectors include them and exclude items from rolled-back runs. Apply that same condition in both active-work selectors so an update does not begin over pending background work.

Check the due time of frequent scheduled tasks.

If an interval task repeats every ten minutes and its next run is one minute away, this branch skips it before the five-minute lookahead check. runIfOpen can then start a provider install that overlaps the task. Do not discard near-due tasks solely because they repeat frequently. Coordinate the task with the install if frequent tasks must remain eligible for an update window.

Removed the frequent-interval exemption. Every enabled task due within five minutes now keeps the window closed. In the lab, the ten-minute task environment deferred updates while its imminent task ran, then updated when the next run was outside the lookahead.

@maria-rcks Thanks for the fix. Removing the exemption addresses this finding. Your lab result confirms that the ten-minute task blocks updates while imminent or running, then permits updates outside the five-minute lookahead.

Provide ProviderMaintenanceAdmission once from the application layer.

UpdateWindow.layer, ProviderMaintenanceRunner.layer, and ProviderTurnStartService.layer each provide admissionLayer locally. The current server build shares the layer reference, but an independently built graph creates a separate Semaphore.make(1). Such graphs do not share admission.

Remove the local providers and provide admissionLayer once from the application-owned root layer that builds these consumers.

Suggested fix
diff
1-export const layer = Layer.effect(UpdateWindow, make()).pipe(
2- Layer.provide(ProviderMaintenanceCoordinator.admissionLayer),
3-);
4+export const layer = Layer.effect(UpdateWindow, make());

Apply the same change to ProviderMaintenanceRunner.layer and ProviderTurnStartService.layer, then provide ProviderMaintenanceCoordinator.admissionLayer once at the root.

Note

Written by gpt-6.1-sol on behalf of Maria

The production server builds these consumers once in runServer via Layer.launch(layerMakeServer). All three providers reference the same exported admissionLayer, and Effect 4.0.2 propagates the same MemoMap to Layer.provide dependencies. WebSocket handlers receive the server-owned maintenance runner via Layer.succeed at ws.ts:3198; they do not create another semaphore. Independent application graphs would still allocate separate semaphores if provision moved to each root. No current runtime path builds these consumers independently, so no change is needed. Independently rechecked by a final source reviewer on eb9277c4ff.

@maria-rcks, thanks for clarifying. I withdraw this finding.

The shared admissionLayer reference uses the same MemoMap within the production layer build, so the three consumers share one semaphore. WebSocket handlers receive the server-owned maintenance runner rather than construct another instance. Moving provisioning to separate application roots would not make those roots share a semaphore.

No change is needed for this finding.

5Learning: In pingdotgg/t3code, the production server builds ProviderMaintenanceAdmission consumers together through Layer.launch(layerMakeServer) in apps/server/src/server.ts. UpdateWindow.layer, ProviderMaintenanceRunner.layer, and ProviderTurnStartService.layer reference the same exported admissionLayer from apps/server/src/provider/providerMaintenanceCommandCoordinator.ts. Effect propagates the shared MemoMap through Layer.provide dependencies, so these consumers share one semaphore within that build. apps/server/src/ws.ts supplies the server-owned ProviderMaintenanceRunner through Layer.succeed. Local provisioning of a shared layer reference does not by itself imply duplicate service instances; verify build and MemoMap boundaries before flagging it.

Keeping this as a wait, matching provider turn starts. The permit is a single process-wide semaphore, so holding it for the whole generation would serialize every title, commit, PR, and branch generation across all threads. The overlap the wait leaves open needs an install to begin while a generation is running. That can't happen in practice: an install only starts inside the update window, and the window stays closed for 15 minutes after any client interaction. Commit, PR, and branch generation follow a click, and title generation comes with a turn start, which already keeps the window closed.

3On Windows, `installChildrenAreAlive` returns `false` when the installer exits after spawning a still-running grandchild, so the gate and install lease can be released while that helper is still writing. The scan rebuilds `descendants` from the supervisor PID, but `Win32_Process.ParentProcessId` can point to the already-terminated intermediate process, leaving the helper unlinked; retain descendant identities while ancestors are alive or use lifetime tracking.

If the server is killed after the detached installer starts but before recordChildPid(pid) completes, the lock still names the dead server, so another server can start a second installer while the first is modifying the installation. The installer can outlive its parent because it is detached; gate its installation work until lock ownership is durably transferred to a surviving process.

The parent-to-installer PID handoff leaves a crash window that breaks the cross-server lock. acquire writes only process.pid; the detached installer is already running when runDetachedInstall calls recordChildPid (after awaiting its spawn event). If the server is killed between spawning the installer and completing this write, the file still names the dead server. Another server's isHeld therefore reports it free and can start a second installer or launch the CLI while the surviving installer is writing it. The surviving installer/supervisor needs to own the lock before it starts modifying the installation, rather than relying on a later write by the server.

Not changing this. The window is the spawn event plus one small file write, and a second installer also needs another server to try the same install target inside that window, right as this server is SIGKILLed. Closing it fully needs the installer itself to own the lock before it runs, which is the supervisor process this PR removed to keep the design small. A server killed in that interval is the accepted gap.

Leaving this as is on purpose. A missing directory or lock file already counts as free, so this path only handles a lock directory that cannot be read at all (permissions, a broken cache mount). Keeping admission closed there would block every turn start on the server forever, with no way out short of fixing the filesystem. Letting the turn start is the smaller failure: a background install also needs that directory, so it cannot be running from this server in that state.

Fixed in 0483ac0e74. Reclaiming a stale lock now takes a .reclaim sentinel created with wx, rechecks the lock under it, then removes and recreates it. A sentinel older than 10 s counts as abandoned. A test races two servers on a lock holding a dead pid and checks exactly one runs.

Two servers can both acquire the reclaim sentinel and reclaim the same lock, allowing concurrent installers for one target. When both stat an expired .reclaim file, the unconditional removal here lets the second server delete the first server's fresh sentinel; both exclusive creates can then succeed. Use an ownership-safe stale-sentinel reclaim protocol so only one server can proceed.

Leaving this as is. Mobile reports recentlyInteracted: active, and active is only true while the app is open in the foreground. When the phone locks or the app is backgrounded, it reports false, and the 15-minute idle window starts. Treating an open, foregrounded app as presence is intended: the background updater should not swap a provider CLI while someone has T3 Code in front of them and may start a turn. Web reports real input interaction, so desktop and browser idle detection is finer-grained.
