Problem

An initial generic title remains generic after the agent supplies useful context. Waiting for that run must not overwrite a manual title or retain an unbounded listener.

Change

Queue one existing full-context title-generation effect after the matching successful run. Reconcile already-completed runs, atomically guard the expected title, and release the waiter on manual edits, newer requests, terminal failure, archive, deletion, expiry, or scope close.

Rebuilt from @t3dotgg’s #15040, keeping this layer to one concern. Six original claims are already fixed on the pinned main, including configured-home session forking. Those implementations are preserved.

Stack: #21969 → #21970 → #21971 → #21972 → #21974 → #21975 → #21976

Scope and approval

Maintainer takeover authorized for batch scheduler-tasks-05. Layer 7 of seven; coordinator owns landing and approval decisions. This is a focused correction to existing behavior.

Verification

Title suite: 25 passed, covering completion before/after initial generation, one refinement, manual-title races, exact read/dispatch CAS, replay, and waiter cleanup.

Fresh post-fix verification: workflow/transcript 8, selected ACP 11, complete launch suite 87: 106 distinct passes, independently reproduced by Opus. Four previously failing regressions now pass. Earlier unaffected checks are reused; the historical 422 count is not a fresh run. Full server TS7 found zero errors and one fixture warning (exit 2); the warning was corrected and its scoped fixture/import check passed with zero warnings. Targeted lint and diff checks pass.

R5 Opus and Sol cleared all seven layers and the cumulative diff. Four completed parallel rounds: R1, R3, R4, R5; R2 was cancelled before verdict. Current head-specific before/after evidence and limits.

This layer: e23910a, based on bf59077. CI will receive one delayed check; unfinished checks are handed to the coordinator.

Headless Linux, Node 24. Native Windows, macOS, iOS, Android, packaged Electron, real account-based provider execution, and relay/tunnel connections were not verified. No UI behavior changed.

Implementation: Codex gpt-6.1-sol; tests: Codex gpt-6-astra; review: Claude claude-opus-5-5 and Codex gpt-6-sol, through T3 Code.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This change adds an asynchronous, event-driven second title-generation workflow after generic initial titles, including timeout, cancellation, race protection, and an additional model call. Although the contract addition is backward-compatible and tests cover many races, the production behavior and lifecycle complexity merit human review.

📝 Walkthrough

Initial title generation can now schedule a background refinement after the associated run completes. The refinement observes run and thread events and requests a metadata update only when the title still matches its expected value.

Changes: Thread Title Refinement

Layer / File(s)Summary
Conditional metadata update
packages/contracts/src/orchestrationV2.ts, apps/server/src/orchestration-v2/Orchestrator.ts
The metadata update command accepts an optional, trimmed, non-empty expectedTitle. The orchestrator rejects the update when that value differs from the current thread title.
Background title refinement
apps/server/src/orchestration-v2/ThreadTitleRegenerationService.ts, apps/server/src/orchestration-v2/ThreadTitleRegenerationService.test.ts
Qualifying initial titles schedule refinement after the associated run completes. The service observes run and thread events, cancels under specified thread or run conditions, and dispatches a conditional update. Tests cover manual title changes, replayed completion, and lifecycle cancellation.

Merge Risk: 🔵 Low · up to c0e1f

This change refines a generic thread title after the first successful run without overwriting manual edits. One narrow gap remains. If the generated title has leading or trailing whitespace, the refinement may be skipped silently, leaving the initial title in place. Sending the trimmed title is a one-line fix. The change is otherwise safe to merge.

Store the same title that the refinement waiter expects.

generatedTitle is trimmed before the refinement stores it as expectedTitle, but the completion path sends the original result.title. The regeneration-complete branch stores that value without normalization. A title with surrounding whitespace can therefore cancel the refinement, and the later expectedTitle guard can reject the update.

Send generatedTitle to complete.

Verified against every successful current title producer; this is a false positive for the current code. TextGenerationOperations.fromRunner returns sanitizeThreadTitle(generated.title), and Muse uses the same sanitizer. Managed Codex and OpenCode delegate to sanitized implementations; generic ACP title generation is unsupported. The sanitizer trims, unquotes and collapses whitespace before this service receives the title. Existing provider normalization controls pass. Both independent R5 reviewers confirmed this, so no artificial spaced-stub regression or speculative change was added.

@juliusmarminge Thanks for the clarification. The successful title producers sanitize titles before ThreadTitleRegenerationService receives them. My finding missed that upstream normalization, so the whitespace mismatch does not apply. I withdraw the finding. No code change or spaced-stub regression test is needed.

5Learning: In pingdotgg/t3code, successful thread-title producers normalize titles before apps/server/src/orchestration-v2/ThreadTitleRegenerationService.ts receives them. TextGenerationOperations.fromRunner in packages/provider-core/src/server/textGenerationOperations.ts and Muse in packages/provider-muse/src/server/textGeneration.ts use sanitizeThreadTitle, which trims, unquotes, and collapses whitespace. Managed Codex and OpenCode delegate to sanitized implementations; generic ACP title generation is unsupported. Do not flag result.title versus result.title.trim() as a refinement expectedTitle mismatch without evidence of a successful producer that bypasses normalization.
