Route untracked receipts by the current native thread.
emitUntrackedTaskReceipt emits the opening input.threadId, but a forked native thread can belong to turnInput.threadId. ProviderEventIngestor then searches the wrong thread and drops the receipt, leaving the durable task running.
Use lastTurnRouteByNativeThread with the current live native thread:

@juliusmarminge Thanks for the clarification. My finding assumed cross-thread reuse of a single Claude provider session, which the supported fork path does not permit. The target-specific session makes the captured input.threadId correct. I withdraw the finding; no routing change is needed.

| 5 | Learning: In pingdotgg/t3code, ClaudeAdapterV2.ts declares supportsMultipleProviderThreadsPerSession: false. On the supported orchestration fork path, ProviderTurnStartService opens a target-specific provider session with the target app thread before calling forkThread with the same target. Therefore, the opening input.threadId in apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts remains the correct app thread for untracked task receipts on that path. Do not infer cross-thread single-session reuse from the adapter's forkThread API alone. |

Verified the actual fork path rather than a cross-thread single-session fixture. Claude declares supportsMultipleProviderThreadsPerSession: false, so the orchestrator allocates a target-specific session. ProviderTurnStartService opens it with the target app thread before calling forkThread with that same target. An existing actual-adapter/orchestrator fork replay, strengthened temporarily to assert distinct source and target provider session IDs, passed. The captured opening thread therefore is the current target thread on this supported path. Resolving as a false positive; no routing change is needed.

Filter registered transcript notifications before routing.
Each synthetic transcript receipt creates a new SDK message. The WeakSet<SDKMessage> guard therefore does not stop a same-ID raw notification or replayed transcript receipt from applying another terminal update. A duplicate can cancel a task after task_started reopens it.
Keep opaque notifications on their existing route. Their handler records wake reports, updates the background-task roster, and clears replay tombstones, while wasBackgroundTask already prevents the native subagent update. Filter only parsed notifications for registered task IDs.

Add a regression test that delivers synthetic and raw or replayed notifications with the same registered task ID. Keep coverage for opaque notification routing.

β Confirmed as addressed by @juliusmarminge
Fixed in 7d26aae48c. Parsed transcript outcomes for already registered subagents are skipped before synthesized lifecycle routing. Their raw SDK lifecycle remains authoritative; unregistered receipts and opaque shell wake/roster handling remain unchanged, and the original source frame still reaches echo/goal/wake processing. Extended the existing source-frame case without adding a test: raw stop, authoritative reopen, and two XML replays previously emitted two extra cancellations; the corrected sequence stays running until a new raw completion. All 448 focused tests and server typecheck/lint/format pass. Both independent full-stack reviewers found no further issues. The original CI and bot verdicts predate this push; fresh CI is pending.
@juliusmarminge The supplied code confirms that registered task IDs are skipped before synthesized lifecycle routing. Raw SDK lifecycle handling remains unchanged. Unregistered receipts, opaque shell handling, and source-frame processing retain their existing paths.
The extended regression sequence covers the stale XML replay after an authoritative reopen. This addresses the finding. I have not independently run the reported checks.
