f14e168Resolved by Julius Marmingefeat: a running turn with no agent activity for 10 minutes is flagged#27254
Problem
A frozen agent looks exactly like a busy one. During the incident every thread said Working for hours while nothing arrived from any provider.
Fix
- Server.
ProviderSessionManagerkeeps one in-memory timestamp per running turn:- set when the turn starts;
- touched on every provider event, including command output deltas (events for provider threads it doesn't track, such as native subagents, touch every turn in the session);
- touched only for its own thread when a shared-session event names one (plans, runtime requests), so a busy sibling cannot hide a frozen thread;
- dropped at
turn.terminal, when the thread is detached from its session (Force stop on a shared session), or when the session is released.
- Quiet runs. A
Stream.shared check every 30 s, which runs only while a client listens, emits the threads quiet for 10 minutes or more, and only when that set changes. Cost per event is oneMapwrite; no database, projection or shell writes. - Transport. Clients opt in with
quietRuns: true(capabilityquietRuns) and getquietRunsUpdatedon the config stream, like memory pressure. - Waiting is not quiet. A thread waiting on an approval, a question, or another thread (
t3_thread_wait) is never flagged; the shared selectors decide this for web and mobile, lists and detail views alike. - Wording. Neutral: "No activity for 12m — A long tool may still be running, or the agent may be stuck." Tools legitimately run that long, so nothing is called stalled or broken.
- Web.
- The thread shows that line with Stop and Force stop.
- The sidebar row says "No activity 12m" in the warning tone instead of "Working".
- Each row selects only its own entry, so an update re-renders just that row.
- Mobile.
- The floating status shows "No activity for 12m"; its action offers Stop or Force stop.
- The thread list row is marked the same way.
Verification
- Tests:
- The session manager flags a started turn after 10 minutes of test-clock time and clears it once the provider sends an event.
- The config projection carries quiet runs across a reconnect snapshot and clears them when empty.
- A sibling's plan update in a shared session doesn't refresh a quiet thread, and detaching the thread drops it.
- The selectors ignore threads waiting on approval, input, a subagent request, or another thread.
vp run check:changedpasses for the stack.
Evidence (web)
For the capture only, the threshold was lowered to 30 s in an uncommitted patch. The Replay agent was frozen with SIGSTOP; nothing else was simulated.
| Before (main): a frozen agent is indistinguishable from a working one | After: thread and sidebar both flag the quiet turn |
|---|---|
| Before: main shows Working 1m with no hint that the agent sent nothing | After: No activity for 58s with Stop and Force stop in the thread, and No activity 57s on the sidebar row |
Limits
- Mobile is not captured; Metro on nucbox-1 was ruled out while it hosts the live server.
- The timestamps are in memory, so after a server restart a turn is flagged only once it has been quiet for 10 minutes again.

Approvability
Verdict: Needs Human Approval
Macroscope's review found this PR requires human approval — Web and mobile now automatically flag turns after ten minutes without provider activity and offer Stop/Force stop, without automatically terminating anything. Human review must confirm this default warning threshold and presentation, since legitimate long-running tools can also trigger it.

Valid, fixed in 363108d3e0. The quiet-turn dialog's Stop now sends the quiet run's id (runtime.activeRunId) to interruptTurn, and Force stop sends it too, so a confirmation left open cannot interrupt or kill a newer turn.
This fixes the identified-run case, but activeRunId can be absent while the quiet status is shown. In that case Stop still sends only threadId, so a stale confirmation can interrupt a newer turn.
Right, activeRunId omits silent runs. Fixed in 3bab8fd036 by taking the run from the server: each quiet run now carries its runId, which the session manager records at turn start (startTurn and compactThread both receive it), so it is always present. Web and mobile send that id with both Stop and Force stop, so a confirmation left open cannot reach a newer turn. The quiet-runs test now also asserts that the reported run is the newer one after an older run's late terminal.
🟠 High state/threadExecution.ts:318

Valid, fixed in 01e58461e0. When no run is active, presentRunStop falls back to the latest run that is not queued, matching the server. New table row: a settled run whose background work ignored Stop, with a queued follow-up, still shows Force stop.
The fix in 01e58461e0 addresses the finding. I couldn't verify that commit on the branch snapshot available to me.
Same here: 01e58461e0 was later rebased. The fix is in the Layer 1 head c39c31a66b and in this branch's head 919f2f5901 (threadExecution.ts: the fallback skips runs with status === "queued").
At 919f2f5901, the version I can inspect still selects latest without a status filter. Could you check whether the fix was included in that commit?
It is in 919f2f5901 on GitHub. packages/client-runtime/src/state/threadExecution.ts line 320 at that commit reads if (candidate.status !== "queued" && candidate.ordinal > (latest?.ordinal ?? -1)) latest = candidate;, under the comment "Queued follow-ups never ran, so they hold no Stop of their own." Valid, fixed in 527a799ed3. The quiet-turn Stop now awaits interruptThreadTurn (still pinned to quietRunId) and shows "Could not stop the turn" with the error for any failure other than an interruption.
The current branch still discards the result, so failures remain silent. I couldn't find 527a799ed3 on the remote; could it have been lost in a force-push?
That SHA was replaced by later rebases of the stack (the layers below changed, so every layer was re-pushed). The fix is on the current head, 919f2f5901: ThreadDetailScreen.tsx defines stopQuietRun, which awaits interruptThreadTurn with the quiet run's id and shows "Could not stop the turn" for any failure other than an interruption. The dialog's Stop calls it (onPress: () => void stopQuietRun()).
🟠 High orchestration-v2/ProviderSessionManager.ts:2467

Valid, fixed in 01e58461e0. otherWorkingThreads now reads each attached thread's runs (preparing/starting/running/waiting) rather than the shell's activeRunId, which omits silent runs such as idle compaction, and still counts pending background tasks. Unreadable threads still count as working. There is no dedicated test: reproducing a silent idle-compaction run needs the idle-compaction prompt message and its turn items in the projection, and the change is a predicate on the raw run rows.

Valid, fixed in 88ab3df1a2. stopQuietRun now uses the same statusRequest pending state as the other status actions: it returns early while a request is pending, sets the state, and clears it once the request settles. The quiet action is also disabled and labelled "Force stopping..." while forceStoppingKey matches the thread, so neither Stop nor Force stop can be sent twice.

The second publish call (provider_thread.updated) offers the event. It then runs runHead on an ended queue, which returns immediately. The comment "Observing the event ensures the manager handled it first" is false for that call. The following TestClock.adjust("30 seconds") can then run before the manager touches runActivity. This makes the expected [] emission order-dependent.
Subscribe inside publish so each call has its own live subscription. This also satisfies the rule to await a specific event, not timing.

✅ Confirmed as addressed by @juliusmarminge
Valid, fixed in 88ab3df1a2. publish now opens a fresh subscribeEvents for each call, because runHead ends the subscription. The second event (provider_thread.updated) is now observed before TestClock.adjust runs, so the expected emissions no longer depend on scheduling.
@juliusmarminge The supplied code confirms that publish creates a fresh subscription for each call. It now awaits provider_thread.updated before advancing TestClock, which addresses this finding.

@coderabbitai Both outside-diff findings from your review of 6611a2c1e8 were valid. They're fixed in 78f0e40682, each with a test that failed first.
- Stale terminal event refreshing the newer run (
ProviderSessionManager.ts).eventActivityTargetnow passes therunOrdinalfromturn.terminal, andtouchRunActivityrefreshes a tracked run only when the ordinals match. A late terminal from run 1 still marks its own turn idle, but no longer refreshes run 2'slastEventMs. Test: the existing "flags a quiet startTurn/compactThread, despite an older run's late terminal" case now delivers the stale terminal one minute before the threshold. Before the fix, run 2 was never flagged and the test timed out; now it's flagged on time. - Match quiet state to the active run (
threadExecution.ts).quietRunSinceandquietRunIdnow take the thread'sruntime.activeRunIdand return an entry only when its run is still the active one. A follow-up that starts before the next 30 s refresh therefore hides the notice and its actions right away. A follow-up that is only queued leaves the quiet run active, so the notice stays. Every caller passes the active run: web thread view, web sidebar row, mobile thread screen and mobile thread list. The selectors still return primitives, so each row re-renders only when its own value changes. A new selector test covers the matching run, a newer run, and no active run.

@juliusmarminge Thanks. Source inspection confirms both fixes:
turn.terminalcarriesrunOrdinalinto activity tracking. A mismatched ordinal does not refresh the newer run.- Both quiet-run selectors match
runtime.activeRunId. All four web and mobile caller groups pass that ID.
The regression tests cover the stale terminal for both startTurn and compactThread, plus selector behavior for matching, newer, and absent active runs. The active-run check also preserves the notice when a follow-up is only queued.
These changes address both findings. I inspected the test code but did not run the tests.

@coderabbitai Re your review of 78f0e40682:
- Outside-diff Minor, "Include quiet status in both row accessibility labels" (
thread-list-v2-items.tsx): valid, fixed in e7fc4c3deb. BothRowPressablevariants now share one label built from the title, "No activity" whilequietSince !== null, and "messages queued to send", joined with ", ". VoiceOver and TalkBack therefore announce the quiet state. There's no test: this is label wiring in a React Native row with no existing test harness, andcheck:changed(typecheck, lint, react compiler) passes. - Unverified additional comment, "Show the quiet duration in folder rows" (
SidebarThreadRow.tsx): leaving it as is. Folder rows have no status line, so the status takes the time slot and shows only its label for every status, "Working" included. Adding a duration only for quiet rows would make them inconsistent with the rest, and card rows already show it.
(The GitCafe→GitHub mirror is lagging behind a GitHub API rate limit, so e7fc4c3deb may take a few minutes to appear here.)

Adversarial review follow-up (f899061):
- A thread waiting on an approval, a question, a subagent request or
t3_thread_waitis no longer flagged as quiet. Before, web chat and both detail views could offer Stop or Force stop on a turn that was waiting on the user. The rule now lives in the shared selectors. plan.updatedandruntime_request.updatedrefresh only the thread they name, so a busy sibling in a shared session can't keep a frozen thread unflagged.- Detaching a thread from a shared session drops its run tracking. Force stop on a shared session gets no terminal event, so that entry used to stay in every quiet-runs payload.
