78ddc7aResolved by Julius Marminge78ddc7aResolved by Julius MarmingeOn 2026-10-29 a memory-heavy tool run pushed the agents slice on nucbox-1 into kernel memory throttling. Every provider CLI froze for 4 h 20 min with no provider events, turns stayed "running", and Stop did nothing. In code, ClaudeAdapterV2.interruptTurn awaited query.interrupt(), a control request with no timeout, and the per-thread effect lane queued every later effect behind it. Where adapters did time out (Claude's 10 s closed wait, ACP's 10 s cancel wait), the fallback only marked the turn interrupted in T3 and left the agent's processes running.
Per Julius: nothing is killed automatically. The user decides, with a clear UI.
Every interrupt is bounded. All callers reach adapters through ProviderSessionManager's decorated runtime, so a single 15 s bound there covers Claude, Codex, ACP (Cursor, Grok, Pi, Muse, registry agents), OpenCode and Antigravity, plus the detach and restart paths. It is set just past the adapters' own 10 s deadlines so they settle first whenever the provider answers.
Claude's and ACP's 10 s fallbacks no longer fake a stop. They now return ProviderAdapterInterruptTimeoutError and leave the turn open instead of finalizing it while the agent may still be running.
A timed-out Stop finishes its effect instead of retrying 5× on the thread's lane. The new internal command thread.interrupt.unresponsive stamps unresponsiveAt on the run_interrupt_request item.
The Stop is visible from the moment the server commits it. On main the request item exists but the timeline hides it, and its renderer was deleted as unreachable in 67661cfbb5, so nothing showed between Stop and the result. Clients now derive the stop state from the request/result pair (presentRunStop): "Stopping" until the turn really ends, or "Agent not responding to Stop · Force stop" once the server flags it. ACP agents that confirm the cancel before the turn ends keep showing Stopping while they wind down.
Force stop (orchestration.forceStopThread, operate scope, threadForceStop capability):
cgroup.kill, which works on throttled processes, falling back to systemctl kill -s KILL.thread.background-work.settle with the reason "Force stopped by user".It is a direct service call rather than an outbox effect because effects are serialized per thread, so a hung interrupt would queue it.
Codex and OpenCode run one provider session per provider instance, serving every thread on it. AgentScope never tracks those scopes per thread, so kill cannot reach them. Force stop asks the session manager which other attached threads have an active run:
A shared process is therefore never killed while other threads work in it, and never without the user knowing why it kept running. Force stop is not exposed as an agent MCP tool: killing processes stays a user decision.
unresponsiveAt, settles with the Force stop reason, and ignores a late flag.AgentScope.kill writes cgroup.kill for the thread's own scope and never touches a shared Codex scope.forceStopThread releases the thread's own session, and only detaches when another working thread shares the process.presentRunStop table.vp run check:changed passes: format, lint, knip, typecheck (32 packages), tests, React compiler.SIGSTOP mid-turn and pressed Stop. Stopping appeared within a second, "not responding" at about 10 s, and Force stop killed the frozen process through its scope. The other thread's agent in the same agents slice kept running. Server log: AcpProcessExitedError: ACP process exited after signal SIGKILL.| Before (main): ACP's fallback says "Run interrupted" while the frozen agent is still alive in its scope | After: Stopping, shown as soon as Stop is accepted |

Provider Stop timeouts are recorded as unresponsive requests. The change adds a permission-gated Force stop operation that can terminate an agent scope, settle a run, and release or detach provider sessions. Web and mobile clients display Stop status and available Force stop controls.

c7f2eStop on ACP-based agents now returns after the bounded wait, and Force stop stays available when the agent does not respond. Several earlier concerns still need resolving before merge: the Force stop released result can precede session teardown and hide a failed teardown; the shared-session kill decision may miss sibling background work; and the mobile Force stop controls can be hidden by pending prompts or confirm against a stale run.

Valid, fixed in 57586736ce. The staleness check now compares against the run Force stop acts on: the active run, else the latest that started. A queued follow-up with a higher ordinal no longer causes a rejection or becomes the target. Every Force stop test row now includes a queued follow-up.

Valid, fixed in 01e58461e0, but not with the suggested diff: orchestrator.dispatch takes the same per-thread lock and the keyed semaphore is not reentrant, so wrapping all of forceStopThread would deadlock on the settlement dispatch. Instead, re-reading the thread, the superseded check and agentScope.kill now run inside threadDispatch.withLock(threadId), so no new run can be created between the check and the kill. The settlement dispatch runs after the lock is released. A queued run cannot start its provider turn before the stuck run ends, which happens only through this settlement.
The fix in 01e58461e0 addresses the race, but I couldn’t find it on the current branch head (da85d70f6f): the check and kill are still unlocked there. Could you check whether the fix was included in the latest push?
It is included; da85d70f6f is stale. The branch was re-pushed after later review fixes, and the current head is c39c31a66b. In ThreadManagementService.ts there, forceStopThread re-reads the thread records and runs the superseded check and agentScope.kill inside threadDispatch.withLock(input.threadId, ...) (around line 1099). The settlement dispatch runs after the lock is released, because orchestrator.dispatch takes that same lock.

Valid, fixed in bd6d2842ea. releaseUnlessBusy captures the live entry before releasing and reports it released when that same entry is no longer installed (identity, not has). A replacement opened under the same id while the scope closes no longer reads as "kept", so Force stop does not wrongly report shared and detach.
