Problem

Four unanswered provider control requests could occupy all four effect workers indefinitely. Sending a message to Claude then left its start and checkpoints queued behind the stalled Codex provider.

Change

Bound provider control calls to two minutes, with a twelve-minute executor backstop for provider turn effects. Exhausted starts and restarts settle their runs through the existing ownership-checked failure path, including when the backstop fires. An unanswered turn start releases its session before failure is published. Steering with uncertain acceptance fails without retry or follow-up delivery. OpenCode compaction runs in its session scope so the control deadline does not limit healthy compaction duration.

Scope and approval

Fixes #16636

Triaged bug via the automated issue flow; GitCafe report #16636.

Verification

  • vp test run apps/server/src/orchestration-v2/EffectWorker.test.ts apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts apps/server/src/orchestration-v2/RunExecutionService.test.ts: 94 passed on the final revision.
  • The unchanged OpenCode adapter passed all 32 adapter tests, including delayed compaction; control-service and recovery coverage passed in the earlier 138-test focused run.
  • Server and OpenCode package TypeScript checks passed. Targeted lint has only existing unused-variable warnings; git diff --check passed.
  • Two independent reviews of the full fix and a fresh audit of the backstop settlement fix returned MERGE.
  • Server-only change: visual fixture matrix is empty. No client rendering paths changed.

Backstop regression red/green output, exact commands, typecheck and lint.

Evidence

BeforeAfter
Main runtime transcript, main 22840ab943: four Codex starts running beyond their leases, Claude pendingFinal runtime transcript, 6349b26ccc: Claude started at two minutes and completed with its checkpoint; all four Codex runs failed after five bounded attempts. Backend process identity stayed unchanged.

Executed starvation regression: Claude never starts after fifteen simulated minutes on main. The fixed test passes. Both final-attempt backstop settlement cases and the OpenCode delayed-compaction case also failed before their fixes and pass afterward.

Final evidence index and reproduction environment.

Verified on Linux using a real Codex CLI with a request-dropping proxy and real Claude. The original Windows app-server resource exhaustion was not induced.

Models and harnesses: GPT-6.1 Sol via Codex; Claude Opus 5.5 via Claude Code, orchestrated in T3 Code.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval β€” This change alters production orchestration and provider lifecycle semantics across several services, including timeout-driven failure, session release, retry suppression, and asynchronous compaction. An unresolved high-severity finding also concerns final-timeout run settlement.

πŸ“ Walkthrough

Provider control operations now have timeouts. Timed-out starts can trigger run containment or failure settlement, and timed-out steer effects are terminal rather than retried. Tests cover startup, control operations, worker retry behavior, and delayed OpenCode compaction.

RunExecutionService applies a two-minute timeout to startTurn. If the start times out, it runs the containment effect before failure handling. Provider recovery, session opening, thread loading, and thread resumption also have timeouts. ProviderTurnStartServiceV2.start accepts failBeforeStart to fail a run before provider startup. The OpenCode test verifies that a pending summarize operation can complete after turn start.
Bound interrupt and steer operations
apps/server/src/orchestration-v2/ProviderTurnControlService.ts, apps/server/src/orchestration-v2/ProviderTurnControlService.test.ts
Interrupt, restart, and steer provider calls now use PROVIDER_CONTROL_TIMEOUT. A steer timeout sets turnCompleted to false. Tests cover interrupt and steer timeouts.
Apply worker deadlines and settle failures
apps/server/src/orchestration-v2/EffectWorker.ts, apps/server/src/orchestration-v2/EffectWorker.test.ts
Provider-turn start, interrupt, steer, and restart effects receive a 12-minute worker timeout. A timed-out steer is marked failed rather than retried. Final-attempt start and restart failures can fail the run before startup.

Explanation

The description covers the problem, change, linked issue and scope, and detailed verification results. It also addresses the template’s visual-evidence requirement by stating that no client rendering paths changed.

Full details: Linked Issues check

Explanation

Issue #16636 requires stalled provider effects to fail or time out without blocking work for another provider. The PR adds two-minute deadlines to provider control calls and a 12-minute backstop for start, interrupt, steer, and restart effects. The executor settles exhausted final start and restart attempts through the failure path. An unanswered start releases its session before publishing failure, and a steer timeout is not retried. The added tests cover worker availability, timeout handling, and final-attempt settlement. The author reports 94 passing tests on the final revision.

Full details: Out of Scope Changes check

Explanation

The server orchestration changes and tests directly address issue #16636. The OpenCode adapter change only updates compaction comments and adds a test that verifies healthy compaction can continue beyond the start-control deadline. This supports the timeout behavior and does not add unrelated runtime behavior.

  • fix(server): a timed-out final turn start no longer leaves its run starting6349b26

Fixed in 6349b26ccc. The executor now catches its backstop timeout and settles final start/restart attempts through the existing ownership-checked failure path. Both final-attempt regression cases failed before the fix; all 94 scoped tests pass.
