fix(server): a timed-out final turn start no longer leaves its run starting

The 12-minute provider turn control backstop ran in the worker, outside the
executor, so the timeout interrupted the start/restart before it could settle
its run. Move the backstop into the executor and, on the final attempt of a
start or restart, fail the run that is still starting. Retries and worker
fail handling are unchanged.
