Claude background subagents could be denied tool permissions as soon as their root turn ended, before the normal permission policy ran. This allows callbacks from running children owned by the latest settled run, if it completed, to use the existing approval flow. The callback must still belong to the current live query; stopped, aborted, stale-process, mismatched-agent and unrelated requests remain denied.

Credit to @ScottN-PV for the original fix in #20347. This maintainer takeover preserves the contributor commits and merges main. Independent implementation review retained the small scoped fallback, replaced the polling-heavy tests with four explicit lifecycle cases, and removed one redundant condition. Unknown settled child calls attach approval to the root without fabricating a dynamic tool node.

Validation at 83aa2eb:

  • Original 11 added test cases β†’ 4 lifecycle cases in the existing adapter suite. Exact-main negative control: 4 fail / 286 pass; final adapter: 290/290 pass. Adapter plus orchestration replay: 434 pass before the behavior-neutral condition cleanup.
  • Final scoped server typecheck, lint and format pass; lint retains one existing unused layer warning outside this diff.
  • Actual SDK 0.3.276 / Claude CLI 2.1.303 native background Agent: after root terminal, Read approval accepted and Write approval declined, marker absent, child continuation detected.
  • Actual completed foreground Agent resumed through native SendMessage: the resuming root ended, then the child received both approvals with the same accept/decline results.
  • Integrated web client on isolated worktree state: root persisted complete before the read and write requests appeared; clicking Approve allowed the read, clicking Decline prevented the write, both requests resolved and the continuation completed. These probes use deterministic loopback Anthropic responses with the production SDK, CLI, adapter, orchestration and client. They do not claim remote-model behavior.

Red/green and live verification details. Before/after behavior is established by the negative control; these captures show the final client with the root already complete.

Post-root child read awaiting approval:

Post-root child read approval

Post-root child write awaiting decline:

Post-root child write approval

The fallback deliberately excludes earlier-run children after a newer root settles. It does not widen permission policy, change contracts, or change other providers. Full-access callback behavior, missing-agentID variation and stop/restart rejection are covered by adapter tests; full-access normally bypasses callbacks and was not verified live. macOS, Windows, Electron desktop, iOS, Android, remote/relay/tunnel connections were not verified.

Two parallel adversarial review rounds (Opus and Sol); final round has no code findings. Authorization-lifecycle changes still require human approval; Macroscope's human approval gate is not bypassed.

Prepared through T3 Code / Codex by gpt-6.1-sol (implementation), gpt-6-astra (tests), with independent Claude Opus 5.5 and gpt-6-sol adversarial reviews. Unmerged.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval β€” This targeted server fix changes Claude’s production tool-permission lifecycle so background subagent requests can proceed after the root turn, including creating approval requests or allowing them under the configured policy. The behavior is narrowly guarded and tested, but it affects an authorization-sensitive execution gate and merits human review.

πŸ“ Walkthrough

The Claude adapter now accepts qualifying permission callbacks from settled subagents. It passes query closure state to approval handling, assigns request artifacts to parent nodes, and adds lifecycle tests.

Changes: Claude permission callbacks

Layer / File(s)Summary
Delayed callback handling and validation
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts
The callback checks query closure state and settled subagent ownership. Approval and user-input artifacts can use an explicit parent node. Settled requests use the turn root when no tool call exists, and tool-call creation is skipped for those requests.
Settled permission tests and lifecycle validation
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
The tests cover settled subagent requests with missing agent IDs, access restrictions, cancellation, shutdown, process replacement, stale callbacks, and resumed foreground subagents.

Merge Risk: 🟑 Moderate · up to a706f

The server test suite currently fails because two new permission tests hang. A background subagent from an earlier run can still have its tool approvals denied after a later continuation completes. Some settled approvals can also appear detached from the execution tree. Fix the tests and the eligibility tracking before merging.

Security architecture risk: πŸ”΅ Low Β· up to a706f

Background tool requests can now seek approval after their parent finishes. Existing permission decisions still apply, but the origin of late requests without a child identifier is not fully established.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred β€” The new eligibility window is scoped to the owning live query and latest completed run, excluding children retained from earlier processes. Resulting actions inherit the selected run's existing tool and permission authority. End-to-end tenant, credential, and asset exposure beyond that callback flow was not established.

Security Findings and Attack Paths

  • inferred β€” The deferred authorization-bypass candidate remains unproven. At the adapter boundary, an agent-less callback can qualify through an unrelated eligible child in the same run, but qualification is not itself approval. The SDK forwards optional identity without establishing that a root-origin callback can arrive after root completion. Native producer ordering and identity guarantees are the remaining reachability gap.

Trust Boundaries and Controls

  • observed β€” Each opened query captures a distinct closure token in its permission callback. Settled fallback compares that token with the current query and rejects an already-aborted callback or a stopping query. Completed-turn context is replaced by later completed runs and removed for non-completed turns.

Resilience and Maintainability Implications

  • observed β€” Approval waiting races the decision against callback cancellation and removes the pending entry on exit. Query shutdown marks the query stopping before close. The inspected SDK suppresses duplicate in-flight control requests, aborts callbacks on control cancellation, and suppresses callback responses after cleanup. Concurrent stop/replacement during approval creation and final externally visible record cleanup remain unverified.

Hardening Proposals

  • proposed β€” Document and validate the provider guarantee for late callbacks without agent identity. If origin cannot be established, consider correlating such requests to a known child tool call before admitting them to settled-run handling. Separately validate cancellation and replacement while an approval is pending; these are proposals, not verified defects.

🎯 Functional Correctness | 🟑 Minor | ⚑ Quick win

The parent-node assertion passes when the approval node is missing.

node can be undefined if no node.updated event matches the request's nodeId. In that case the first argument is null and the second argument is undefined. Chai assert.equal compares with ==, and null == undefined is true. The test therefore passes without checking that the approval is attached to the root node, which is the behavior this PR adds. To fix it, first assert that the node exists, then compare the two values with strictEqual.

βœ… Addressed in commit dc2de97

Fixed in dc2de972c4. The test now asserts that a node.updated event exists for the request's node, then compares parentNodeId and rootNodeId with strictEqual. All five settled-request cases still pass.

Written by Claude Opus 5.5 on behalf of Scott Norteman.

@ScottN-PV Thanks for the fix. The assertion now fails if the approval node is missing and uses strictEqual to check its root attachment.

🎯 Functional Correctness | 🟠 Major | ⚑ Quick win

Remove the duplicate runtimeRequestReceipts queue and the duplicate handler.

Line 2591 declares const runtimeRequestReceipts. Line 2599 declares it again in the same Effect.gen scope, and line 2599 is not part of this change. A second const binding with the same name in one block is a SyntaxError ("Cannot redeclare block-scoped variable"). As a result, the whole test file does not type-check or load, so vp test run and typecheck fail for every test in this file.

The handler at lines 2722-2724 duplicates the handler at lines 2728-2730. If only the declaration is removed, each runtime_request.updated event goes into the queue twice. The second harness.approve(...) call would then Queue.take a stale copy of the first request instead of the new pending request.

This looks like an artifact from merging main, which already declares the queue and the handler. Remove both added blocks and keep the existing ones.

Keep permission eligibility for children of earlier completed runs.

If two background subagents start in one run and one finishes, its wake continuation can complete while the other remains running. finalizeActiveTurn then replaces settledTurnByNativeThread with the continuation. The remaining child's runId no longer matches previous.input.runId, so this check denies its next tool request. Track the owning completed context for each running child instead of comparing only with the latest completed run.

Resolve the replacement-process approval before asserting allow.

The replacement request at ClaudeAdapterV2.test.ts:8575 enters the approval path because "full-access" maps to bypassPermissions. The request waits for a deferred decision, but the test does not call harness.approve. The test therefore times out at harness.request("auditor"), not at the stopping-phase request on line 8569.

Use the root node as the parent for settled approvals.

findToolCall also searches child-thread calls. A settled approval can therefore reference a child-thread node while the approval node belongs to the root thread. Thread-scoped projection queries do not include that child parent, which can leave the approval orphaned from the execution tree.

Resolve the eligible full-access request through the approval flow.

The hanging call is harness.request("auditor"), not one of the deny cases. Full-access maps to bypassPermissions, and requiresClaudeApproval(context) remains true for that mode. The callback therefore waits for a deferred approval decision, while this test expects a direct allow and never calls harness.approve.
