🎯 Functional Correctness | 🟠 Major | ⚑ Quick win

Remove the duplicate runtimeRequestReceipts queue and the duplicate handler.

Line 2591 declares const runtimeRequestReceipts. Line 2599 declares it again in the same Effect.gen scope, and line 2599 is not part of this change. A second const binding with the same name in one block is a SyntaxError ("Cannot redeclare block-scoped variable"). As a result, the whole test file does not type-check or load, so vp test run and typecheck fail for every test in this file.

The handler at lines 2722-2724 duplicates the handler at lines 2728-2730. If only the declaration is removed, each runtime_request.updated event goes into the queue twice. The second harness.approve(...) call would then Queue.take a stale copy of the first request instead of the new pending request.

This looks like an artifact from merging main, which already declares the queue and the handler. Remove both added blocks and keep the existing ones.

🩺 Stability & Availability | 🟠 Major | πŸ—οΈ Heavy lift

Keep permission eligibility for children of earlier completed runs.

If two background subagents start in one run and one finishes, its wake continuation can complete while the other remains running. finalizeActiveTurn then replaces settledTurnByNativeThread with the continuation. The remaining child's runId no longer matches previous.input.runId, so this check denies its next tool request. Track the owning completed context for each running child instead of comparing only with the latest completed run.

8Update finalizeActiveTurn and the permission eligibility check around
9subagent.task.runId so running children retain their owning completed context,
10rather than matching only the latest settledTurnByNativeThread or
11previous.input.runId. Preserve permission eligibility for children from earlier

🎯 Functional Correctness | 🟑 Minor | ⚑ Quick win

The parent-node assertion passes when the approval node is missing.

node can be undefined if no node.updated event matches the request's nodeId. In that case the first argument is null and the second argument is undefined. Chai assert.equal compares with ==, and null == undefined is true. The test therefore passes without checking that the approval is attached to the root node, which is the behavior this PR adds. To fix it, first assert that the node exists, then compare the two values with strictEqual.

Fixed in dc2de972c4. The test now asserts that a node.updated event exists for the request's node, then compares parentNodeId and rootNodeId with strictEqual. All five settled-request cases still pass.

@ScottN-PV Thanks for the fix. The assertion now fails if the approval node is missing and uses strictEqual to check its root attachment.
