Problem

Closing an authenticated /ws connection with code 4001 and reason client heartbeat timeout left only an HTTP 101 response in the server log. Effect's RPC server consumes the socket close error, so the handler could not report why the connection ended.

Change

Observe the upgraded socket reader before RPC consumes its close error, then emit one structured close record during connection cleanup. The record includes the close code and bounded reason, connection duration, last inbound-frame age when available, and the existing client session, surface and address context. It distinguishes close frames, lost transport, session invalidation and server interruption.

This reuses only21mil's close-observation implementation, with a prototype wrapper that also supports a non-configurable request upgrade property. The final diff is limited to WebSocket close diagnostics.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/24280

GitCafe: https://git.cafe/pingdotgg/t3code/issues/24280

Triaged bug via the automated issue flow.

Verification

  • Red on main: the real /ws handler test failed with expected '[]' to include '4001' before implementation.
  • Focused tests cover exactly one structured close record and its connection fields, classification, and successful inbound-frame timestamp capture using TestClock.
  • vp run check:changed passed format, lint, both knip checks, GraphQL generation, affected typechecks and tests. Three existing unused-variable warnings remain in ws.ts.
  • Real isolated dev server on Linux with Chromium through T3 preview: a valid RPC followed by close 4001 records the reason and context; abrupt TCP loss records 1006; disposable-session revocation records session_invalidated; server shutdown records server_interrupted.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This PR is a broad, always-on server observability enhancement spanning WebSocket lifecycle handling, update logging, Linux procfs sampling, and shared tracing. It changes production runtime overhead and expands retained operational data, including connection identifiers, addresses, close reasons, and query-related attributes.

The branch now contains only WebSocket close diagnostics. Update-requester logging, Linux stall sampling, SQL attributes, and span annotations are outside this fix and have been removed. Their earlier findings no longer apply. The bounded close-frame reason remains because preserving that diagnostic is the reported bug; it is protocol close metadata, and no RPC payload or credential field is recorded. Both independent final reviews found no code blocker. Real-server proof covers close frames, abrupt loss, session revocation, and shutdown; the transcript and drivers are linked in Verification.

Carry the Stop reason into the pre-start result.

If Stop reaches a preparing or starting run without a provider turn, Line 9784 records command.reason, but this result always says "Run interrupted before provider start". The result loses the supplied reason or the user-stop default. Include that reason in this branch while retaining the pre-start context.

Name the span for the provider delete operation.

Effect.fn(function* ...) without a name does not create a span. It only captures stack frames. The sweep has a span inside sweepPreviewStage. The stage guard and the Config reads are outside it. Use a stable span name.

Filter the CNAME listing by stage prefix.

The sweep lists every CNAME in the zone, 100 per page. previewSweepTargets then keeps only this stage's records. Other stages' records are fetched and discarded, so the page count grows with all managed-endpoint CNAMEs in the zone, not with this stage's records.

Each extra page is a Cloudflare API request. A 429 on any listing page fails the destroy step, so more pages raise the chance of a failed teardown.

Cloudflare's DNS records list endpoint accepts a name.startswith filter. Add it with the stage prefix. The delete guard limits output.stage to pr-<n>, so the value needs no escaping. Keep previewSweepTargets as the safety net. The filter matches a superset of isPreviewTunnelRecordName, so it does not drop any record the sweep would delete.

Document the setup the preview workflows need.

This paragraph names the relay-preview approval environment. It does not say what the preview workflows need to run.

relay-preview-cleanup.yml reads the Cloudflare, PlanetScale, Axiom and Clerk variables and secrets. It runs in the relay-preview-cleanup environment. The nightly retry and the closed and unlabeled handlers would block on an approval gate. The workflow's header comment says teardown needs no reviewer approval.

Add one or two sentences with these facts:

  • Which variables and secrets the preview environments provide. The "Required production environment variables" list above covers only production.
  • That relay-preview-cleanup must not require reviewers.

63- Around line 122-131: Add a name.startswith filter using the stage prefix to
64the DNS-record query inside collectPages, alongside the existing CNAME and

Verified every new review finding against base 27e0c68 and head e986783. The five inline findings concern files byte-identical at base and head: stale-needs-more-info.yml (blob b7b475a6117d), markdownDocument.ts (c398325a4920), reserved-regions.ts (bb8ff981a6de), Orchestrator.ts (d2e9333c6999), and connection/platform.ts (aeb76f416593). The outside-diff pre-start Stop finding and release.md/previewTunnelSweep.ts nits likewise concern unchanged main code. Resolving these as outside this fix: the incremental review range includes main commits brought in through the history merge 72f0858. The actual main-to-head diff contains only the WebSocket helper, route, their tests, and observability documentation. The old EventLoopMonitor finding is already resolved and its latest edit confirms it was addressed. No new finding applies to the five changed files.
