Closes #23871

Problem

Pressing Stop on a thread is supposed to stop the subagents it delegated to. Since #22052, the Stop button sends thread.stop, and that path never stops the children. It still turns off their completion notifications. So a child delegated in an earlier turn keeps running after Stop, finishes later, and the parent never hears about it. That's the stall in #23871.

#16002 had put the child-stop cascade on the old Stop path (run.interrupt with holdQueue). thread.stop was internal back then, and the caller walking the tree stopped each child itself. #22052 moved the Stop button to thread.stop but didn't move the cascade with it.

Fix

  • A thread.stop with delegated children now queues the same delegated-tasks.stop effect the old Stop path queues. It goes after the interrupt, since a thread's effects run in order.
  • The server's own tree walks (stopDelegatedTasks and the MCP cancel_task tool) already stop each child themselves under their caller's permission-mode limit. They pass the new stopDelegatedTasks: false, so a child doesn't queue a second walk below itself.
  • A Stop that changes nothing still queues the cascade. Before, a command that wrote no events kept only its receipt and dropped its effects. That's the case of pressing Stop again on a thread an older server already stopped without stopping its children. EventSink.commitCommand now commits a command that has effects but no events.
  • Older clients need no change: leaving the field out means Stop also stops the children.

The issue also proposes a way to selectively resume disabled notifications after Stop. This PR doesn't add that. Stop now stops the children as the docs say ("Stop on a thread also stops the subagents it delegated to"), so nothing is left running with no way to wake the parent.

Evidence

Same steps on main and on this branch, in the web UI on Linux with a Codex thread and empty test state:

  1. Ask the agent to delegate one async child that runs sleep 600, then end its turn.
  2. Send an unrelated turn that runs sleep 120, and press Stop while it runs.

Before (main): after Stop the child is still running ("Lineage · 1 running"). pgrep on the host still showed its sleep 600 process 20 seconds later.

before

After (this branch): Stop also stops the child. It moves to "Previous agents", and pgrep on the host no longer finds its sleep 600 process.

after

Test: ThreadStop.test.ts now runs the "Stop stops the delegated tasks under the thread" case for both run.interrupt and thread.stop. The thread.stop case fails on main (expected [] to include 'delegated-tasks.stop') and passes here. It also checks that the walk queues only the one root effect. A new test, "a repeated Stop that changes nothing still stops the delegated tasks", fails without the EventSink change and passes with it. vp run check:changed passes.

Approvability

Verdict: Approved at 9dfb8f6

Macroscope's review found this PR approvable — This is a targeted server regression fix that makes thread.stop terminate its delegated tasks using the existing cancellation workflow, with safeguards for internal tree walks and coverage for repeated no-op Stops. The contract change is optional and backward-compatible, and no product-setting defaults or static-analysis overrides are changed.

📝 Walkthrough

The orchestration contract adds an option to disable automatic delegated-task stopping. Eligible thread.stop paths queue delegated-task stop effects. MCP and recursive stop dispatches disable automatic stopping when they perform an explicit delegated-task walk. Tests cover both stop entry points and repeated walks.

Stop now stops delegated child tasks again, and a repeated Stop on a thread with no new events still stops them. Two issues remain before merge. A client can pass an internal opt-out flag that leaves children running with no way to notify the parent. Stop also walks completed historical children, which can add a stray "Interrupt requested" entry to finished runs and adds avoidable work on long-lived threads.

Full details: Title check

Explanation

The title "fix(server): Stop stops a thread's subagents again" describes the main change. The PR makes thread.stop queue the delegated-tasks.stop effect, so Stop again stops delegated child tasks. The title follows the conventional commit format and is specific. It is concise and clear, and a teammate scanning history would understand the primary change.

Full details: Description check

Explanation

The description follows the template. It has Problem, Fix, Evidence, and a test description. The Problem section explains the regression since #22052 with the expected behavior and reproduction steps. The Fix section explains the changes to thread.stop, the stopDelegatedTasks flag, and EventSink. The Evidence section gives reproduction steps and before/after results on main and this branch, plus the test results. It links the issue with 'Closes #23871'. The template's 'Scope and approval' section is not filled in. The description does not link a maintainer-approved triage or discussion, and it does not explain why the fix qualifies without one. The description also does not state the model and harness, which the template asks for when an agent was used, although it names Claude Opus 5.5 in Claude Code. Because the description is mostly complete, the check passes, but the missing Scope and approval section should be added.

Full details: Linked Issues check

Explanation

Issue #23871 requires a task-scoped way to resume completion notifications after Stop, selective per-task behavior, authorization and eligibility checks, visible notification status, and automated coverage for resume and delivery cases. The PR diff adds the delegated-tasks.stop effect to thread.stop and commits effect-only commands. ThreadStop.test.ts covers the stop cascade and repeated Stop, but the supplied changes add no resume operation or MCP tool, no task_status notification status, and no tests for the required resume behavior.

Resolution

Implement the task-scoped resume command and MCP surface. Add authorization and eligibility checks, per-task suspension handling, and task_status notification status. Add tests for selective resume, results before and after resume, repeated resume, a later Stop, rejected requests, restart recovery, and status reads.

Persist effect-only Stop commands.

A Stop can produce no events and still queue delegated-tasks.stop. This branch records only the receipt and drops that effect. Narrowing the guard is not sufficient: EventSink.commitCommand currently fails when events is empty, before it calls effectOutbox.enqueue.

Add an effect-only commitCommand path that records the current sequence and enqueues the effects atomically. Then restrict the receipt-only branch to plans with no events and no effects.

The worker retries ordinary failures five times by default. A later Stop has a distinct effect ID, but the current branch drops that replacement when it also produces no events. The remaining impact is therefore a narrow terminal-failure case where the child can remain running.

Update EventSink.commitCommand so its empty-event path uses the current thread sequence, persists the receipt, and enqueues input.effects instead of failing on the missing last event.

✅ Confirmed as addressed by @shivamhwp

Fixed in 9dfb8f68bf. EventSink.commitCommand now takes a command with effects and no events: it uses the thread's latest sequence and still enqueues the effects. It still dies when there are neither events nor effects. The orchestrator only takes the receipt-only path when both are empty. New test: "a repeated Stop that changes nothing still stops the delegated tasks" fails without this and passes with it. That's also the real case of pressing Stop again on a thread an older server stopped without stopping its children.

@shivamhwp Thanks. The updated receipt-only guard and effect-only EventSink.commitCommand path address this finding. The regression test covers a repeated Stop that produces no events but must still stop delegated tasks.

You are also right about the impact: this affects threads stopped by older servers, not only effects that exhausted their retries.

Restrict stopDelegatedTasks: false to internal tree-walk callers.

The public RPC accepts this field and forwards thread.stop unchanged. A client can set it to false, which skips the delegated-task stop effect. holdStoppedThread still disposes the child completion cohort, so child work can continue without notifying its parent. Keep this override internal, or strip or reject it at the public boundary.

Valid, and known before merge. No client sends false. A client can only send it for a thread it can already stop. Moving the flag to a server-only command is a follow-up.

@shivamhwp Understood. This is a known behavior gap, not an authorization bypass. I accept deferring the server-only override to a follow-up. The finding remains open because the change is not yet implemented.

Would you like me to open a GitHub issue for the follow-up?

Skip completed app-owned tasks in the child-stop walk.

A completed app-owned task with a child thread still receives thread.stop. The child has no active run, so dispatchThreadStop takes its no-target path. That path calls markStoppedRun on the child's latest executed run, which is not live. markStoppedRun then emits a run_interrupt_request item titled "Interrupt requested" on that completed run. The first Stop can therefore add this item to a completed run. Repeated Stops are guarded by stopReachedRun, which was not inspected.

Each historical child also adds a sequential thread.stop dispatch with its own recursive walk. Long-lived threads accumulate these dispatches.

Limit the walk to tasks with active work in both places: the predicate in Orchestrator.ts and the loop in ThreadManagementService.stopDelegatedTasks. Use a status test that keeps idle held follow-ups in the walk, because Stop holds their queued runs for Resume by design.

8Update the app-owned child-task predicate in the Orchestrator child-stop walk
9and the loop in ThreadManagementService.stopDelegatedTasks to include only tasks
10with active work, while keeping idle held follow-ups eligible for the walk. Use
11the task status to distinguish active work from completed tasks so completed
12children do not receive thread.stop dispatches.

Valid, and left for a follow-up. The walk over every app-owned task was already on main for the run.interrupt holdQueue path. This PR reaches it from thread.stop too. The effect is one "Interrupt requested" item on a finished child's last run, written once, since stopReachedRun guards repeats. Skipping finished children correctly means telling them apart from idle children with held follow-ups, which Stop must keep holding. That change needs its own tests, so it is not in this fix.

@shivamhwp Understood. stopReachedRun guards against duplicate items, so repeated Stops do not add more “Interrupt requested” items to that finished run. A separate change with tests can distinguish finished children from idle children with held follow-ups.

Deferring this minor finding is reasonable for this fix. Would you like me to open a follow-up GitHub issue?

5Learning: In pingdotgg/t3code, apps/server/src/orchestration-v2/Orchestrator.ts intentionally preserves bound queued delegated follow-ups on Stop: child thread.stop uses holdStoppedThread to hold ordinary queued runs for Resume rather than cancel them. Do not flag resumability of these stopped queued follow-ups as a defect. Distinguish these tasks from orphaned app-owned tasks that never had a bound child run.

5Learning: In the TypeScript orchestration flow, `packages/client-runtime/src/operations/commands.ts` dispatches `run.interrupt` with `holdQueue: true` for Stop. In `apps/server/src/orchestration-v2/Orchestrator.ts`, this invokes `holdStoppedThread`, which clears every pull-request watch in the same command, including when interrupting a settled run with pending background work. Client-side unwatching is only needed when there is no run to interrupt. `apps/server/src/orchestration-v2/ThreadStop.test.ts` covers watch cleanup after Stop.
