fix(web): screen readers hear which background thread finished, even with the sidebar collapsed#23839

Refs #23811. Stacked on #23752, which makes the open thread announce its own status.

Unproven: NVDA, and VoiceOver on the final markup. VoiceOver was recorded on an earlier commit of this PR that still used role="status" (below); NVDA wasn't run.

Problem

Screen readers didn't hear a background thread finish, fail or ask for something when the sidebar was collapsed. A collapsed sidebar is inert, and the only live regions for those statuses were the labels on its rows. With the sidebar open you heard "Done" without the thread's name, and the wording differed from the open thread ("Pending Approval", "Input").

Fix

  • One polite live region now sits next to the sidebar, outside the inert part. For any thread except the open one it says <status>: <thread title>: "Needs approval" or "Needs your input" when it asks for something, and "Done", "Failed" or "Stopped" when a run ends.
  • The region is a plain aria-live="polite" div with no role="status". Orca 50 maps role="status" to a status bar in Chromium and drops its updates; a plain polite region is spoken. #23752 makes the same change for the open thread.
  • A run's ending is said once, keyed on the run, the way #23752 does it for the open thread. Marking a thread unread doesn't replay it, and a second run is caught even if it finishes between two updates. A second approval or question of the same kind is announced too (a subagent's requests carry no id, so back-to-back ones from a subagent are said once).
  • A queued message cancelled while another run works isn't announced as "Stopped". The shell then only reports that cancelled run, so the working run's own ending stays quiet in this case rather than being guessed. The app's completion notifications and sounds miss this ending for the same reason; fixing it means changing which run the server's thread shell reports as latest, which is out of scope here.
  • With in-app notifications on (off by default), a background completion or request is heard twice: once from the toast, which sits in a polite live region, and once from this region. Muting this region then would drop what toasts don't cover ("Stopped", a second request in the same run, anything while the window isn't focused).
  • The words come from #23752's announcer, now shared from one place.
  • A thread is quiet the first time it's seen. Its environment counts only once its shell is live, so opening the app or reconnecting doesn't read out every thread.
  • A snoozed thread stays quiet until its timer, like its sidebar row. A question or a fresh failure ends the snooze, so those still speak.
  • The sidebar row label is no longer a live region, so each change is announced once.

Mobile isn't touched; the issue is about the desktop and web client. At phone width the sidebar is a modal sheet, which likely hides this region while the sheet is open; that wasn't tested.

Evidence

Both recordings run the same script against a dev server with Claude Haiku: start a turn in "Fix the Login Redirect", switch to "Update README documentation", collapse the sidebar and wait for the turn to end; do it again with the sidebar open; then reopen "Fix the Login Redirect". The yellow banner names the build and step.

Orca 50.2 with Chrome on Linux

Orca runs on a private X screen with its own speech-dispatcher, so the audio is only Orca. The captions are Orca's own speech log, burned in at the time it spoke.

VoiceOver's caption panel, bottom left, shows "Done: Fix the Login Redirect" at 0:21 with the sidebar collapsed and at 1:00 with it open. This commit still used role="status", which VoiceOver reads; the background logic hasn't changed since.

The "changed files" cards in the recordings are me switching builds under the running dev server, not the agent.

After an Opus review, the final code was also run in a web dev build on corex with real Codex turns, logging every live-region write: a turn finishing in a background thread wrote "Done: Wait 20 Seconds", and the open-thread cases from #23752 behaved as listed there.

Tests

backgroundThreadAnnouncement.test.ts covers the wording, saying each run's ending once (no replay, a second run caught, rollbacks quiet), a repeat request of the same kind, a queued message cancelled mid-turn, staying quiet on first sight and after a resync, leaving the open thread out, snoozed threads, joining changes that land together, and subagent requests raised on the parent row. vp run check:changed passes.

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval β€” This PR adds a globally mounted screen-reader announcement workflow for background threads and changes existing sidebar announcement behavior, with substantial new production logic. Unresolved comments also identify possible stale live-region announcements, so the accessibility behavior merits human review.

I tested this live with NVDA with T3 Code in front. The main fix works: background threads are announced by name, including with the sidebar collapsed. There are some issues that need to be addressed, listed below. Tested with Claude (Opus 5.5), using a tool I developed to let the agent monitor and drive NVDA.

The NVDA column is that tool's output, numbered by utterance. Focus and navigation speech (for example "Message edit has auto complete multi line opens list") is trimmed, so gaps in the numbers are expected.

10. settledRunId updates while the thread is snoozed, so the outcome is lost rather than delayed. The sidebar shows "Woke"; announcing "Woke: Greeting" would match it.

15. ThreadActivityAnnouncer takes the cached snapshot (working) as its baseline, then the fresh one (idle, completed) reads as the run ending, so the end that BackgroundThreadAnnouncer already spoke is spoken again. Both could share which runs were already announced.

16. The removed queued run becomes latestRun (cancelled), so when the first run finishes there's nothing new to announce. The sidebar shows no "Done" either. Treating activeRunId going to null as the run ending might cover it.

17. The background region keeps its last text, and NVDA reads it again when a modal closes. Clearing it a few seconds after each write would probably avoid it.

When in-app notifications are enabled, this return permanently drops Stopped: <title> announcements for background runs that become interrupted or cancelled, even though EnvironmentNotifications emits no replacement toast for them. The memory has already advanced at line 60, so those announcements cannot be retried; consecutive approval requests in the same run are also deduplicated by the toast coordinator. Right: toasts skip "Stopped", a second request in the same run, and anything while the window is unfocused, so muting the region dropped those for good. Reverted in ac463d5fbe. With in-app notifications on, a completion is now heard twice (toast plus region) rather than risk losing one; the PR description notes it.

Review in Change Stack β†’

πŸ“ Walkthrough

The change adds background-thread announcements for pending requests and run outcomes. It tracks announcement state, filters eligible threads by environment and thread state, and renders announcements in a polite live region. The sidebar row’s status label is no longer a live region.

Clear the retained text after a delay.

This region keeps its last announcement indefinitely. NVDA can read it again when a dialog closes. See the comment on BackgroundThreadAnnouncer.tsx Line 62 for the cause and the proposed timer-based fix. Apply the same fix here. Cancel the pending clear on a newer announcement, on a threadKey change, and on unmount.

Clear the live-region text after a delay so a screen reader does not re-read stale announcements.

The region keeps its last announcement indefinitely. The PR comments report that NVDA reads the retained text again when a dialog closes. This happens while the thread is still working. The user then hears an outdated "Done" or "Needs approval" as if it were new.

Schedule a clear after the announcement has had time to be spoken. Cancel the pending clear when a newer announcement arrives. Cancel it on unmount. The trailing-space write at Line 62 still handles back-to-back identical text.

ThreadActivityAnnouncer.tsx Line 51 has the same retention behavior. Apply the same fix there.

3+ clearTimerRef.current = setTimeout(() => {

Verify the result with NVDA after you change it. The clear delay must be long enough for slow speech rates.

Based on learnings: "recommend verifying behavior with real screen readers (e.g., NVDA, JAWS, VoiceOver) rather than relying solely on automated a11y linting." and the retained-text re-announcement guidance about clearing and restoring in a later task.

NVDA results on the final markup (ac463d5fbe)

I tested the final markup with NVDA in the desktop app using claude, which used a second web client to drive the other threads and a tool developed for driving and capturing NVDA speech output. For each case I checked what NVDA actually said and the text that landed in the announcer's live region. Every case I could trigger from the UI behaved as described. Should I make this an issue and link it to this pr? A queued message cancelled while another run works isn't announced as "Stopped". Also if anything in my approach is missing or it isn't what you are looking for feel free to inform me so I can keep it in mind in the future.

The NVDA column is the tool's output, numbered by utterance. Focus and navigation speech (for example "Message edit has auto complete multi line opens list") is trimmed, so gaps in the numbers are expected. For silent cases, the tool prints the last utterance number before the step. If you rather have recordings I can do that instead.

Claude subagent approvals. The documented limitation (back-to-back subagent requests said once, since they carry no id) didn't come up with Claude. I asked a subagent to make two Bash calls in parallel. Both approvals were stored on the parent thread, each with its own request id, and the second was only raised after the first was approved (resolved 12:33:02.843, next created 12:33:03.425). So each one was a new request on the parent, and both were spoken. The id-less path, where a request lives on a child thread and surfaces on the parent row, wasn't exercised here.

Keep a live region for uncovered non-route statuses.

When a non-route thread changes to working, waiting, or limited, the plain label gives screen readers no status update. Add a live region only for those statuses. The separate Woke branch already retains its own region, and approval, input, and terminal statuses remain covered by the dedicated announcers.

8Update the `topStatus.label` rendering to expose a status live region only when
9`props.isRouteThread` is false and `status` is `working`, `waiting`, or
10`limited`; leave the label non-live for other statuses. Preserve the separate
11`Woke` region and existing coverage for approval, input, and terminal statuses.
