fix(server): ended roots no longer leave provider turns running#22846
Problem
T3 can end a run or a root turn but leave the provider turn under it running (#16557):
- After an ingestion failure, the failure path overwrote a cached terminal root-turn snapshot (
completedorfailed) asinterrupted. That snapshot might not be stored yet. - When startup recovery or a native subagent Stop ends a native child's runless root, the provider turn of that root (
runAttemptId = null) staysrunning.
Fix
- Ingestion failure (
RunExecutionService): after a successful interrupt, replay a terminal snapshot unchanged, and mark an active snapshotinterrupted. If the interrupt fails, write nothing. The stored turn staysrunning, so the next-start repair from #23169 can retry it, as on main. - Runless roots (
endRunlessRootTurns): also end active runless provider turns on the roots it ends.providerTurnsis now a required input. The startup recovery query and both Orchestrator callers (interrupt cascade, subagent Stop) now load it.
Out of scope: a runless reasoning node under a cancelled root, and backfill of rows that are already stranded.
Proof: new and updated tests in RunExecutionService, ProjectionRecovery and ThreadStop fail on main's production code and pass here (109/109 focused tests). vp run check:changed is green.

Round 1 removed a change that wrote failed when the interrupt failed. That write disabled the next-start repair. Round 1 also removed a 5s timeout that was shorter than the adapters' own interrupt limits. Round 2 stopped a terminal snapshot from replacing a running turn after a failed interrupt.

Failure finalization now selects an update for an abandoned root provider turn and preserves snapshots that already have a terminal status. Startup recovery now selects eligible runless root provider turns, and cleanup emits terminal updates for active turns attached to those roots.

Explanation
The title "fix(server): ended roots no longer leave provider turns running" matches the main change. The changeset ends provider-turn projections when a run or root ends. It covers failure finalization, the interruption fallback, terminal snapshot replay, and startup recovery of runless root turns. The title is specific and follows conventional commit format. It names the server scope and the core symptom, and it does not claim a broader change than the diff makes.
Full details: Out of Scope Changes check
Explanation
The changes are limited to provider-turn failure finalization, terminal snapshot handling, runless-root recovery selection, shared recovery event generation, related orchestration reads, and regression tests. These changes support issue
#16557. No unrelated client, dependency, wire-contract, or product changes are shown.
Full details: Description check
Explanation
The description is a near-complete match to the template. It gives a clear Problem (provider turns left
runningafter ingestion failure or runless root end, linked to#16557), a Change section that splits the ingestion-failure fix from the runless-root fix, and a Verification section with specific focused test counts (109/109) and avp run check:changedresult. Two gaps remain. The Scope and approval section has no approval link or triaged-issue reference. The issue#16557is linked in the Problem section, but the template asks for a maintainer-approved issue or discussion, and the description does not state whether approval exists. Also, the Verification section cites a 109/109 focused-test run, but it does not explain how the author verified the non-test runtime behavior, and the PR objectives say proof for several server scenarios is missing. The description itself is detailed and mostly complete, so the check passes, but the approval gap should be noted.
Full details: Linked Issues check
Explanation
Issue
#16557requires the provider turn under a root to become terminal when failure finalization ends the run or root. The runless-root recovery path now selects active provider turns andendRunlessRootTurnsemits terminal provider-turn events, while independent background work remains outside that scope. However, inRunExecutionService.ts, the ingestion-failure path maps a failed interruption tonulland emits no provider-turn event. The code comment and the reported test behavior state that the persisted turn remainsrunningso a later startup can retry it. This leaves the issue's failed-run reproduction unresolved and also does not match the PR description's failed-interruption requirement.Resolution
When provider interruption fails or reaches the cooperative timeout, include the root provider turn in the existing ownership-gated failure finalization with a terminal
failedstatus and completion time. Preserve an already-terminal snapshot when applicable. Add or update a regression test that verifies the turn is terminal after failed interruption.

Use a complete provider-turn fixture.
The cast hides missing required fields: nativeTurnRef, ordinal, startedAt, and completedAt. The finalization code spreads this fixture into provider-turn.updated, but the mocked sink and status-only assertion accept the incomplete payload. Supply a complete turn and check it with satisfies ProviderAdapter.ProviderAdapterV2Event so the regression test validates the event contract.

Fixed in 5dbc02daeb. The fixture now has nativeTurnRef, ordinal, startedAt and completedAt, and uses satisfies ProviderAdapter.ProviderAdapterV2Event in place of the cast, so the spread provider-turn.updated payload is contract-complete.
@t3dotgg The supplied file includes all four required fields and uses satisfies ProviderAdapter.ProviderAdapterV2Event. This addresses the fixture completeness finding.

3728adfResolved by Julius MarmingeAddressed the terminal-preservation finding in d7e3815173. The root snapshot is cached before publication, so failure finalization replays terminal snapshots unchanged, preserving their original status and completion time even when their own publication failed. Active snapshots still receive the failed/interrupted result, and native cleanup still runs. Added three durable-state regressions; the full service suite passed 46 tests, and final focused tests, lint and server typecheck passed. Verification.

For Claude ACP, this path enters
AcpAdapterV2.interruptTurnwithrequestRuntimeRestart: true. The adapter then waits forruntime.closeSession()insideEffect.uninterruptibleMask. The ACPsession/closerequest has no independent deadline. Effect 4.0.5 must interrupt the losing effect before a timeout returns, so neither this inner timeout nor the five-second caller timeout can complete if the agent never responds. Checked both outside-diff findings at d9e554c; neither requires a code change.
- ACP close: the caller’s five-second timeout is cooperative and may wait for the adapter’s ten-second close deadline. Effect 4.0.5 starts interruptible race fibers for that inner timeout, so an unanswered close does not hang finalization indefinitely. An independent TestClock/Deferred experiment observed the caller pending at five seconds and returning a timeout failure at ten seconds. Reproduction output. This is why the PR describes the outer timeout as cooperative.
- Starting turns: the provider-turn schema accepts pending, running, completed, interrupted, failed and cancelled. It does not accept starting. The existing predicate covers both valid active states; adding starting would handle an invalid projection shape. The recovery SQL’s wider status list does not extend the schema. Schema excerpt.
All 30 latest-head checks pass, including CodeRabbit and Macroscope. The earlier terminal-preservation finding remains fixed and resolved.

Persist a known terminal snapshot even when interruption fails.
If ingestion rejects a terminal provider_turn.updated event and interruptTurn also fails, this branch discards the cached terminal snapshot. The run then becomes failed, but the stored provider turn stays running. ProjectionStore.getRuntimeRecoveryProjection does not select attempt-bound turns under failed runs, so startup recovery cannot correct it. Replay the cached snapshot when its status is terminal; require a successful interrupt only before changing a nonterminal snapshot to interrupted. The new test at RunExecutionService.test.ts, Line 2723, currently expects the stale running status.

Not changing this. Leaving the stored turn running after a failed interrupt is intended.
- Startup recovery does not repair this row. The next start does:
ProviderTurnStartService.ts(around line 596) selectsrunningroot turns of earlierfailedruns on the same provider thread. If a session is live, it retries the interrupt. If not, it marks the turn ended. - A failed interrupt means background work started with
requestRuntimeRestart: truemay still run. Replaying acompletedsnapshot here would remove the only signal that triggers that retry. Main has kept this behavior since #23169. - The row is not visible to users:
client-runtime/src/state/threadExecution.tsreads provider-turn status only for active runs.
The test at RunExecutionService.test.ts asserts running on purpose, and it fails if the terminal snapshot is replayed after a failed interrupt.
@t3dotgg You're right. I missed the next-start cleanup in ProviderTurnStartService.ts.
The stored running status preserves the signal to retry cleanup after a failed interrupt. Replaying the cached terminal snapshot would remove that signal while background work may still be active. The test expectation is intentional.
I withdraw this finding. No change is needed.
