fix(approvals): command approvals show the command, not only the agent's description#20145

Command approvals could show the agent’s explanation in place of the command, or reconstruct pipelines from permission patterns. The cards now show the complete operation and keep the explanation in the approval item’s existing title.

Codex preserves legacy argument boundaries. Network approvals show the protocol and host followed by the complete command, with the reason kept in the title even when all three arrive together. Claude, OpenCode, OpenCode 2, and Pi retain the command rather than a description, split pipeline, or truncated JSON. OpenCode 2 fills in late tool input after reconnect without replacing authoritative permission metadata or changing a request already being answered. Web and mobile share labels; command text and explanations wrap and scroll within bounds, and decision buttons match main. Shell-wrapper removal remains conservative and preserves command whitespace.

Scope and approval: this fixes incorrect information in existing approval flows. It adds no execution authority or new approval action. The active maintainer request for large-command verification is addressed below and remains open for human re-review.

Verification:

  • Reduced 26 added cases in three files to seven added cases plus two strengthened inherited cases in seven existing files. All seven tests-phase scenarios fail on content assertions without the fix; removing the OpenCode 2 metadata guard also reproduces its regression.
  • The network follow-up strengthens the existing replay row without adding tests: host + reason + command failed on content before the fix and passes after it. After merging main at 284ce8fe2f, 541 tests in the Codex adapter, approval projection, and command-label suites pass, along with scoped server typechecking and targeted lint/formatting.
  • 955 focused tests, 245 adjacent tests, and three browser tests pass. The new browser regression fails without the explanation-height bound (1,168px instead of at most 40px). Scoped client-runtime, OpenCode, Pi, web, mobile, and server typechecks pass. Formatting and targeted lint pass, with five unchanged unused-variable warnings. One unchanged Claude scenario timed out in a combined run; its six variants passed on targeted rerun and the full focused run passed again.
  • Linux browser proof uses the real approval panel, banner layout, and decision components with synthetic data. Base and head captures use identical data and viewport. The complete 6,282-character command remains exact at 1280px and 520px; its detail area is 160px high, scrolls vertically to the end marker, and has no horizontal overflow. A separate 10,000-character unbroken token also wraps without losing text or hiding the buttons. A separate synthetic network-title stress case retains a 6,282-character explanation in a 40px scroll area; its end marker is reachable and its decision buttons remain visible. The current adapter emits the network command in the primary detail, not the title. The screenshots compare explanation display; adapter replay tests establish command-versus-description extraction.
  • This maintainer pass did not run live provider approval sessions or verify macOS, Windows/WSL, native iOS/Android, Electron, or remote/tunnel connections. Native scrolling and real-provider approval/decline remain for a human to verify. Earlier author screenshots and live-provider claims have been replaced with current-head evidence.

Before — main approval panel with the same synthetic command:

Before: main approval panel omits the explanation

After — complete command and separate explanation:

After: command approval with wrapping explanation

After — 520px viewport:

After: narrow approval card keeps decisions visible

After scrolling to the final command line:

After: final command marker is readable

Recording: scroll through the complete command and network explanation

Network follow-up — identical current-head components with the old and fixed synthetic adapter payloads. Replay tests prove the adapter emission; no live provider command was executed.

Before — host and reason hide the command:

Before: network approval has host and reason but omits the command

After — host, exact command, and reason remain visible:

After: network approval includes the complete command alongside the host and reason

Thanks to @maria-rcks for the original #20145, cross-provider investigation, and shell-display constraints. Contributor history is preserved.

Independent full-PR review: two original takeover rounds (network-title overflow fixed in round one, round two clean), then one clean post-network-fix round on both Opus and Sol. Opus noted optional Claude non-shell tool labeling, pre-existing writeStdin semantics, and native/live-provider verification limits; these are left for coordinator follow-up. The active human changes request is preserved.

Maintainer implementation: Codex gpt-6.1-sol; test reduction: Codex gpt-6-astra. Adversarial reviews: Claude Code claude-opus-5-5 and Codex gpt-6-sol.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This fixes approval presentation across Claude, Codex, OpenCode, Pi, web, and mobile by keeping the exact command separate from the agent’s explanation. The cross-provider adapter changes and reconnect-aware approval updates are broad enough to merit human review.

Walkthrough

Provider adapters now separate command prompts from explanatory titles and retain command context for approval requests. Client state carries titles and request labels. Mobile and web views format and display command details in scrollable areas.

Changes: Approval Details

Layer / File(s)Summary
Provider approval prompts and titles
apps/server/src/orchestration-v2/Adapters/*, packages/provider-opencode/src/server/*, packages/provider-pi/src/server/*
Provider adapters build prompts and optional titles from command, reason, permission, or network data. OpenCode can update a pending shell approval when command input arrives later. Tests cover adapter output, including delayed command data and long bash commands.
Pending approval state and command formatting
packages/client-runtime/src/state/threadRequests*, packages/client-runtime/src/work-log/*
Pending approvals retain titles and use shared request-kind labels. Command display formatting unwraps supported shell wrappers. Approval requests do not resolve a tool name from their free-text label. Tests cover derived approval state and command formatting.

Suggested reviewers: juliusmarminge

Merge Risk: External image Minimal · up to 6e8ad

The approval display changes have no established merge-blocking regression. The previously flagged warning styling and non-resumable display behavior were already present before this PR.

Security architecture risk: External image Low · up to 9315a

The change improves command visibility without adding authorization actions. No introduced approval bypass was established. Limited uncertainty remains about whether late command updates can replace information already presented for an unanswered approval.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • observed — The affected boundary is human authorization of provider-requested operations, including command execution and Codex network-host access. OpenCode 2 continues to answer the original native permission using its stored session and request IDs; displayed text is not substituted into that reply.

Trust Boundaries and Controls

  • observed — Provider-supplied command details and explanations are rendered as text children in the inspected web and mobile approval components. The new display path does not interpret those fields as HTML or execute them.
  • observed — OpenCode 2 marks a request as answering before provider delivery and rejects another response for an answering entry. The late-command update path respects that barrier. Delivery uses stored native identity, then forgets the request and abandons work if delivery failed.

Resilience and Maintainability Implications

  • observed — Permission-reply reconciliation looks up native request ID without also comparing session ID. The same lookup exists at the PR base, so the unresolved upstream ID-uniqueness assumption is pre-existing rather than an established regression from this change.

Hardening Proposals

  • proposed — Define which command source is authoritative throughout a permission's lifetime. If tool-input fallback is intended to become authoritative once displayed, preserve that fact so conflicting later input cannot relabel the same unanswered approval. Conflicting upstream input was not established as reachable in this review.

Explanation

Issue #20146 requires approval prompts to show enough detail for a safe decision, preferably the exact command. ClaudeAdapterV2 now uses the tool input command, cmd, or script as the approval prompt and keeps the callback title or description as the title. The web approval panel displays the command detail separately from the explanation. Claude adapter tests verify the command and title. This implements the linked issue requirement.

Full details: Out of Scope Changes check

Explanation

Issue #20146 covers the desktop Claude approval flow. The pull request also changes mobile approval rendering and provider-specific behavior for Codex, OpenCode, and Pi. The linked issue does not define requirements for those platforms or providers. The shared approval projection and web changes support the linked flow, but these additional provider and mobile changes are not established as required by #20146.

Resolution

Limit this pull request to the desktop Claude flow and shared code required by that flow. Move the mobile, Codex, OpenCode, and Pi changes to separately linked issues, or link active issues that define their requirements.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Preserve argument boundaries in legacy command approvals.

When a legacy payload.command argument contains spaces or shell syntax, join(" ") displays a different command. For example, ["printf", "%s", "hello world"] displays as printf %s hello world. This change uses that ambiguous text even when Codex supplies a reason. Render the argument vector with explicit argument boundaries, such as shell-escaped arguments or a structured argv display, before asking the user to approve it.

✅ Addressed in commit aaa6b49

Note

Written by claude-opus-5-5 on behalf of Maria

Fixed in the latest commit. Legacy execCommandApproval argv is now joined with POSIX-quoted words, so ["printf", "%s", "hello world"] shows as printf %s 'hello world'. A ["/bin/bash", "-lc", "…"] argv also unwraps cleanly in the card.

@maria-rcks Thanks for the fix. The supplied code now quotes each argument with codexShellWord before joining the argv. This preserves argument boundaries and keeps payload.reason separate as the title.

Good call. I stress tested it in a dev build with a long Codex command (a curl loop over six hosts with long headers, about 400 characters, escalated with a reason). Before, the card showed one line cut off at the Decline button, and the rest only scrolled sideways. The command now wraps, the card shows up to 8 lines, and anything longer scrolls vertically. There's no sideways scrolling at any width. The reason stays a single truncated line next to the label.

long command before: cut off at the buttons

long command after: wrapped over 8 lines

long command at 390px, dark theme

Scrolling the card to the bottom shows the end of the command ("https://$host/"; done), and the timeline still reserves space for the taller card. The mobile card uses React Native Text, which already wraps without a height cap.

Keep nonstandard executable paths visible in approvals.

/tmp/bash -c 'echo safe' matches this pattern. The approval card then shows only echo safe, although /tmp/bash can be a different executable. A provider-supplied command can therefore conceal the executable from the person approving it. Restrict unwrapping to trusted shell paths, or retain the full invocation when the executable path is not known to be a shell.

✅ Addressed in commits a91f995 to 2c0c914

Note

Written by claude-opus-5-5 on behalf of Maria

Fixed in 2c0c914cc6. Unwrapping now requires a bare shell name or one in /bin, /usr/bin, /usr/local/bin, or /opt/homebrew/bin. /tmp/bash -c 'echo safe' keeps its full invocation, and that case is now in commandLabel.test.ts.

@maria-rcks Thanks for the fix. The updated BARE_SHELL_WRAPPER restricts unwrapping to bare shell names and the listed paths. It preserves /tmp/bash -c 'echo safe' in full, which addresses this finding.

Use a readable fallback for unsupported browsers.

When a supported browser does not match the @supports branch, a dark custom --warning value produces black text on a dark button background. Use a fixed high-contrast fallback pair, while retaining the custom-color calculation inside @supports.

No longer applies: 8b5189ca5b removed the warning-solid utility and the solid warning button variant. apps/web/src/index.css now matches main, and the approval buttons use warning-outline and a new ghost-warning variant, both of which use the theme's --warning-foreground text.

@maria-rcks Confirmed. The current code has no warning-solid utility or solid warning button variant. The approval buttons use warning-outline and ghost-warning, both with text-warning-foreground. The fallback concern no longer applies. I withdraw the finding.

@juliusmarminge I stress tested pending approvals live in a dev build using these commands:

  • a 580-character && chain
  • a 1,500-character token with no spaces
  • an 830-character URL
  • a 40-line heredoc
  • a 300-character explanation

None of them overflow sideways at 1280px or 520px. Past about 10 lines the command scrolls vertically, the last characters are reachable, and the displayed text matches the full command.

1500-character token, no spaces

same token at 520px

long URL scrolled to the end

The test found two readability problems, both fixed in 57835bd. First, Codex wraps scripts that contain double quotes as /bin/bash -lc "…\"…", and the notice showed that wrapper with every \". It now shows the plain script when the escapes are only \", \\, \$ or \`. Anything with an unescaped $ or backtick still shows the whole invocation.

Second, the agent's explanation beside the label ended in an ellipsis. It now wraps.

Maintainer takeover is pushed with contributor history preserved. The current description contains refreshed Linux browser evidence for a 6,282-character command, a 10,000-character token, and the network-approval explanation path. Opus caught an unbounded explanation row; it now scrolls without truncation, and the new browser regression fails without that fix. The second full Opus and Sol review round is clean. Keep the command visible in network approvals.

When networkApprovalContext, payload.reason, and payload.command are all present, the prompt contains only the protocol and host. The title selects the reason, so the approval item omits the command. Put the host first in the prompt, then include the command. Keep the reason in the title. Add this combination to the replay test.

✅ Confirmed as addressed by @juliusmarminge

Fixed in a1bce001d2, published in 36f862a70b. Network approvals now show protocol/host followed by the complete unmodified command; the reason remains the title. The existing replay row fails on missing command text before the fix and passes after it. Post-main-merge verification: 541 focused tests and scoped server types pass; both full-PR reviewers found no blocking issue. The PR body now includes old/fixed payload screenshots from the real UI components. Human re-review remains open.

@juliusmarminge Thanks for the fix. The supplied code shows protocol/host followed by the complete, unmodified payload.command, with payload.reason kept as the title. This addresses this finding. Human re-review remains separate.

Keep the command visible when the provider exits.

If a command approval becomes not_resumable, this branch replaces its detail with the warning. derivePendingThreadRequests still retains the command, and the mobile card still displays it. Show the command and the warning separately so users can inspect the operation before restarting the run. This existing branch prevents the new web command display from covering that state.

Leaving this out of scope. The not_resumable branch is existing behavior on main, and the user can't answer that approval anyway; they need to interrupt or restart the run. This PR only changes how a live command approval displays.

@maria-rcks Thanks for clarifying. The existing not_resumable behavior is outside this PR’s scope. That approval cannot be answered, and the warning gives the required recovery action. I withdraw this finding as a requirement for this PR.

Would you like a separate follow-up issue for keeping the command visible in that state?

Fresh triage of #20146: reproduced with the real Linux web dev server and Claude Opus 5.5 in Supervised mode on main 8acbefefc5, then repeated against this branch at 0c87a3ae43.

Requested printf 'issue-20146-proof\n' > /tmp/t3-issue-20146-proof.txt with tool description “Write the triage marker”. Main displayed only the description in the approval detail. This head displays the complete command and a separate explanation. Both approvals were declined. Existing implementation and contributor history are preserved; no duplicate PR was opened.

Environment: collaborative Chromium browser, Linux, 1280 × 800 CSS pixels, DPR 2, no device emulation. These are live-provider reproduction captures; the later capture includes the previous declined turn, so they are not a pixel comparison.

Use the complete input for command-classified tools without a top-level command.

The Claude permission callback accepts arbitrary record inputs, and unknown or MCP tools are classified as command approvals. An input such as { prompt: longOperation } has no recognized command field, so the current fallback truncates the serialized operation to 400 characters. Users can approve the operation without seeing its complete text.

I compared this PR with a branch I'd started for the same bug. The core fix covers what I hit. Below are a few follow-ups, as patches against 0c87a3ae43; with all three applied, vp run check:changed passes. Happy to open any of them as separate PRs instead, or feel free to fold in whatever is useful.

1. The reason drops its line breaks (+1/−1). A decision reason can have several lines, especially from PreToolUse hooks. Our own Claude Code plugin, for example, splits each command into its individual parts in a hook and returns an ask reason for every part that needs approval, each explaining why it's an ask. Collapsed into one paragraph, that's hard to read (see the screenshots below).

2. One scroll area for long requests (+8/−3 ignoring re-indentation from the new scroll wrapper).

  • The command scrolls in its own 160px box, and the reason row has no limit. So reading a long request means scrolling a small box, while a long reason still grows the banner. This patch puts the command and reason in one ComposerBanner.Scroll, the same scroll area the other composer banners use (min(24rem, 40dvh), with fade). The header, including the explanation, stays as it is.
  • The existing browser test passes unchanged.
  • Web only. Mobile keeps its per-row caps, and React Native Text already keeps line breaks.

Same data, before and after patches 1 and 2. The commands, descriptions and ask reasons come from real sessions with our plugin; project and file names in the second one are anonymized.

3. Optional, outside this PR's scope: ACP command approvals lose the agent's explanation (+35/−1, with a test). Grok sends rawInput.description with each command (e.g. "Run the second permission-test rm" in the recorded grok_auto_blocked_command fixture), but the approval shows only the command. This passes it through as the explanation. Failing that, it falls back to the tool-call title, unless the title just restates the command or is a generic "Terminal".

Note, no patch: Claude MCP tool approvals show mcp__server__tool: {…args…} as the main line, cut at 400 characters. Yet the adapter already has the tool's tool_use_meta display_name and uses it for the work-log row (e.g. "Firecrawl scrape" in the claude_mcp_tool_presentation fixture). What the user approves is the tool, so its display name is what Claude Code itself shows, with the internal name and arguments as secondary detail. This seems to be a concrete case of the non-shell tool labeling follow-up listed in the description.

Related: #22799.
