fix(server): a follow-up promoted to Steer no longer reads as stopped#19352
Promoting a delegated queued follow-up to Steer cancels its queue run, so the task can read as stopped even while its work continues in the running turn. Resolve the task through its moved user message and any restart continuations, then settle it with the joined turn’s actual result. Tasks sharing a joined turn reuse one result transfer; if the turn has no owning task, its first promoted task publishes that transfer.
Rebuilt from @murayy’s work in #19352, preserving the contributor commits and merging current main. The remaining paginated-read bug is also fixed: reading an assistant-only result page acknowledges both tasks even when the promoted user message is outside that page. No wire or client changes.
Verification
- PR-specific tests: 4 cases / 406 added lines in three files → 5 integrated cases / 454 added lines in one existing file. All 60 upstream cases remain; 65/65 tests pass across the three targeted suites.
- All four initial replacement regressions fail on pinned main 1451e7c. They exercise success/failure, unrelated later restart work, two restart cuts including a continuation cut before starting, shared-transfer/idempotent recovery, and assistant-only pagination acknowledgement.
- The incoming merge had 53 passing / 11 failing targeted tests. Correcting the finalizer’s updated task argument left 63 passing / one failing pagination test. The completed rebuild adds one monitor-owner variant and passes all 65.
- Server and provider-core typechecks, changed-file lint/format and whitespace checks pass. Two independent full-diff review rounds on Claude Opus 5.5 and Codex GPT-6-Sol; round two has no confirmed code findings. Round one found and fixed stale stored card state after promotion and a missing result publisher for taskless monitor turns.
- Root integration probe ran the actual queue-to-Steer command through the real MCP toolkit, SQLite event/projection stores, effect worker and a deterministic provider adapter. It awaited provider receipt and persisted milestones, without polling or sleeps. The cancelled queue run’s task stayed running, completed with the joined provider result, reused one transfer, and both tasks were acknowledged by a one-item assistant-result page. A second actual-command probe joins a monitor turn without an owning task and verifies one result transfer and acknowledgement. Both probes assert the stored task, node and turn item move from running to completed. The temporary probe is not committed.
Headless integration proof and output.
| 1 | After promotion: queue run cancelled; task/node/item running; workState working |
| 2 | After provider completion: task completed; shared result transfers 1 |
| 3 | Assistant-only page: 1 item; acknowledged tasks 2 (delegated) / 1 (monitor) |
Verification limits
Headless Linux server/service integration only, using a deterministic provider adapter rather than live provider inference. Restart settlement is verified against persisted recovery state; a real process crash/relaunch was not exercised. A crash between promotion commit and the live status-sync handler can leave the stored card pending until final settlement, as with existing start-status synchronization. The pre-existing completion-before-steer-delivery fallback can still create a new follow-up if the provider turn ends before delivery; this race was not redesigned. While a promoted task is working, latestTerminalStatus may still describe its cancelled queue run. Native macOS, Windows, Electron, iOS and Android clients and the visual Steer control were not exercised. No client-specific changes are required by this shared server fix.
Implemented and tested with Codex GPT-6.1-Sol and GPT-6-Astra in the T3 Code Codex harness; reviewed by Claude Opus 5.5 and Codex GPT-6-Sol.

📝 Walkthrough
The orchestrator and MCP service now select the run associated with a delegated task’s result, including when a queued follow-up is promoted to Steer. The task remains pending while that turn or its restart continuation runs, then settles with the assistant result.

A thread read can leave a promoted follow-up’s terminal result unacknowledged when its user message is not available to the lookup. This is a localized MCP read-path issue; the acknowledgement behavior should be corrected before relying on it.
Security architecture risk: 🔵 Low · up to fb4b2
The inspected changes do not demonstrate broader permissions or data access. Some uncertainty remains around acknowledging shared results when only part of a conversation is read.
Retained concerns No architecture-level concerns identified.
Security review details
Security Blast Radius
- inferred — The inspected result-attribution expansion is bounded to tasks sharing an existing delegated child thread and their parent completion records. No new cross-service authority, credential access, or unrelated-thread result path was identified in these flows; broader security coverage remains unknown.
Trust Boundaries and Controls
- observed — task_status still requires a thread invocation scope and capability. Task lookup requires an app-owned task belonging to the caller's parent thread, then follows its persisted childThreadId. Terminal acknowledgement targets that same parent thread and task identity.
Resilience and Maintainability Implications
- inferred — Thread reads do not reliably acknowledge every promoted task sharing a consumed result: task enumeration uses the returned run's delegatedTaskId, page-local messages may omit the promoted user message, and the final transfer check receives assistant-only records. The base already had the ownership-based enumeration limitation, so worsened exposure is not established. Direct task_status reads preserve user-message identity and can acknowledge the promoted task separately.
Hardening Proposals
- proposed — If consuming a shared result should acknowledge all associated tasks, define that ownership rule explicitly and carry authoritative resolved-run identity through task enumeration and acknowledgement instead of re-deriving it from paginated messages.

Scope restart holds to the steered run's continuation chain.
childAwaitsRestartContinuation also checks every supplied run for which runRanAfter(run, resultRun) is true. A steered replacement has no delegatedTaskId, so both the pending check and finalization pass all child runs. A later unrelated run with a pending restart-continuation effect can therefore keep the completed replacement pending and prevent result delivery. Match the result run and its transitive restart-continuation descendants instead.

Valid for the steered case: it passed every child run, so a pending continuation on an unrelated later task's run held the promoted task. Fixed in b6ca13453c by scoping to the steered run's own task runs (runsOfTask), the same runs that task's own settlement checks. I kept childAwaitsRestartContinuation unchanged: its runRanAfter inclusion covers a second restart cutting a continuation before it started, and changing it would affect every delegated task.
The regression test now seeds that unrelated pending continuation. While extending it I found that the promoted task's result transfer made the steered run's own task skip settlement, so that commit also stops steered tasks from publishing a second transfer for the run.
@murayy, thanks for the explanation. Your scoping is sound, so I withdraw the finding.
runsOfTask limits the hold check to runs of the steered run's own task. Finalization and delegatedTaskResultPending both use it now. An unrelated later task's pending continuation no longer holds the promoted task.
Keeping childAwaitsRestartContinuation unchanged is also correct. The runRanAfter inclusion covers a second restart that cuts a continuation before it starts. My proposed rewrite to transitive restartContinuationOfRunId descendants would drop that case for every delegated task.
One edge case remains by design. If the result run has no delegatedTaskId, runsOfTask returns all runs. That is the original-task path, and the old behavior stays there.
Skipping the second subagent_result transfer for steered tasks is also consistent. The steered run's own task already publishes that transfer, and the task.result !== null check marks the steered task as settled.

| 5 | Learning: In apps/server/src/orchestration-v2/Orchestrator.ts (TypeScript, Effect), `childAwaitsRestartContinuation` intentionally includes runs for which `runRanAfter(run, resultRun)` is true, to cover a second restart that cuts a continuation before it started. Do not suggest replacing it with a transitive `restartContinuationOfRunId` descendant walk. Scope its input with `runsOfTask` instead, so a steered or promoted follow-up is only held by restart continuations of its own task's runs. |

Keep promoted follow-ups behind the steered run's restart continuation.
recoverDelegatedTask passes the steered source run as settledContinuationOf. The finalization boundary then excludes that same run from the restart-effect check, even though the effect key is effect:restart-continuation:${sourceRunId}. A promoted task can therefore settle from the cancelled steered run before its continuation completes. The result selector also does not advance from that steered run to its restart continuation.

Valid. Once the continuation run exists, recoverDelegatedTask(child, R) excludes R's restart effect and the continuation has none, so the promoted task settled as cancelled from R while the continuation was running.
Fixed in the shared resolver instead of the finalizer: delegatedTaskResultRun now follows the steered run's restartContinuationOfRunId chain to the latest continuation. The finalizer, delegatedTaskResultPending and MCP task_status all read it, so they agree. With the continuation selected, settledContinuationOf (R) no longer identifies the result run, so no change is needed there. If the continuation declines to start, no continuation run exists and the promoted task settles from R as cancelled, the same as the steered run's own task.
New test keeps a promoted follow-up open while a restart continues its turn fails without the chain walk (expected 'cancelled' to equal 'pending') and passes with it.

@murayy Thanks for the explanation. I verified that delegatedTaskResultRun follows the steered run’s restart-continuation chain. This fixes result selection in the shared resolver, so no finalizer change is needed.
The new test checks that the promoted task stays pending while the continuation runs, then completes with the continuation’s result. I inspected the test but did not run it.

| 5 | Learning: In packages/provider-core/src/server/subagentProjection.ts (TypeScript), delegatedTaskResultRun resolves a promoted queued follow-up through its reassigned user message, then follows the steered run's restartContinuationOfRunId chain. The finalizer, delegatedTaskResultPending, and MCP task_status share this resolver. In apps/server/src/orchestration-v2/Orchestrator.ts, settledContinuationOf should still exclude the source run whose continuation declined or failed; selecting an existing continuation in the shared resolver prevents premature settlement without changing that exclusion. If no continuation run exists because continuation was declined, settlement from the cancelled steered run is intentional. |

@coderabbitai The outside-diff finding on taskResultTransfer (OrchestratorMcpService.ts) is valid: a promoted task writes no transfer of its own, so resultContextTransferId read null. Fixed in fb4b22f7ad: taskResultTransfer adds delegatedTaskResultRun(child, task.id) to the result runs. The thread-read acknowledgement path passes the page's already-loaded target messages, so it needs no extra query. New task_status test fails without the change (expected null to equal 'context-transfer:mcp-steer-result'). The branch is also merged with current main.

Keep the promoted user message available for both transfer lookups.
If the page contains the steered assistant result but not the promoted user message, targetMessages cannot identify the steered run. The later pageIncludesTerminalTaskResult call also receives assistant-only resultRecords.messages. It cannot identify that run even when the first lookup succeeds. Load the promoted user message independently of page contents, and pass it to both lookups so reading the terminal result acknowledges delivery.

✅ Confirmed as addressed by @juliusmarminge
Fixed in 589b34e. The read path now loads promoted user messages independently of the page, considers every directly owned child task, and passes the resolved result run into the terminal-page check without re-resolving against assistant-only records. The focused pagination regression and actual-command MCP probes verify that a one-item assistant-result page acknowledges both shared tasks (and the promoted task joining an ownerless monitor turn). All 65 targeted tests pass; proof is attached in the PR body.
@juliusmarminge Thanks for the fix. Loading promoted user messages independently of the page addresses the pagination gap. Passing the resolved result run to pageIncludesTerminalTaskResult avoids re-resolving against assistant-only records. The reported regression coverage addresses both shared-task and ownerless-monitor cases.

Taking over this one as maintainer for batch composer-chat-16. I’ll reconcile it with current main, reduce the tests, and rebuild the smallest server fix. I’ll verify promoted follow-ups, shared result transfers and restart settlement, then push back here while preserving your commits. No merge.
