fix(pi): fork and revert past turns whose session entry was never captured#20521
Pi turns interrupted before T3 captures their session entry keep a weak turn reference. Forking or reverting past one then fails even though Pi persisted the prompt.
Recover the discarded turn's user entry from the active session branch, bounded by the retained turn's completion and the discarded turn's completion. Reuse the existing branch walker and reject missing ancestry, timestamps, or an unresolved boundary. At thread start, use the earlier of provider thread creation and turn start so adopted wakes are included while inherited parent history stays intact.
Thanks @henrychu04 for the reproduction and original fix in #20521. Maintainer takeover preserves the author's commit and adds the current-main merge, a smaller branch walk, and inherited-history coverage. Adversarial review caught a wrong-cut case where a later steer could hide a wake predating T3's turn; the revised root test reproduces it and the creation-time bound fixes it.
Verification on Linux / Node 24:
- PR regression tests: 2 β 3 in
packages/provider-pi/src/server/adapter.test.ts; the file now has 96 tests. All three recovery cases fail without the PR fix; the revised root case fails with the pre-review implementation (selects the later steer). The interrupted implementation failed all three; the rebuilt implementation passes them. - All 152 Pi package tests pass; all 8 Pi orchestration replay scenarios pass.
- Scoped Pi typecheck, changed-file lint, formatting, and whitespace checks pass, with existing warnings only.
- Real Pi 1.3.0, isolated state and a local mock OpenAI-compatible endpoint: complete prompt 1, kill Pi during prompt 2's model request, then recover through T3's Pi adapter. Fork and rollback retain prompt 1 and discard prompt 2. Root rollback also recovers a wake predating T3's first turn while retaining inherited prompt 1 (failed before the review fix, passes after it). No model credentials or live user state were used.
The change has no visual UI, so behavior tests and the native session files are the proof. macOS, Windows, iOS, Android, and Electron desktop were not verified. Authenticated model-provider behavior was not exercised; the live check used a local mock model.
Native before/after behavior proof.

Approvability
Verdict: Approved at adeccbe
Macroscope's review found this PR approvable β This is a focused Pi adapter bug fix that makes previously failing fork and rollback operations recoverable by locating existing session-tree entries, while preserving normal behavior and failing closed when no reliable match exists. The change is isolated to the provider adapter and its tests, with no product-default, schema, deployment, security, billing, or static-analysis suppression impact.

π Walkthrough
Rollback and fork operations now resolve turns without captured start-entry references by searching timestamped session entries. Session-listing parsing is centralized, and tests cover successful boundary selection and cases where no qualifying entry exists.

Merge Risk: π‘ Moderate Β· up to b8d2f
Reverting to thread start can still fail after an uncaptured adopted wake. Resolve or explicitly accept that recovery limitation before merging.
Security architecture risk: π΅ Low Β· up to b8d2f
Recovery now relies on session ancestry and timestamps rather than only captured turn references. Existing ownership checks and rejection of unresolved boundaries limit the exposure. No introduced security vulnerability was established, but timestamp assumptions and concurrent session changes remain incompletely validated.
Retained concerns No architecture-level concerns identified.

Security Blast Radius
- inferred β The demonstrated scope is the history retained in the selected Pi session and its forked destination. The changed path introduces no new session-file selector or privilege grant; source selection and destination policy assignment use the existing adapter inputs.
Trust Boundaries and Controls
- observed β Rollback checks that the session hosts the requested thread and rejects active turns or pending native work. Fork filters supplied turns by source thread ID and performs recovery through a source-derived fork process launched with tools and extensions disabled.
Resilience and Maintainability Implications
- observed β An unresolved lookup fails before issuing native fork. After native mutation, rollback retains existing failure containment: unreadable or missing session state invalidates the local thread binding, interruption terminates the connection, and finalization clears the rollback barrier.

Handle thread_start as an unbounded root.
When the rollback target is thread_start, piRollbackForkEntry sets target to null. piUncapturedForkEntry then uses the boundary turn's startedAt as from. An adopted native wake can have a user entry earlier than that time, so the entry is excluded and rollback can fail with "Pi rollback target has no captured session-tree entry".
Add an explicit root-boundary case and cover it with a direct thread_start regression test.

Checked, and keeping the bounded lower bound on purpose.
For a thread_start target the Pi session can hold entries from before T3's first turn: a forked thread's session carries the parent's history up to the fork point, and a session opened with initialNativeThreadId carries whatever was already there. With an unbounded from, an uncaptured first turn would resolve to the earliest user entry on the branch β the parent's first prompt β and the revert would cut through the inherited context. That is a wrong result, which the fallback must never produce.
With from = startedAt, the only case lost is an adopted native wake whose prompt landed before turn 1 opened, and there it refuses with the existing error rather than guessing. Fail-closed is the contract for this path; the thread_start + wake combination has no safe lower bound without more data than the projection holds.
@henrychu04 Thanks for the clarification. My suggested unbounded lower bound was unsafe because it could select an inherited prompt and discard retained context.
Keeping from = startedAt preserves the fail-closed contract. If an adopted native wake predates the first T3 turn, refusing the operation is safer than selecting an uncertain boundary. I withdraw this finding.

| 5 | Learning: In packages/provider-pi/src/server/adapter.ts, piUncapturedForkEntry must retain a bounded timestamp lower bound for thread_start targets. Pi sessions can contain inherited history from a fork or initialNativeThreadId, so an unbounded lower bound can select an inherited user prompt and discard retained context. The uncaptured-boundary fallback follows a fail-closed contract: if an adopted native wake predates the first T3 turn and the projection provides no safe lower bound, return the existing error rather than guess. |

- fix(pi): fork and revert past turns whose session entry was never capturedb8d2fba
Filed a triage issue for the bug this PR fixes: #20530. It has the no-key reproduction, the root cause with file references, and links back to this PR as the proposed repair.
Taking over this one. I'll trim the tests to the minimum, rebuild the implementation to the repo's conventions, and push to this branch.
