Problem

An oversized context handoff can fail every new turn on a thread. On a near-full Codex conversation, dispatching a deferred run, interrupting it before provider start, and sending “Continue” fails with “Insufficient context allowance” even though the provider can still accept the user's prompt.

Change

Defer a handoff that cannot fit the existing allowance and send the user's prompt unchanged. Keep its history on the existing durable handoff record and retry it on later turns to the same provider thread. Pending or uncertain delivery preserves the retry marker; confirmed delivery clears it. Missed-run recovery reuses deferred history instead of allocating duplicate handoffs.

The reserve policy stays the same. No provider protocol or client behavior changes. Original implementation by @Alb11747; this update preserves that implementation and authorship, merges current main, and adds the interrupted-before-start Codex regression.

Scope and approval

GitCafe: https://git.cafe/pingdotgg/t3code/issues/23275

Triaged bug via the automated issue flow. This preserves the maintainer's same-provider deferral scope. Forwarding portable fork/merge history to another provider remains outside this fix.

Verification

  • New focused regression fails on main fef851bee0 with the reported context-allowance error and passes at cdc28c82a5.
  • 93 focused tests pass: ProviderSwitch.integration.test.ts and ContextHandoffBudget.test.ts (51), ProviderTurnStartService.test.ts (32), and provider-core failure.test.ts (10). Coverage includes repeated deferral, uncertain native delivery, abandoned inline preparation, compaction, and eventual delivery.
  • Scoped server, provider-core, and contracts typechecks pass. Changed TypeScript files pass lint with three existing unused-variable warnings; formatting and diff whitespace checks pass.
  • Independent GPT-6.1-Sol and Claude Opus 5.5 full-diff reviews found no blockers.

Evidence (server behavior; visual matrix is empty):

Environment: Linux fs-40, real development server with isolated worktree state, Codex GPT-6.1-Sol at low reasoning effort, configured context window 44,000 and auto-compaction threshold 42,000. The main failure was also observed in the real web client. No renderer changed, so screenshots and recordings are not the behavior proof.

Fresh live-provider coverage is Codex on Linux. Native desktop/mobile clients and Cursor, Grok, OpenCode, and Antigravity were not separately exercised; they use the shared turn-start service.

Approvability

Verdict: Approved at 27cde70

Macroscope's review found this PR approvable — This is a focused server bug fix that replaces an unrecoverable oversized-handoff failure with deferred delivery and later retry, using an optional backward-compatible state field. The behavior is contained to handoff orchestration and is covered by targeted native, inline, retry, and provider-thread tests.

Walkthrough

Over-budget context handoffs are now marked for deferred delivery instead of causing turn-start failure. Later turns can select and deliver those handoffs, while missed-history recovery excludes items already represented by deferred handoffs.

Merge Risk: External image Minimal · up to 27415

Deferred handoffs remain eligible for later attempts, and no issue requiring a fix before merge was established.

Security architecture risk: External image Low · up to 27415

The retry lifecycle remains scoped to the intended conversation and provider thread, and preserves uncertain delivery until recovery. No introduced security issue was established. Some concurrent-start, provider-specific recovery, and downgrade behavior remains unverified.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The changed exposure is delayed delivery of previously selected conversation history to later turns on the same logical provider thread. The inspected path does not establish new tenant-wide, environment-wide, or credential authority. Its demonstrated scope is conversation context and provider-session recovery, not a new externally callable endpoint.

Trust Boundaries and Controls

  • observed — Stored history enters the provider through native history injection or rendered inline context after budget selection. The over-budget branch returns no history context rather than bypassing the allowance. When no context or restart note is present, turn startup sends the user's text unchanged.

Resilience and Maintainability Implications

  • observed — An uncertain pending delivery on the current native thread prevents ordinary resume and triggers a fresh native session while retaining the logical provider-thread identity. Direct native injection also rejects an already-pending delivery to that native thread. These existing controls contain ambiguous replay while the new deferral marker preserves retry selection.

Keep deferred handoffs selectable until the inline prompt is accepted.

On the no-native-thread path, persist("pending") clears deferred before the prompt is sent. A process stop before startTurn accepts the prompt leaves the handoff ready with no deferred flag and no delivery marker. The turn-start selection then skips it, because the source run is a completed ordinary turn rather than a /compact turn. Clear deferred only when the inline delivery is recorded, so a stopped send stays selectable. A crash after acceptance but before that record can resend the handoff once.

✅ Addressed in commit 72a3594

Fixed in 72a3594ef1. Pending writes now retain the deferred retry marker; injected/inline delivery clears it only when settled. Regression coverage checks both interrupted inline preparation without a native thread and lost native-injection acknowledgement followed by replacement.

@Alb11747 Thanks for the fix in 72a3594ef1. The supplied code retains deferred during pending writes and clears it when injected or inline delivery settles. This addresses the interrupted-send finding.

P2 Badge Reuse the deferred missed-run handoff

When missedItems is nonempty because a failed or interrupted attempt never produced a provider turn, this persists the freshly allocated retryHandoff as deferred. On the next turn that handoff is selected, but only settled deliveries populate coveredItemIds, so missedItems remains nonempty and prepareProviderHandoff allocates another handoff for the same items; every low-budget turn then adds another durable handoff and duplicate context-handoff.updated events, and eventual delivery updates all of them. Reuse or suppress the already-deferred missed-run handoff instead of accumulating one per turn.

P2 Badge Retain the retry marker through pending delivery

When a deferred handoff is retried after its original target run has completed, this pre-delivery pending write removes the only condition that selects it on later runs. If injectHistory then fails ambiguously (or the final delivery write fails), the next start replaces the uncertain native thread, but the original handoff is excluded because its target run is completed, deferred is gone, and delivery is defined. Portable fork or merge-back history cannot be reconstructed by the replacement thread's same-thread history handoff, so that context is silently lost; keep the marker until injected/inline settles or explicitly select pending handoffs for the replacement.

Tested this PR against another trigger for the same wedge: a provider round trip in a long Claude thread (#17952 has the same zero-budget failure).

Scenario. On 0.0.46-nightly.20261014.3105, a thread-bound scheduled task ran GPT-6.1-Sol inside a Claude Opus 5.5 thread. Claude's last recorded usage was 840,960 / 1,000,000, and the Sol turn used 87,369 / 475,000. On the return to Claude, the reserve is max(16k, window/4) = 250k, so the handoff budget is 0 even though about 159k is still free. The Codex→Claude handoff threw ContextHandoffBudgetError, and every later turn failed until a manual /compact.

Regression tests (test-only, +258 lines in two files, based on this PR's head 72a3594ef1):

  • ProviderSwitch.integration.test.ts: Claude reaches 840,960/1M, Codex runs in the same thread, then three ordinary Claude turns follow on the same native thread. Variants: normal start, and a refused target start (the failed-turn retry). After that comes a real /compact dispatch (reports 14,938/1M) and two more turns.
  • ContextHandoffBudget.test.ts: with budgets of 0 and 64, nothing is injected and the full history is kept. At a usable budget, oversized items are omitted whole, the omitted IDs and the t3_thread_read pointer are kept, and nothing is injected again on retry.

Results (Node 24.13.1, vp test run, from apps/server):

  • main @ 7ac952c630: 3 failed. All three Claude return turns fail with "Insufficient context allowance for the provider handoff…".
  • This PR plus the tests: targeted 3/3 pass. ContextHandoffBudget.test.ts, ProviderTurnStartService.test.ts and testkit/ProviderSwitch.integration.test.ts: 115/115 pass. File-scoped vp lint and server tsc-rs --noEmit are clean.
  • Asserted behaviour on this PR: the user text reaches the provider unchanged, including non-ASCII text and trailing whitespace. The pending handoff stays ready + deferred with its content intact. Repeated low-budget turns create no duplicate recovery records. After /compact, the Sol result is delivered exactly once. There are no extra provider threads or handoffs.

Branch with the tests on top of this PR: https://github.com/EtanHey/t3code/compare/72a3594ef1b5632f62112cc9da366f2733f42892...test/19414-claude-round-trip-handoff (3 commits, ac58c9b154). You're welcome to cherry-pick them if they help.

One question, not a blocker. When the budget is 0, the deferred handoff sends the model nothing, not even a coverage line or a t3_thread_read pointer. Here, Claude answered "continue from what Sol did" turns without knowing Sol had run. Would a pointer-only marker, emitted when it fits in the physical headroom rather than the reserved budget, be in scope here or for a follow-up? #16147's triage mentions "defer it, or use a pointer". This is a maintainer policy call, so I'm only raising it.

Not live-tested: these are unit and integration tests only, with no real Claude or Codex run.

Tests written by GPT-6.1-Sol (Codex harness, high effort), reviewed in two rounds by Claude Opus 5.5 (T3 Code), coordinated by Claude Opus 5.5 (Claude Code).

Synced from GitHub

Thanks for the additional offline regression results and test branch. The pointer question matches the source: a zero handoff budget also defers the coverage marker. This PR preserves the existing reserve policy while allowing turns to proceed and retaining history for later delivery. Sending a pointer out of reserved headroom would change that policy, so I would leave that to a maintainer-approved follow-up.

P2 Badge Carry deferred context to newly selected provider threads

When the first turn after a portable fork or merge defers its handoff and the following ordinary turn chooses a different provider thread (for example, a larger-window provider), this equality leaves the deferred record attached to the old thread, so it is never passed to deliverContextHandoffs. The replacement provider-switch handoff cannot substitute for it: apps/server/src/orchestration-v2/Orchestrator.ts:5827-5830 reads only the target thread, and historicalMessage in packages/provider-core/src/server/handoffBudget.ts:138-168 rejects the handoff turn item that references the source transcript. The new provider therefore starts without the fork or merge context; carry or fold deferred records into the newly selected provider thread.

Useful? React with 👍 / 👎.

Confirmed: switching providers can omit the deferred portable fork/merge context. The row owns one destination and delivery receipt; broadening selection would misattribute delivery, while retargeting it would steal the original provider's retry state when switching back. Safe forwarding needs separate per-destination tracking or propagation through provider-switch history. The same propagation gap also exists after successful portable delivery. That exceeds this PR's bounded same-provider retry scope, so I have left the guard unchanged and this finding unresolved pending an explicit scope decision.

Maintainer scope decision: keeping this PR to oversized-handoff deferral and retries on the same provider thread, as requested for this takeover.

I rechecked the forwarding finding against ProviderTurnStartService and provider-switch history selection. The destination guard protects a record with one destination and one native delivery receipt. Removing it would misattribute a receipt or consume the original destination's retry state. The missing propagation of portable fork/merge history also occurs after successful delivery, so fixing that broader existing behavior requires its own forwarding/receipt design.

@Alb11747's analysis is correct. I am treating this as a separate follow-up, not a blocker for the bounded deferral fix, and resolving this thread with that scope decision. The current takeover verifies repeated budget-zero turns, durable restart, compaction, eventual delivery and no replay using real Codex/Claude sessions; the PR body records the remaining forwarding limitation.

Taking over this one. Keeping this scoped to durable oversized-handoff deferral, with focused regression tests, live provider verification, and independent reviews before publishing.
