Problem

When a Claude thread runs a native Agent subagent, the subagent's chat lists its tool calls but never its thinking, during the run or after it. Claude Code (2.1.304 here, 2.1.312 in the report) no longer forwards a subagent's thinking over the Agent SDK stream: frames with a parent_tool_use_id carry only tool_use blocks. The thinking is written only to the subagent transcript, <claude config>/projects/<project>/<session>/subagents/agent-<task_id>.jsonl.

Reproduced on main (dd14848153) with the real server. A Claude Opus thread is asked to run one foreground subagent that reads three files and thinks between reads. The child thread gets user_message, command_execution ×3, assistant_message and no reasoning, while the subagent's transcript holds 3 thinking blocks. The same holds with Sonnet and with an Explore subagent.

Change

The adapter now reads a native subagent's thinking from its transcript, using the contained, cursor-based reader that Workflow members already use.

  • Finding the transcript. The project directory is located by session id (projects/*/<session>.jsonl), not rebuilt from the cwd, so symlinked and long paths work. It is cached per session, along with a miss.
  • Ordering. Clients order child items by first-commit position, so each thinking block must be committed before the tool call it preceded. Before projecting a subagent's tool frame, the adapter reads the transcript up to that frame's line, matched by uuid (the SDK frame and its transcript line share it). It re-reads every 25 ms, for at most 500 ms, while Claude Code flushes, about every 100 ms. Lines past the frame stay buffered for later frames.
  • Settling. A completed subagent waits the same way for its turn-end line, so its final thinking lands before the reply. A cancelled, stopped or failed subagent, or one whose query is closing, reads once and does not wait.
  • Ending a wait early. A new closing deferred, set wherever the live query closes, ends any wait at once. A subagent whose transcript never appears holds only its first frame.
  • Duplicates. Older Claude Code that still forwards thinking is deduplicated by line uuid. Re-reading after a restart re-emits the same item ids, which keep their positions.

Verification

  • vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts apps/server/src/orchestration-v2/workflowAgentTranscript.test.ts: 265 passed. The new tests cover:

    • thinking shown while the subagent runs
    • SDK-forwarded thinking shown once
    • real frame order: thinking before its tool call, final thinking before the reply, two thinking blocks in one message
    • transcript backlog ahead of queued frames
    • a transcript that doesn't exist yet
    • a frame whose line never appears (held exactly 500 ms)
    • a subagent without a transcript holds only its first frame
    • a stopped subagent doesn't wait
    • interrupt ends the wait with no further reads
    • restart recovery

Each fails on the commit before its fix.

  • vp run check:changed: all pass.

  • Runtime transcript. Real server (t3 serve, fresh database), Claude Code 2.1.304, claude-opus-5-5, one foreground subagent reading three synthetic files.

Before (main dd14848153): the child thread has no reasoning.

    1user_message, command_execution, command_execution, command_execution, assistant_message

After (fb5dadc341): the child thread follows the transcript order.

    11 user_message 00:30:18.928
    22 reasoning 00:30:21.857 "I should start by reading f1.txt…"
    33 command_execution 00:30:21.968
44 reasoning 00:30:30.312 "The file 1 content seems to say…"
    55 command_execution 00:30:30.425
66 command_execution 00:30:34.006 (no thinking before this call in the transcript)
77 reasoning 00:30:39.441 "Each file has a number appearing twice…"
    88 assistant_message 00:30:39.587

    Subagent transcript: thinking 21.857, text, tool_use 21.867, thinking 30.312, text, tool_use 30.318, text, tool_use 33.872, thinking 39.441, text 39.455.

In earlier runs on this branch, frames were held 82–115 ms per subagent tool call and 24–73 ms at settle. None hit the 500 ms bound. Two parallel foreground subagents each kept their own order.

  • Not checked:

    • background subagents end to end (same code path, covered by reading only)
    • macOS and Windows paths
    • Claude Code versions older than 2.1.304

Closing as a duplicate of #27211, which fixes the same bug (Claude subagent child threads showing no thinking, #27116 / #27178) by turning on the Agent SDK's forwardSubagentText option. Its probe shows Claude Code 2.1.304 and the reporter's 2.1.312 forward subagent thinking with that option on, so the fix stays a one-line query option instead of reading subagent transcripts from disk and holding frames. Thanks for the careful reproduction and the frame-ordering analysis here: the runtime transcript and the ordering tests were useful in comparing the two.

Walkthrough

ClaudeAdapterV2 now reads native subagent transcripts to recover thinking omitted from SDK frames. It orders transcript thinking with SDK items, avoids duplicate thinking, and bounds transcript polling during query closure.

Explanation

The description includes complete Problem, Change, Scope and approval, and Verification sections. It explains the bug, implementation, linked issue, focused tests, runtime validation, and unverified environments.

Full details: Linked Issues check

Explanation

The only directly linked issue, #27116, is marked closed and completed. Its state text says it is historical context only and not a requirement for this pull request. No active directly linked target remains, so no linked-issue coding requirements apply. The pull request still addresses the reported behavior: it reads native subagent thinking from the transcript in ClaudeAdapterV2.ts. That work does not create a requirement.

Full details: Out of Scope Changes check

Explanation

Based on the whole-PR summary and description, the changes stay within the native Claude subagent thinking path. In ClaudeAdapterV2.ts, the changes locate the transcript by session id, read it with a cursor, match lines to SDK frames by uuid, add ordering and settle waits, add a closing deferred, suppress duplicate thinking IDs, and retain transcript state across lifecycle updates. Closing helper use in query replacement, interruption, session close, and native-thread close supplies the closing signal that ends waits. In ClaudeAdapterV2.test.ts, the new tests cover these behaviors. The wake harness change that sets CLAUDE_CONFIG_DIR to a nonexistent default isolates transcript lookups and supports those tests. No change in the summary falls outside native subagent thinking. The source diff was not read in full; this verdict relies on the summary, which establishes the scope.

119L95:
120L96: 【12†】
121L97:
122L98: Set variables in settings files
123L99: Add variables under the `env` key in a `settings.json` file, creating the file if it doesn’t exist. Claude Code reads them directly from the file, so they take effect no matter how `claude` was launched. A running session applies new and changed values to its environment when you save the file, but a feature that reads its variables once at startup, such as 【13†OpenTelemetry monitoring】, keeps its startup values until you relaunch.
124L100: Removing a variable from the file doesn’t unset it in a running session; the removal takes effect the next time you launch `claude`.
125L101: ~/.claude/settings.json
126L102:
127L103: `{
128L104: "env": {
129L105: "API_TIMEOUT_MS": "1200000",
130L106: "BASH_DEFAULT_TIMEOUT_MS": "300000"
131L107: }
132L108: }
133L109: `
134L110:
135L111: Claude Code copies these values into its environment as written. No shell processes them, so shorthand such as `~` or `$HOME` stays as typed. For a variable that takes a path, such as `CLAUDE_CONFIG_DIR`, write the absolute path: `"CLAUDE_CONFIG_DIR": "/home/you/.claude-work"`. The file you choose controls who the variables apply to:
136L112: File | Applies to
137L113: --- | ---
138L114: `~/.claude/settings.json` | You, in every project
139L115: `.claude/settings.json` | Everyone working in the project, checked into source control
140L116: `.claude/settings.local.json` | You, in this project only, gitignored when Claude Code saves a setting to it; add it to your gitignore if you create it by hand
141L117: Managed settings | Everyone in your organization, deployed by an admin
142L118: See 【14†Settings files】 for where each file lives and 【15†Settings precedence】 for how they combine when more than one sets the same variable.
143L119: ##
144L120:
145L121: 【8†】
146L122:
147L123: Precedence
148L124: Some behaviors have both an environment variable and a dedicated settings key, and which one Claude Code reads first differs per key. For `ANTHROPIC_MODEL` and `CLAUDE_CODE_AUTO_CONNECT_IDE`, Claude Code reads the variable first and uses the `model` or `autoConnectIde` setting only when the variable is unset. For the pair you’re setting, check the variable’s row below and the key’s entry on the 【16†settings reference】.
149L125: When the same variable is set in both your shell and a settings file `env` block, the settings file value applies in most sessions. Claude Code writes each `env` entry into the process environment, replacing the value inherited from the shell. 【17†How `env` values interact with your shell】 covers the sessions that keep the inherited value instead, and the 【18†`env` setting】 says when it applies them. A few variables are special-cased; the 【19†`env` setting】 lists the exceptions.
150L126: In a settings file you can set a variable but you can’t remove one. To override a variable you can’t unset, such as a stale `CLAUDE_CODE_USE_VERTEX` exported by a shell profile you don’t control, set it to an empty string in the `env` block: `"CLAUDE_CODE_USE_VERTEX": ""`. Claude Code treats the empty value as unset for provider selection. Subprocesses still inherit the empty value.
151L127: Between settings files, `env` values follow 【15†settings precedence】, so a managed settings entry overrides the same variable in user or project settings. Project and local settings can’t set some variables, such as `CLAUDE_CONFIG_DIR` and the OpenTelemetry exporter variables. 【20†Variables Claude Code ignores in `env`】 lists them, along with the OpenTelemetry off values that still apply.
152L128: How an environment variable interacts with CLI flags and in-session commands varies per feature: `--model` and `/model` override `ANTHROPIC_MODEL`, while `CLAUDE_CODE_EFFORT_LEVEL` overrides `--effort` and `/effort`. When a variable interacts with another configuration source, its row in the 【9†Variables】 list states the precedence or links to the page that documents it. Claude Code reads shell environment variables at startup, so changes to them take effect the next time you launch `claude`.
153L129: Variables set under the `env` key in settings files are reapplied to a running session when the file changes, with the startup-only exception described in 【21†Set variables in settings files】.
154L130: ##
155L131:
156L132: 【9†】
157L133:
158L134: Variables
159L135:
160L136: Numeric variables such as timeouts, token budgets, and retry counts accept scientific notation and digit-separator spellings in addition to plain digits, except where a variable’s row notes it takes plain digits only. For example, Claude Code reads `2e3` as 2000 and `64_000` as 64000. Before v2.1.211, these spellings could silently set a much smaller value, such as `1e6` setting a timeout to 1.
161L137: For variables that turn a behavior on or off, set `1`, `true`, `yes`, or `on` to turn it on and `0`, `false`, `no`, or `off` to turn it off, in any casing. Some variables read only whether you set them at all, so any non-empty value including `0` turns the behavior on, and you turn the behavior off by unsetting the variable or setting it to an empty value. These variables work that way:
162L138: * `CLAUDE_CODE_DISABLE_NONESSENTIAL_TRAFFIC`
163L139: * `DISABLE_TELEMETRY`
164L140: * `DISABLE_ERROR_REPORTING`
165L141: * `CLAUDE_CODE_TMUX_TRUECOLOR`
166L142: * `IS_DEMO`
167L143: One other variable has its own rule: `FORCE_HYPERLINK` reads a number, so only `0` turns it off. Each variable’s row also states its own rule.
168L144: Variable | Purpose
169L145: --- | ---
170L146: `ANTHROPIC_API_KEY` | API key sent as `X-Api-Key` header. When set, this key is used instead of your Claude Pro, Max, Team, or Enterprise subscription even if you are logged in. In non-interactive mode (`-p`), the key is always used when present. In interactive mode, you are prompted to approve the key once before it overrides your subscription. To use your subscription instead, run `unset ANTHROPIC_API_KEY`
171L147: `ANTHROPIC_AUTH_TOKEN` | Custom value for the `Authorization` header (the value you set here will be prefixed with `Bearer `)
172L148: `ANTHROPIC_AWS_API_KEY` | Workspace API key for 【22†Claude Platform on AWS】, generated in the AWS Console. Sent as `x-api-key` and takes precedence over AWS SigV4
173L149: `ANTHROPIC_AWS_BASE_URL` | Override the 【22†Claude Platform on AWS】 endpoint URL. Use for custom regions or when routing through an 【23†LLM gateway】. Defaults to `https://aws-external-anthropic.{region}.api.aws`. Claude Code resolves the region with the 【24†same precedence as on Amazon Bedrock】
174L150: `ANTHROPIC_AWS_WORKSPACE_ID` | Required for 【22†Claude Platform on AWS】. Sent on every request as the `anthropic-workspace-id` header
175L151: `ANTHROPIC_BASE_URL` | Override the API endpoint to route requests through a proxy or gateway. For what happens when that gateway rejects MCP tool search, see 【25†Tool search through an LLM gateway…[truncated]</excerpt>

493EDIT (corrected): the test below verifies only the spawned agent path. The extension host still ignores `CLAUDE_CONFIG_DIR` on Windows (session list/switch, `settings.json`, MCP/plugin persistence all default to `~/.claude`), so this is not macOS-only or fixed — the host-side half of this issue still reproduces. See my follow-up below and @pasrom's root-cause analysis. Original kept for context.

497Windows 11: works via the `claudeCode.environmentVariables` setting (ext 2.1.178)
498
499On Win 11 (extension 2.1.178, CLI 2.1.177) the `claudeCode.environmentVariables` setting does point the spawned Claude session at a non-default `CLAUDE_CONFIG_DIR`:
500
501 "claudeCode.environmentVariables": [{ "name": "CLAUDE_CONFIG_DIR", "value": "C:\\Users\\<me>\\.claude-alt" }]
502
503To rule out a leaked shell variable, I opened the repo in a throwaway instance (`code --new-window --user-data-dir <temp> <repo>`) with no `CLAUDE_CONFIG_DIR` in the environment and a fresh profile; `/status` then came up under the account in the custom dir, not `~/.claude`. The setting was the only possible source.
504
505Since the issue is labeled `platform:macos` and the report was on 2.1.63, this suggests the bug is macOS-specific and/or was (partly) fixed by 2.1.178 — might be worth a maintainer scoping. (I only verified the setting path, not the shell-export path.)

517@kdsrc this only partially works. I am also on Windows 11, though using WSL.
518
519 * Switching between sessions does not work, it will show up empty when switching to an older session.
520 * Plugins and MCPs will not survive the VSCode session, I would need to reinstall every time with a new VScode session.

542
543When asked to edit Claude Code configuration, Claude Code operates on `~/.claude/settings.json` even when `CLAUDE_CONFIG_DIR` is set to a different directory. The rest of the CLI honors `CLAUDE_CONFIG_DIR` correctly, so the config it edits is not the config it actually reads. Edits land in a file that is never loaded.
544
545## Steps to reproduce
546
547 1. Set `CLAUDE_CONFIG_DIR` to an XDG-style path, for example `export CLAUDE_CONFIG_DIR="$HOME/.config/claude"`. Real settings live at `~/.config/claude/settings.json` and `~/.claude/settings.json` does not exist.
548 2. Start Claude Code and confirm the env var is set.
549 3. Ask it to change a setting, add a permission, or set an env var (this routes through the update-config skill).
550
551## Expected behavior
552
553Reads and writes `settings.json` from `$CLAUDE_CONFIG_DIR`.
554
555## Actual behavior
556
557Reads and writes `~/.claude/settings.json`, ignoring `CLAUDE_CONFIG_DIR`. On a machine where that file doesn't exist, it silently creates a new one that Claude Code never loads, so the change appears to succeed but has no effect.
558
559This is the same root cause as #16899 (statusline-setup agent, closed as stale) but broader. The path resolution used when editing config should honor `CLAUDE_CONFIG_DIR` everywhere, not just in the main read path.
560
561## Environment
562
563 * macOS (Darwin 25.5.0)
564 * `CLAUDE_CONFIG_DIR=~/.config/claude`

576If you have a legitimate default `~/.claude` configuration dir, and then another one (or even several) for other purposes (multi-tenant setup) to which you point with `CLAUDE_CONFIG_DIR` (e.g. using `direnv`), then it's even worse:
577
578In addition to reading and reasoning from the wrong setup - the wrong file already contained the exact binding I'd asked for, so its contents get reported back as the current state - it does not just create a stale `~/.claude` tree when writing changes, but actually silently modifies another account's live configuration.

Trim CLAUDE_CONFIG_DIR before resolving it.

When config.homePath is empty, makeClaudeEnvironment preserves the inherited value. The current check then converts an empty value to session.cwd and a whitespace-only value to a whitespace-named child of session.cwd. Native and workflow transcript readers use that path instead of their default Claude configuration directory.
