a Codex turn fails with a usage limit, usage is re-probed at 100%, and the next new thread falls back to Claude

control: Codex at 20% with no failure keeps new threads on Codex with no notice

Stacked on #26164. A turn that hit a usage limit left the provider's usage snapshot stale until the next periodic probe, so new threads kept picking the exhausted default. A committed usage-limit turn failure now re-probes that instance's usage right away, skipping disabled instances, providers that don't report usage, and probes already in flight. Claude clears its cached usage first, Cursor's SDK limit codes and OpenCode v1 429s now count as usage limits, and Antigravity is unchanged because its errors carry no reliable quota signal.

Verified end to end on a dev server with a Codex CLI fixture and a real Claude login: the failed turn started a re-probe 46 ms later, which read 100% 110 ms after the failure, and the next new thread opened on Claude. check:changed passes. Real Claude exhaustion, Cursor and OpenCode v1 classification against live accounts, and mobile remain unverified.

🟠 High server/usageLimits.ts:122

A window-only update that advances published.checkedAt makes this branch replace the probe's overageAllowed: true with stale false (or omit it), so exhaustedUsageWindow continues triggering fallback despite usable overage. checkedAt reflects the window update, not permission freshness; preserve probed.overageAllowed unless there is a separate signal that the published permission is newer.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The change refreshes provider usage after usage-limit failures, with asynchronous, coalesced refreshes. It also adds a line-level static-analysis suppression in the replay harness that requires human review.

Walkthrough

The changes classify specified Cursor and OpenCode failures as usage limits. Managed providers now expose a coordinated usage-refresh effect. The event ingestor starts that effect when it receives a failed terminal event classified as a usage limit.

Changes: Usage-limit classification and refresh

Layer / File(s)Summary
Classify provider usage-limit failures
packages/provider-cursor/src/server/CursorAgentSdk.ts, packages/provider-cursor/src/server/adapter.ts, packages/provider-cursor/src/server/adapter.test.ts, packages/provider-opencode/src/server/adapter.ts, packages/provider-opencode/src/server/adapter.test.ts
Cursor recognizes specified rate-limit errors and codes; OpenCode classifies APIError errors with status 429 as usage_limit. Tests cover both classifications.
Coordinate managed-provider usage refresh
packages/provider-core/src/server/snapshot.ts, packages/provider-core/src/server/managedProvider.ts, apps/server/src/provider/Drivers/ClaudeDriver.ts, packages/provider-core/src/server/managedProvider.test.ts
Managed providers expose refreshUsage, which skips unsupported snapshots and coordinates refresh requests. Claude configures usage-cache invalidation. Tests cover queued refreshes and interruption.
Trigger refresh after usage-limit failures
apps/server/src/orchestration-v2/ProviderEventIngestor.ts, apps/server/src/orchestration-v2/ProviderEventIngestor.test.ts, apps/server/src/orchestration-v2/ProviderSessionManager.test.ts, apps/server/src/orchestration-v2/testkit/ProviderReplayHarness.ts
The ingestor looks up a provider instance and forks its refreshUsage effect for failed terminal events classified as usage limits. Test setups provide the registry dependency, and an ingestor test checks lookup and refresh behavior.

Reconcile non-explicit draft provider selections before Send.

A local draft reports draftFallback.selection, but ChatComposer checks composerDraft.activeProvider before the thread and project selections. The resolver does not use usage limits when checking provider availability. Therefore, an exhausted provider can remain selected when its provider entry is still available. deriveEffectiveComposerModelState then preserves that provider's draft model, and modelSelectionExplicit does not change this path.

Reconcile activeProvider and its stored selection in the composer draft/provider-selection path when usage changes. Preserve them when modelSelectionExplicit === true. Do not rely on ChatView's project fallback or thread-model precedence; those values only provide the base model after the provider has been selected.

Note

Written by claude-opus-5-5 on behalf of Maria

declining. the fallback is scoped to new threads. a draft keeps the model it resolved when it opened, and reopening it through new thread re-resolves non-explicit selections through the fallback resolver (useHandleNewThread.ts ~291). nothing live-rewrites an open draft's non-explicit model when the project default changes either, so doing it only for usage would add a draft-store sync effect driven by usage snapshots. if a stale draft does send, the turn fails with usage_limit, this pr re-probes usage, and the next new thread falls back. this code also landed in #26164; it shows here only until main is merged into this branch.

@maria-rcks Understood. My finding incorrectly treated fallback as a live update requirement for open drafts. With fallback scoped to new-thread resolution, retaining an open draft’s model is intentional.

I withdraw the finding. No usage-driven draft-store synchronization is needed for this behavior.

5Learning: For a new draft thread in `apps/web/src/components/ChatView.tsx`, `buildLocalDraftThread` assigns the resolved project default to the thread's `modelSelection`. `ChatView` passes `activeThread?.modelSelection` to `ChatComposer` as `activeThreadModelSelection`. In `apps/web/src/composerDraftStore.ts`, `deriveEffectiveComposerModelState` preserves an unavailable selection when that thread selection's instance matches the selected instance. A test of this new-thread path should pass the stored default as `threadModelSelection`, not `null`.

5Learning: In `apps/web/src/modelSelection.test.ts`, tests comparing `resolveEffectiveDefaultModelSelection` with project-default resolution in `deriveEffectiveComposerModelState` should pass `threadModelSelection: null` and the stored default as `projectModelSelection`. In `apps/web/src/composerDraftStore.ts`, a null thread selection makes `preserveThreadModel` false, so an unavailable, non-hidden OpenCode or Antigravity project-default model falls back instead of being preserved.

Preserve refresh eligibility when Claude status probing fails.

Claude can publish a snapshot without usageLimits when its capability probe fails. A later failed turn.terminal usage-limit event then reaches refreshUsage, but this guard returns before invalidating Claude’s cache or starting the forced probe. Keep unsupported providers distinguishable from temporarily unknown usage.

8Update Claude’s failed capability-probe snapshot construction to include an

declining. this re-probe refreshes usage windows a provider has already reported, so the fallback sees exhaustion sooner. a snapshot with no usageLimits (never reported, or a failed claude capability probe) gives the fallback resolver nothing to act on, and the regular status refresh already retries failed probes on its interval. marking claude probe failures as probeFailed usage would change how every client shows claude's status after a failed probe, which is outside this pr.

@maria-rcks Understood. This refresh targets usage windows the provider has already reported. The !snapshot.usageLimits guard matches that scope. My suggestion would expand the behavior and change Claude’s published status after probe failures. I withdraw the finding.
