Problem

A delegated child's "asked a question" notification is queued on its parent with queue_after_active. Nothing ties that queued run to the question's lifetime. When the child finishes, or its question is answered or closed, before the parent's turn ends, the wake still starts after the parent's final turn. Its answer to the stale question then becomes the parent's latest result, which is what delegate_task and task_status report upward. Reproduced on main with the orchestrator tests below: the stale wake stays queued after the task settles, and starts (starting) when the settle event was missed.

Change

A question wake now lives only as long as its question:

  • DelegatedTaskQuestionReactor cancels a parent's queued wakes when the child's question is answered or closed, and when the app-owned task settles (completed, failed, cancelled, interrupted). A failed cancel is logged and the others still run.
  • Orchestrator.startNextQueuedRun sweeps queued question wakes before choosing the next run. It cancels any whose task is gone or settled, or whose request is no longer pending. This also covers events missed across a restart and wakes queued by an older server. A failed child-thread read is logged and the wake is kept, so it can never fail the parent's unrelated next queued run.
  • dispatchMessage accepts a stale wake as a no-op (the existing thread.metadata-updated pattern) instead of starting a run.

Result selection is unchanged: a question wake that is still valid when it starts is real work and is treated like any other turn.

Scope and approval

Fixes pingdotgg/t3code#25639

Triaged via the automated issue flow; GitCafe issue #25637.

Verification

  • Orchestrator.control-reads.test.ts, the new drops a stale question wake cases (live settle event, and missed event plus queue resume). Each also checks that a live follow-up question on the same child still wakes the parent, and that a stale wake dispatched directly is accepted without starting a run. On main both cases fail: expected 'queued' to equal 'cancelled' and expected 'starting' to equal 'cancelled'.
  • On e041743849: vp run check:changed passed (format, lint, knip, typecheck of 3 packages, tests). Orchestrator.control-reads.test.ts: 14 passed.

Walkthrough

The reactor cancels queued delegated-question wakes when the child task becomes terminal or its request is no longer pending. The orchestrator detects stale wakes before queue selection and message dispatch. Tests cover normal and missed terminal-event paths.

Changes: Delegated question wake lifecycle

Layer / File(s)Summary
Cancel wakes from task and request updates
apps/server/src/orchestration-v2/DelegatedTaskQuestionReactor.ts
The reactor finds queued parent runs for delegated-question messages and dispatches cancellation when an app-owned subagent becomes terminal or a request is no longer pending. Cancellation failures are logged.
Prune and block stale wakes
apps/server/src/orchestration-v2/Orchestrator.ts, apps/server/src/orchestration-v2/Orchestrator.control-reads.test.ts
Before selecting a queued run, the orchestrator cancels wakes whose task or matching pending child request is missing or terminal, then reloads the queue projection. It skips stale incoming messages. Tests cover missed and normal terminal-event paths.

Explanation

Active direct issue #25639 requires stale delegated-question wakes to stop when the child settles or the question is answered or closed. DelegatedTaskQuestionReactor cancels matching queued wakes for those events and logs cancellation failures. Orchestrator.startNextQueuedRun removes stale wakes for missing or settled tasks and non-pending requests. It retains a wake when the child projection read fails. dispatchMessage accepts a stale wake as a no-op. The added control-read tests cover live settlement, missed terminal events, follow-up questions, and direct stale dispatch. These changes prevent a stale notification-only run from replacing the parent result while preserving valid question wakes.

Full details: Out of Scope Changes check

Explanation

The reactor changes manage delegated-question wake lifetime. The orchestrator changes validate and suppress stale wakes. The added tests verify the linked issue behavior, including restart or missed-event recovery and valid follow-up questions. No unrelated feature, configuration change, or unrelated refactor is identified.

Return only when no user_input request exists.

Do not return merely because no request is pending. If a resolution event was missed, a closed request can still have a queued wake. startNextQueuedRun provides a fallback, but it returns while a blocking run exists before it calls cancelStaleQuestionWakes. The terminal event must still cancel that wake immediately.

This still avoids the runs and nodes scans when the child has no user-input requests.

Limit the stale-wake guard to question-wake messages.

isStaleQuestionWake treats every delegated_task notification with outcome updated as stale unless a pending user_input request matches its message ID. It does not first check that the message ID is a question wake. If a notification with this shape is dispatched as a message, dispatchMessage and cancelStaleQuestionWakes drop it. Add a prefix check before the source test:

Suggested fix
diff
1+ if (!messageId.startsWith("message:delegated-task-question:")) return false;
2 if (notification?.source.kind !== "delegated_task" || notification.outcome !== "updated")
3 return false;

The question-wake message ID format is also built in DelegatedTaskQuestionReactor.ts (wakeParent and cancelQueuedWakes) and compared in this guard. Export one helper for the format and use it in all three places, so the copies cannot drift apart.

  • perf(server): skip child run and node reads when a settled task asked nothinge041743

@coderabbitai Outside-diff item (DelegatedTaskQuestionReactor.ts:146-166): fixed in e041743849. The settled-task branch now reads only runtimeRequests first and returns when the child has no user_input request. Otherwise it reads runs and nodes and cancels as before.

Nitpick (isStaleQuestionWake prefix check): not changing. DelegatedTaskQuestionReactor.wakeParent is the only producer of a delegated_task notification with outcome updated; completion notices use other outcomes and are not stored as messages with that source. A prefix guard and a shared id helper would add code without changing behaviour.

@juliusmarminge Confirmed in the current source: the settled-task branch reads runtimeRequests first and returns when there is no user_input request. It reads runs and nodes only when needed. That addresses the outside-diff concern.

I withdraw the prefix-check nitpick. wakeParent produces the delegated_task notification with outcome updated, which isStaleQuestionWake already checks. The extra prefix guard and shared ID helper are unnecessary here.
