Problem

A nonreturning delegated-tasks.stop holds its thread's effect queue indefinitely. Replaying that stop after a server restart can block later provider starts again. A controlled hang in the real dev server reproduced both cases: the stop remained running after lease expiry and new starts stayed pending with zero attempts.

Change

Limit each delegated stop attempt to 30 seconds and use the existing retry/fail path. Once the retry limit is reached, the stop fails and later work can start. Effect serialization remains intact, and other effect types retain their current execution behavior.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/25638

Triaged bug via the automated issue flow; GitCafe mirror #25636.

Verification

  • Regression proof: both fresh and restart-replay cases fail with the base worker and pass with the fix. Red output, green persistence suite: 46 passed.
  • Final merged-head scoped tests: 120 passed in 5 files (persistence, effect worker, thread stop, background-work stop and thread management). Test output.
  • Final vp run check:changed: format, lint, knip, GraphQL, affected-package typecheck and adjacent tests passed. Current merged-head check output.
  • Real dev server, Chromium web composer, isolated state, Codex provider. A controlled interruptible stop hang used unchanged production timeout/retry settings. Implementation evidence was captured at 8cee7bba3b.

The original production stop cause remains unconfirmed. Proof covers a controlled interruptible downstream hang; it does not cover uninterruptible regions, synchronous blocking, or hanging interruption finalizers.

Server-only change; the visual fixture matrix is empty. No client, contract, provider-adapter or connection-mode-specific behavior changed.

Final head c75478e86f merges current main. The two changed files are byte-identical to the reviewed implementation and runtime evidence. Independent GPT-6.1-Sol and Claude Opus 5.5 reviews returned MERGE; proof audit returned SUFFICIENT.

Takeover

Landed as-is from @juliusmarminge's change: EffectWorker.ts is byte-identical to the reviewed implementation. The regression test now also asserts the hung stop ends failed with a "Timed out" error, so a timeout that marked the stop as succeeded would not pass. The replay case's setup check is reduced to one line. No code changes since the last merge of main (802efb97da); the evidence below is recaptured at exactly that head against its merge-base.

Evidence

Server-only change, so there are no screenshots. Each row is a time-ordered transcript of the same driver on fresh databases created by each side's own server (T3CODE_DEV_SEED=0): server log, orchestration_v2_effect_outbox transitions (attempts, leases, errors) and persisted provider/message/run/checkpoint events. Production timeout, retry and backoff settings are not overridden. Hangs are injected only into scratch checkouts, and each transcript states its exact injection diff. Verification log

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The server change introduces a hard-coded 30-second timeout for all delegated stop effects, after which existing retry/fail handling runs; the regression coverage is focused and includes restart replay. Because this establishes a new product operational default, the change requires human review.

Walkthrough

delegated-tasks.stop executions now have a 30-second timeout. Timeout errors follow the worker’s existing retry-or-fail path. Tests cover hung stops in live and replay modes and verify that a later same-thread provider turn starts.

Explanation

The whole-PR diff against the merge base (96fc7b9551) changes EffectWorker.ts (+24/-1) and FoundationPersistence.test.ts (+88). The EffectWorker.ts change adds the timeout helper boundExecution and DELEGATED_TASKS_STOP_TIMEOUT. It is gated on delegated-tasks.stop and leaves all other effect types on their existing execution path. The test file adds regression coverage for the fresh and replayed stop cases. Both changes connect directly to the stop-timeout objective in [#25638]. No unrelated client, contract, provider-adapter, or connection-mode change appears in the reviewed summary. The file list was truncated in the repository tool output, so completeness rests on the supplied PR summary.

Full details: Linked Issues check

Explanation

[#25638] The change satisfies the delegated-stop case. In EffectWorker.ts, boundExecution applies a 30-second Effect.timeoutOrElse only when effect.request.type === "delegated-tasks.stop". The timeout fails with OrchestrationEffectExecutionError ("Timed out after 30s."), which uses the existing retry/fail path. FoundationPersistence.test.ts covers fresh and replayed stops and checks that a later provider-turn.start runs. [#25638] The issue also requires that an effect whose lease has expired is reclaimed or failed, so it does not block its thread. This PR does not meet that general requirement. EffectOutbox.ts is unchanged between the merge base and head. claimableCandidatePredicate still does not check lease_expires_at. Running effects of other types (for example provider-turn.start) still have no expired-lease reclaim or failure path. The 30-second bound is an in-process execution timeout, not a lease-expiry rule. The PR author states that general live reclaim is intentionally excluded because lease expiry does not prove execution stopped. That is a design position. It does not change the issue requirement.

Resolution

Either (a) implement expired-lease handling for running effects beyond delegated-tasks.stop, for example in the EffectOutbox.ts claim predicate or reconciliation path, and add tests showing that later same-thread work starts after lease expiry and after restart; or (b) have the issue owner amend #25638 to limit the expected behavior to delegated-tasks.stop effects. After (b), reassess this check against the amended text.

Reviewed the linked-issue scope warning. This fix targets the reported hung delegated-tasks.stop, including replay after restart. Both regression tests and the runtime transcript show later same-thread provider work proceeding after the existing retry limit. General live expired-lease reclaim is deliberately outside this fix: lease expiry does not establish that execution has stopped, and allowing later work to overlap could violate thread serialization (including rollback ordering). The PR states the controlled-hang proof and its limits. No broader reclaim change is needed for this reproduced case.

Assert stop terminalization before the provider turn starts.

The test only checks that provider-turn.start eventually starts and that delegated-tasks.stop eventually fails. It does not check the stop status when turnStarted fires. Add that ordering assertion inside the executor before signaling turnStarted.
