Problem

A stalled MCP registration could fail a resumed Claude turn without retrying. SDK disposal can also return before the old process exits, allowing a replacement to overlap it.

Change

Retry registration once when reopening an existing session after the 90-second registration deadline. On failed query initialization, stop the owned child and confirm process exit within ten seconds before a retry can start. Fresh session IDs are not retried because they may already have a transcript.

Track process exit through the existing POSIX and custom spawners, and use an explicit Windows spawner with hidden windows and stderr forwarding. Preserve the dedicated registration-timeout message through the shared provider failure formatter so clients receive the specific cause.

Scope and approval

This changes production Claude process startup, termination, disposal, and retry behavior across platforms. Maintainer approval remains required, as recorded in Macroscope's approvability review.

Verification

vp run check:changed passed, including 254 tests across three files and typechecking of 21 affected packages. Regression coverage verifies retry and prompt delivery with the original MCP configuration, repeated timeout reporting, cancellation cleanup, fresh session behavior, confirmed process exit, and refusal to retry when exit cannot be confirmed. CI is green on 9ced219.

Implemented with GPT 6.1 Sol through the Codex harness.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — The PR fixes stalled resumed Claude MCP registration and adds thorough recovery tests, but it also changes cross-platform Claude process spawning, termination, disposal, and retry behavior on the production path. The process-lifecycle changes are substantial enough to require human review.

Walkthrough

Claude query opening now handles stalled MCP registration with a dedicated error and cleanup for failed initialization. It retries once for resumed sessions after confirmed process exit. Fresh sessions do not retry.

Changes: Claude MCP registration recovery

Layer / File(s)Summary
Timeout handling and query cleanup
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, packages/provider-core/src/server/failure.ts
The query runner configures process spawning, reports MCP registration timeouts, and shuts down the prompt queue when initialization fails. It kills a running child process, waits up to 10 seconds for exit, and disposes the query runtime. Provider failure formatting returns the timeout message.
Resumed-session retry
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAgentSdkQueryRunner.test.ts
The query runner retries once after a timeout only when resuming an existing session. Tests cover retry after process exit, repeated timeouts, interruption, prompt delivery, and no retry for a fresh session.

Explanation

The title "fix(server): retry stalled MCP registration for resumed Claude turns" matches the main change. The diff adds one retry for stalled MCP registration when a resumed Claude session opens, plus a dedicated timeout error and cleanup of the spawned process. The title is short, uses a conventional commit prefix, and names the affected path. It does not cover the Windows process spawner or the cleanup details, which is acceptable.

  • fix(server): await Claude process exit before retrying startup10ca568

Note

Generated by GPT 6.1 Sol

I verified the process-cleanup caveat against the pinned SDK. Its async disposal waits at most two seconds for process exit, so awaiting disposal alone could start the retry while the old process was still alive.

Fixed in 10ca568677. Failed startup now stops the owned child and awaits its exit before retrying. If exit cannot be confirmed within ten seconds, no replacement starts. The new regression failed on the previous head and now covers both confirmed exit and a child that never reports exit. All changed-file checks passed, including 254 tests.

The retry registers the same MCP servers again; it does not drop them.

Assert the MCP servers passed to the retry registration.

ClaudeAgentSdkQueryRunner.open intentionally removes mcpServers from the SDK query options and registers them with queryRuntime.setMcpServers. The test currently asserts that call.options?.mcpServers is undefined, but it does not assert the arguments to secondRegistration. A retry that skips MCP registration could therefore still pass while delivering the prompt.

Addressed the test coverage nitpick in 9ced219403. The existing retry regression now asserts that the replacement registers the original MCP server configuration exactly once. All changed-file checks passed, including 254 tests.
