Problem

A Claude worktree handoff can close its session while a turn is streaming. Subsequent cleanup tries to interrupt the already closed CLI and raises ProviderAdapterInterruptError with ProcessTransport is not ready for writing.

Change

Mark the query closed when the session scope closes it, and skip transport writes when interrupting a query already marked closed. Add one regression test for cleanup after session teardown. Interrupt failures from live queries still propagate.

Scope and approval

Fixes pingdotgg/t3code#25146

Triaged bug via the automated issue flow; GitCafe issue #25146. This fixes the closed-transport interrupt failure only. The separate planned-detach stream failure and queued continuation tracked in #22971 still reproduce.

Verification

  • Red first: the new regression failed on main with the same closed-transport ProviderAdapterInterruptError. Red output.
  • All 236 tests in ClaudeAdapterV2.test.ts pass. Post-main-merge vp run check:changed passes format, lint, knip, GraphQL, affected typechecks and tests. Check output.
  • Real app on Linux x64, Claude Opus 5.5: a fresh thread calls t3_worktree_handoff with a continuation prompt in a throwaway Git project. Before (f89855713d), cleanup interrupt fails; after (fddeb88fef, including main 44df336016), the interrupt span succeeds. Before transcript, after transcript.
  • Two independent reviewers (GPT-6.1-Sol and Claude Opus 5.5) found no confirmed issues. This server-only change has no changed render paths; the capture matrix is empty.
  • Evidence index independently checked against the runtime logs.
  • The missing-projection spans observed in reproduction came from the expected thread-creation existence probe. No projection behavior changed.

Models: Claude Opus 5.5 (test and implementation), GPT-6.1-Sol (triage and verification), through T3 Code using Claude Code and Codex harnesses.

Approvability

Verdict: Approved at fddeb88

Macroscope's review found this PR approvable — This is a narrowly scoped Claude session-teardown bug fix: cleanup now records closed queries and avoids interrupting an already-closed CLI, while live-query interruption remains unchanged. The added regression test directly covers the failure scenario without introducing new capabilities or configuration changes.

Walkthrough

ClaudeAdapterV2 now checks whether a query is already closed before interrupting or closing it. Session cleanup marks the query closed after closing it. A regression test covers an interrupt requested after session-scope cleanup.

Changes: Claude query cleanup

Layer / File(s)Summary
Synchronize query closure and interruption
apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts, apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts
Session cleanup completes the query’s closed deferred after closing the query. interruptTurn skips interrupt and close when that deferred is already complete. The regression test covers a runtime-restarting interrupt after session-scope cleanup. The test harness allows the simulated interrupt to fail with ClaudeAgentSdkQueryRunnerError.

Explanation

The changed files are limited to ClaudeAdapterV2.ts and its regression test. The implementation changes session-close and interrupt behavior that directly addresses [#25146]. The test validates cleanup after session teardown. No unrelated production behavior or unrelated tests are changed.

Full details: Linked Issues check

Explanation

Direct issue [#25146] expects both a clean interruption and a successful projection lookup for the handed-off thread. This PR implements the interruption fix: session cleanup completes existing.closed, interruptTurn skips transport writes for closed queries, and the new regression test covers cleanup after scope teardown. The diff does not change ProjectionStore or projection synchronization, and the PR states that projection behavior did not change. Therefore the projection requirement remains unmet.

Resolution

Implement and test the projection lookup requirement from [#25146], or split that requirement into a separate linked issue and update the PR linkage and scope. Keep the existing closed-query interrupt regression coverage.

Serialize interruption with query closure.

If Stop runs while closeSession is closing the query, existing.query.close can close the transport before existing.closed is completed. Deferred.isDone then returns false, and existing.query.interrupt can write to the closed transport. The new test covers Stop after scope closure, so it does not cover this overlap. Serialize the interrupt and close operations so neither can write during transport teardown.
