Problem

Sending a Claude steer while foreground Bash is running immediately shows “Steered the active turn,” even when Claude has not accepted the message. The thread gives no delivery feedback during that wait.

Change

Give each Claude steer a UUID and a notice that stays waiting until a matching acknowledgement arrives. Mark accepted steers as informational; report an unconfirmed steer when the turn ends or the server restarts, and preserve failed-send notices. Web and mobile understand the optional notice severity while existing stored notices keep their warning presentation.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/23948

Triaged bug via the automated issue flow. GitCafe: #23948

Verification

  • Focused tests after merging current main: 322 passed across ClaudeAdapterV2.test.ts, ProviderRuntimeRecoveryService.test.ts, apps/web/src/session-logic.test.ts, and apps/mobile/src/lib/threadActivity.test.ts. Regressions cover waiting and selective acknowledgement, terminal notices, failed sends, recovery, and severity compatibility.
  • Scoped typechecks passed for server, web, mobile, and contracts before the main merge; none of the merged changes overlap these changed files. Targeted lint found only the existing unused layer warning. Two independent code reviews found no confirmed defects; a fresh proof audit found the scoped evidence sufficient.
  • Real Claude Opus 5.5 / CLI 2.1.303 in the web app: two distinct steers stayed waiting during a three-minute foreground command; matching UUID lifecycle events changed both to accepted. Stop changed a waiting notice to unconfirmed. Crashing and restarting the isolated server produced a durable restart notice that survived reconnect, reload, and disclosure open/close.
  • Captured baseline b3d382e100, captured head 9031bd1140. Final head 72af7dfd65 merges main; its 10-file diff is byte-identical to the captured/reviewed diff. Post-merge tests passed again.

Same foreground command and steer, wide dark web at 1280×800 CSS pixels. Full frames differ in generated thread titles, timestamps, and elapsed timers; affected timeline crops are additional evidence. No unrelated pixel-equivalence claim is made.

Recordings: Before, After, two-steer acceptance, Stop, restart/reconnect/reload.

Evidence report with actions, UUIDs, timestamps, environment, and limitations · Complete captures and sanitized event logs.

Unverified: native mobile rendering (current iOS simulator builds fail on missing GhosttyKit), Electron shell, and narrow layouts. The web renderer shared by desktop was exercised. Both baseline and head commands completed; this PR does not claim to fix a permanent CLI stall or change command interruption behavior.

Models: Claude Opus 5.5 through the Claude Agent harness; GPT-6.1 Sol through the Codex harness.

During reconcile("shutdown"), a pending Claude steer is persisted with a restart explanation even though the server was only shut down, so the recovery notice misreports why it was cancelled. cancelledItem hard-codes this message; use wording that covers both shutdown and restart.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This change adds a stateful Claude-steer lifecycle with new persisted notices, acceptance correlation, recovery behavior, and web/mobile presentation across the existing production path. The cross-component runtime impact is substantial enough for human review.

Walkthrough

The PR adds severity to system notices and updates mobile and web presentation accordingly. Claude steers now have UUID-based status notices that reflect offer failures, acceptance, turn completion, and runtime recovery.

Changes: Steer notice flow

System notices accept optional info or warning severity. Mobile uses a zap icon for informational notices; web marks notices as runtime warnings unless severity is info. Tests cover both behaviors and the default severity.
Claude steer lifecycle

Steers receive prompt UUIDs and waiting notices. Matching started events or echoed UUIDs mark them accepted. Failed offers receive a failed notice; steers still pending at turn end are marked unconfirmed. Result attribution also recognizes steer UUIDs.
Steer notice recovery
apps/server/src/orchestration-v2/ProviderRuntimeRecoveryService.ts, apps/server/src/orchestration-v2/ProviderRuntimeRecoveryService.test.ts
Recovery cancels waiting system notices. Recognized Claude steer notices receive the “Steer not confirmed” title and warning severity; other notices retain their existing title and severity.

Full details: Description check

Explanation

The description includes the required Problem, Change, Scope and approval, and Verification sections. It explains the behavior, linked issue, focused tests, manual evidence, screenshots, recordings, and verification limits.

Full details: Linked Issues check

Explanation

The changes satisfy the coding requirements in [#23948]. ClaudeAdapterV2 assigns each steer a UUID and emits a waiting system notice. Matching started lifecycle data or a matching echoed UUID changes the notice to accepted. Turn finalization changes pending steers to unconfirmed, and failed offers receive a failed notice. The existing command activity remains visible because the steer state is an additional system notice. Recovery changes pending steer notices to an unconfirmed warning after a restart. The contract, web projection, and mobile activity mapping support informational accepted notices while preserving warning behavior for notices without severity. The added tests cover waiting, selective acknowledgement, terminal states, recovery, failed sends, and severity compatibility.

Full details: Out of Scope Changes check

Explanation

The ten changed files support [#23948]. Server changes implement steer UUID tracking, acknowledgement state, terminal handling, and restart recovery. Contract, web, and mobile changes render the new optional notice severity. The test changes cover these behaviors. No unrelated change is demonstrated.

Explain why markSteersAccepted uses Effect.fnUntraced.

markSteersAccepted is an Effect.fnUntraced helper that runs for each SDK message. The checked-in Effect guidance requires an explanation beside deliberate fnUntraced use. The comment inside the body explains acceptance semantics, not the tracing choice, so add a short reason above the function.
