Problem

After stopping an OpenCode turn, fork an earlier assistant response and send a message before the interrupted execution's end event reaches T3. The fork can fail with Cannot fork OpenCode session ... while it is still working, although OpenCode has stopped writing to that session.

Change

When the local active turn is marked interrupted, check OpenCode's active-session list before refusing the fork. If the source is idle, settle the stale interrupted turn under the adapter lock before proceeding. Sessions still running remain blocked, and rollback retains its background-work guard.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/24549

GitCafe: #24549

Triaged bug via the automated issue flow. This fixes one provider-state race without changing contracts, settings, or rendering. The reporter also opened #24550; that PR remains untouched. This implementation additionally settles the stale turn before history mutation.

Verification

  • Regression test fails before the implementation with the reported protocol error; red output. Command from apps/server: vp test run src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts -t 'interrupted event never arrived'.
  • vp test run src/orchestration-v2/Adapters/OpenCode2AdapterV2.test.ts: 80 passed, including refusal while OpenCode still reports the source running and existing rollback snapshot/serialization tests. Final-head output.
  • Scoped server and provider-opencode typechecks: zero errors. Lint on both changed files and formatting passed.
  • Two independent diff reviews (GPT-6.1-Sol and Claude Opus 5.5): MERGE, no blocking findings.
  • Real web app on Linux, isolated dev state, OpenCode 2.1.2 with Claude Haiku 5.5. A forwarding proxy drops only session.execution.interrupted frames: before, Stop → Fork → Send fails; after, the fork replies fork works and its source is Stopped. Normal-delivery control also succeeds. Before transcript, after transcript, proxy driver.

No render code changed; the proof audit has an empty render matrix. Screenshots below illustrate the runtime outcome, without visual-diff claims. Evidence index and limitations. Desktop/mobile were not exercised separately; they use the same server adapter. Missing-event rollback recovery was not separately exercised at runtime.

Implemented/tested with Claude Opus 5.5; led, runtime-verified, and reviewed with GPT-6.1-Sol in T3 Code (Claude Code and Codex harnesses).

🟠 High v2/adapter.ts:2923

A delayed session.execution.interrupted from the forcibly settled turn can mark the next turn interrupted, causing its prompt to be skipped. finishTurn clears state.active here without preserving an execution barrier, so route treats the stale end as belonging to the replacement turn; preserve the stale-execution/start barrier when settling this turn.

Approvability

Verdict: Would Approve

Macroscope's review found this PR approvable — This is a localized OpenCode session-state bug fix with regression tests covering both safe forking after a lost stop event and refusal while OpenCode is still running. A separate unresolved high-severity finding flags a possible stale interrupted-event race that independently requires attention.

Walkthrough

The adapter now checks OpenCode’s active sessions when a turn is interrupted. If the source session is idle, it clears the busy marker and finishes the interrupted turn. Regression tests cover idle and still-active sessions.

Changes: OpenCode stopped-session fork handling

ensureQuiet checks OpenCode activity for interrupted turns. If the session is idle, it clears the busy marker and finishes the interrupted turn under the event lock. If the session remains active, the protocol error remains. Tests cover both outcomes.

Full details: Linked Issues check

Explanation

Directly linked issue #24549 has one active coding requirement: when the local turn is marked interrupted, consult OpenCode's active-session list before refusing a fork, proceed if the source is idle, and keep blocking sessions still running. Its regression test requirement also applies. The whole-PR diff of packages/provider-opencode/src/server/v2/adapter.ts (ensureQuiet) meets the behavior requirement. (1) stopped is set only when state.active.interrupted === true, so only stopped turns bypass the local busy and state.active checks. (2) For a stopped source, busyThere calls client.session.active() and blocks with ProviderAdapterProtocolError (Cannot ${verb} OpenCode session ... while it is still working) only when the source is listed. (3) When the source is idle, busy.delete runs and finishTurn(state, { status: "interrupted" }) settles the stale turn under lock.withPermit, guarded by state.active === stopped. The regression tests are described in the whole-PR summary as: a stopped session forks when session.active omits it, even with no interrupted event; and forking fails with ProviderAdapterProtocolError when session.active still lists it. The test file diff could not be read because its changed blob is missing from the checkout, so test content rests on the summary and the PR's red/green run record. The PR description also reports the red run before the fix and 80 passing tests after it. Rollback keeps its hasBackground guard, because busyHere still checks it for verb === "roll back".

Full details: Out of Scope Changes check

Explanation

The whole-PR summary lists two changed code files. Both serve the linked issue #24549. The adapter change is limited to the ensureQuiet fork/rollback busy check and the stopped-turn settlement under the existing adapter lock. The added tests cover the fork-while-idle and fork-while-running cases. The rollback background-work guard and the existing ProviderAdapterProtocolError path are unchanged in behavior. The PR states that no contracts, settings, or render code changed, and the diff shows no changes outside ensureQuiet. Limits: the test file diff was unavailable, and the full changed-file list was not confirmed from a diff. Scope conclusions rely on the whole-PR summary and the adapter diff.

Do not clear a newer execution’s busy marker.

busy.delete(sessionId) runs outside the event lock. ensureQuiet awaits session.active() before acquiring that lock, so a new execution-start event can add sessionId to busy during that gap. The locked check only compares state.active with stopped; it does not re-check the busy marker or an execution generation. ensureQuiet can therefore succeed, and forkThread can copy source history while the new execution writes to it.

Track the execution-start generation across the activity check. If it changes, re-check activity or refuse the fork before clearing busy.
