Problem

A server restart cancels a yielded root Codex command after its agent turn has ended, but loses the command's promised completion notification. The caller stays idle instead of receiving a restart continuation. Reproduced on main with Codex CLI 0.163.0: start sleep 180, end the turn while it runs, then restart the isolated server.

Change

Persist an optional completion-notification ownership marker on root Codex command items when continuation support is available. Carry that marker into restart cancellation notes and use it in the existing awaited-work predicate, preserving the existing eligibility and duplicate-delivery guards.

This includes yielded Codex dev servers, whose normal completion already wakes the caller. Unmarked commands and existing records retain their current behavior.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/24503

GitCafe: https://git.cafe/pingdotgg/t3code/issues/24503

Triaged bug via the automated issue flow. This repairs an existing Codex completion-notification promise; it adds no setting or new workflow. The existing author's proposal #23585 remains untouched.

Verification

  • Red test committed before implementation: recovery cancels the command but produces no provider-runtime.continue effect. Failing output.
  • 146 focused tests passed across the Codex adapter, restart continuation, restart background notes, and provider runtime recovery suites. The two changed test files passed all 126 tests in the evidence run. Green output.
  • Server and contracts typechecks passed. Targeted lint passed with two existing unused-variable warnings.
  • Independent GPT-6.1-Sol and Claude Opus 5.5 reviews found no blocking findings.
  • Real server and web client on Linux fs-05, Codex CLI 0.163.0 / GPT-6-Astra, isolated worktree state. No client rendering changed; visual fixture matrix is empty.

Command cancelled after restart; completed caller gets no continuation. Runtime transcriptOwnership persists through cancellation; exactly one continuation references the source run. Codex receives the restart note and responds by restarting the command. Runtime transcript and persisted JSON

Hosted CI and bot review remain pending. Historical unmarked commands are not backfilled. Other providers are outside this Codex-specific repair.

Implementation and red test: Claude Opus 5.5 through T3 Code's Claude harness. Triage, integrated verification, proof audit, and independent review: GPT-6.1-Sol through the Codex harness; additional independent review: Claude Opus 5.5.

Approvability

Verdict: Approved at ae08fca

Macroscope's review found this PR approvable — This is a focused Codex restart-recovery bug fix: commands that already have a provider-issued continuation are now marked so recovery can wake the agent, while ordinary shell commands retain their existing behavior. The contract additions are optional and covered by targeted adapter and recovery tests.

Walkthrough

Root Codex commands with continuation support now carry completionWake: "always" into restart recovery. Recovery treats marked shell work as awaited. Tests cover late command updates and enqueueing a continuation effect after recovery cancels a running command.

The wakeup path appears to work, but the new test could pass even if a future change causes the promised continuation to be skipped. Add an assertion for the persisted marker; this is a bounded follow-up rather than a demonstrated delivery failure.

Explanation

The description covers the problem, implementation, scope and approval, verification results, limitations, and agent attribution. It provides reproduction details, linked issue context, focused test results, and known pending checks.

Full details: Linked Issues check

Explanation

Issue #24503 requires one restart continuation for an eligible root native Codex command when restart recovery cancels the command after the agent turn. The supplied PR evidence states that eligible root Codex command items persist completionWake: "always", recovery carries the marker into cancellation notes, and isAwaitedWork uses the marker. The head code confirms marker propagation in cancelledTurnItemWork and awaited-work handling in isAwaitedWork. The reported RestartContinuation.test.ts recovery test verifies cancellation and a provider-runtime.continue effect. The reported Codex adapter replay test verifies the marker on running and completed command-item updates. Unmarked commands retain the existing behavior.

Full details: Out of Scope Changes check

Explanation

The reported changes are limited to Codex command-item marking, restart cancellation-note propagation, awaited-work classification, contract fields, and focused tests. These changes directly support issue #24503. The PR evidence states that other providers, historical backfill, new settings, and unrelated workflows remain unchanged. No unrelated production behavior is identified.

Assert the persisted cancelled-work marker.

The recovery test can pass if the queued effect is created from the in-memory item but the persisted run.background-work-cancelled event omits completionWake. The later continueRestartedRun call then returns without dispatching because isRestartContinuationSource does not treat an unmarked shell as awaited. Existing tests do not cover this persistence boundary.
