Problem

After a Codex turn finished in T3, an external codex queue message ran in T3’s app-server but its prompt, reply, tools, and requests never appeared in the thread. Reproduced on Linux with Codex 0.163.0: T3 stayed at READY while the native log recorded QUEUED and completion.

Change

Recognize externally started turns on known root Codex threads, hold their frames, and wake the existing continuation path. The resulting run adopts the native turn instead of starting another one. Replay preserves notification order and lets requests receive user responses without blocking startup. Rejected or cancelled wakes release buffered requests and reject later requests until native completion.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/23919 GitCafe: #23919

Triaged bug via the automated issue flow. This restores recording of work already running in T3’s Codex process. No settings, persisted fields, or client contracts change.

Verification

  • Red-first replay reproduced the dropped QUEUED output. Added focused coverage for adoption, approvals during startup, command-shaped prompts, rejected/cancelled wakes, terminal callbacks, and approvals arriving after rejection.
  • After integrating current main, vp test run src/orchestration-v2/Adapters/CodexAdapterV2.test.ts src/orchestration-v2/ProviderContinuationService.test.ts from apps/server: 97/97 passed.
  • Scoped server and provider-core typechecks: no errors. Targeted lint: no errors; two existing unused-variable warnings.
  • Three independent review rounds were completed. Confirmed findings were fixed, including the final late-approval ordering case, with a regression test.
  • Exact final-head real web app verification: external queue reply, resolved command output, native input request/answer/resume, and normal NEXT followed by reload all passed. Native/projection transcript confirms one original native turn and one T3 run for each external message, with no extra turn/start for adoption.

Queue and resolved tool recording · Request round trip, NEXT, and reload recording

1280×800 CSS, light theme, real web client. Zero-threshold diff and thresholded diff compare richer history rather than an isolated renderer change. Visible differences are the additional turns, request answer, notification rows, timestamps, and workspace sidebar state. No renderer changed. Final stability frames differ only by 16 composer-caret pixels.

Not exercised in the real app: explicit cancellation/late-approval ordering (covered by replay tests), command-approval family (real request proof uses native user input), Electron/native mobile, other providers, relay/tunnel, and the complete size/theme interaction matrix. Earlier 1440/390 light/dark stills are supporting evidence only; matching baselines and final-head interactions at those dimensions were not captured.

Proof audit: scoped A–D sufficient, with limitations. Independent same-state screenshot rebuild was attempted but redirected to pairing; no rebuilt pixel delta is claimed.

🟠 High Adapters/CodexAdapterV2.ts:4438

A child spawned by an external turn can complete before the parent is adopted, leaving the child projected as running with its transcript missing. handleCodexNotification buffers only frames whose turn ID is in heldExternalTurns; the child’s different turn ID falls through to ordinary handlers, which drop its item and completion events before its context exists. Buffer descendant frames too, or register their contexts before dispatching them.

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — This bug fix adds substantial cross-cutting Codex turn buffering, adoption, request handling, and continuation lifecycle logic across production paths. An unresolved High-severity finding also reports that descendant turns can still lose their transcript before adoption, warranting human review.

Walkthrough

Codex external root turns can now be offered as continuations and adopted by T3 runs. Notifications and requests are buffered until adoption. Continuation requests can track when the run for a specific message reaches a terminal status.

Merge Risk: External image Low · up to 73664

The change adopts Codex turns queued outside T3 and replays them. Remaining concerns are narrow: a failing settlement callback could leave a held turn's approvals unanswered until the session closes, and one new test assertion may pass without proving its point. The risk is bounded, so owners should be aware of both but neither blocks merge.

Full details: Linked Issues check

Explanation

Directly linked issue #23919 requires that turns started by codex queue on a loaded Codex root thread appear in T3, with the queued message, agent reply, and tool calls. Approvals raised during that turn must reach the user.

Evidence at head 73664cd3:

  • CodexAdapterV2.ts turn/started: an unmatched turn on a rootProviderThreads thread is now parked in heldExternalTurns. It no longer falls through to rememberSubagentTurnStarted. This addresses the root cause in the issue.
  • handleCodexNotification and handleCodexRequest wrap the existing handlers. Frames for a held turn are buffered in order. Requests are answered through a deferred and forked when the turn is adopted.
  • offerExternalTurnWake offers a provider continuation on the userMessage item or turn end. adoptExternalTurn registers the native turn as the T3 run's root turn and then replays the buffered frames. This makes the run adopt the native turn rather than start another one.
  • ProviderContinuationService.ts: onRunSettled is registered per MessageId before dispatch and invoked on the terminal run.updated. This releases frames if the woken run ends without adoption.

The issue asks for display and approval handling. Both are implemented on the adoption path. Rejected or cancelled wakes release buffered frames and fail later requests. This is the intended fallback, not a missing requirement.

Limits: the replay tests in CodexAdapterV2.test.ts and ProviderContinuationService.test.ts could not be read. The test diff was unavailable in the checkout. The 97/97 test result, typecheck, lint, and real-app results are author-reported and were not reproduced. The assessment relies on the implementation read above and the test descriptions in the PR summary. Issue #23919 does not require automated tests, so this does not change the verdict.

Full details: Out of Scope Changes check

Explanation

All reviewed code changes connect to the objective of recording externally queued Codex turns.

  • CodexAdapterV2.ts: the held-turn map, the notification and request wrappers, the wake offer, and the adoption path implement the objective. The MessageId import and the wrapping of existing handleServerNotification calls with handleCodexNotification are needed for the buffering. The wrapping only changes how handlers are registered. The existing per-event logic is retained.
  • ProviderContinuationService.ts: onRunSettledByMessage and the run-settled stream support the release of held frames. The clearIfCurrent call on non-delegated dispatch failure is a change to shared continuation behavior. It applies only to requests that supply clearIfCurrent. It supports the same held-offer lifecycle, so it is connected to the objective and is not an unrelated change. Reviewers should confirm that other continuation sources do not depend on the previous failure path.
  • ProviderContinuationRequests.ts: the optional onRunSettled property is the public hook for the same lifecycle. It is connected.
  • Test files: the replay tests and the onRunSettled test support the new behavior.

The PR description includes screenshots, GitCafe links, and proof-matrix links. These are evidence, not code changes. They do not create scope issues.

The diff output was truncated at 16000 bytes. The /goal and compaction adoption entry points mentioned in the summary were not inspected directly. The test file diff was unavailable. These uncertainties do not show unrelated changes.

Make the negative assertion wait for the stream to process the events.

assert.isFalse(yield* Deferred.isDone(settled)) runs right after the two Queue.offer calls. The forked stream fiber has probably not consumed those events yet, so the assertion passes even if a running status fires the callback.

The final Deferred.await(settled) also cannot catch that regression. If running fired the callback, the callback would already have resolved settled.

Make the negative check depend on the stream's progress. One option: register a second request with its own callback and message ID. Offer the two non-matching events for the first message. Then offer a terminal event for the second message, await the second callback, and only then assert that the first callback has not run. As per coding guidelines: "Test meaningful logic or observable behavior."

Use named spans for turn-level helpers and document the per-notification exemption.

offerExternalTurnWake runs for each held notification, so it may remain untraced with a nearby reason. releaseHeldExternalTurn and adoptExternalTurn run at turn-operation boundaries and have no tracing rationale. Add the rationale and use named spans for those two helpers.

Keep the settlement stream running after one failure.

The Stream.runForEach callback returns onRunSettled() directly. A defect in one callback fails the whole stream. A failure from streamDomainEvents also fails it. In both cases the catchCause logs once, and the forked fiber then ends.

No later onRunSettled callback runs after that. For Codex, this means a held external turn is only released when the session closes, if its woken run settles without adoption. Until then, its buffered approval requests get no answer.

Contain failures per callback, so one source cannot stop dispatch for all other sources. If an upstream stream failure is recoverable, resubscribe with a bounded retry schedule.
