Problem

An async delegated Claude child can finish its first turn while a background Bash command runs. When reviewers finish before that command, the command's exit clears the published roster before Claude's wake run is registered. The original task then completes with the stale interim result and never delivers the later final result.

Reproduced on Linux with Claude Opus 5.5: a background sleep 45, two delegated reviewers running foreground sleep 2 and sleep 3, and no parent polling or manual follow-ups.

Change

Keep previously published background tasks visible while the idle Claude session still owes their wake run, covering either ordering of the SDK's roster snapshot and task notification. The internal roster still clears immediately, preserving query replacement and background-work checks. Wake completion and existing process-reset paths release the published hold.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/24297 GitCafe: https://git.cafe/pingdotgg/t3code/issues/24297

Triaged bug via the automated issue flow. Changes are limited to the Claude server adapter and its existing regression tests.

Verification

  • Red test at c6008634a5: the adapter publishes an empty roster before the wake run starts. Failing output.
  • vp test run apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.test.ts: 211/211 passed on 0a26c94c75. Includes notification-only, snapshot-before-notification, and snapshot-after-notification orderings. Passing output.
  • Server typecheck: zero errors. Scoped lint: only the existing unused layer warning.
  • Fresh Codex and Claude reviews passed after fixing the snapshot-ordering finding. Independent proof audit matched the runtime transcript to persisted events and confirmed exactly one automatic final-result delivery.
  • Runtime environment: fs-01 Ubuntu Linux, isolated web dev server and browser at 1280Γ—800, Claude Agent with claude-opus-5-5; Codex parent. A parent restart before the final delegation is disclosed in the transcript; all four subsequent child runs completed without interruption.

Walkthrough

The Claude adapter now keeps previously published background tasks in the thread roster while idle, even after an empty SDK snapshot clears internal background-work state. The roster drops those tasks when the wake run starts. Tests cover different snapshot timings.

Retain published tasks through wake handoff

While idle, the adapter carries forward previously published tasks that remain recognized as opaque background tasks. Tests check that the internal roster can clear while the published roster retains the task until the wake run starts, across different empty-snapshot timings.

The fix closes the main race, but a user message sent while a delegated child's wake run is still pending can still let the child finish with a stale interim result. Handle that case or accept the narrower gap before merging.

Explanation

[#24297] The issue requires that a delegated Claude child's original task stay open until the background-exit wake run and the reviewer-driven runs finish, so the parent gets the final result. The cause is an empty published roster during the gap before the wake run registers. The reviewed head fixes that cause in ClaudeAdapterV2.ts. When the native thread is idle (input.activeContext === null), the adapter collects previously published tasks that are still known opaque background tasks (owedWakeTasks). emitProviderThreadRoster then appends them to pendingBackgroundTasks when they are not in the internal roster. The internal roster still clears, so query replacement and background-work checks are unchanged. Once the wake run starts, activeContext is non-null and the hold is no longer applied. Tests in ClaudeAdapterV2.test.ts cover the notification-only, empty-before-notification, and empty-after-notification orderings. Each test asserts that the published roster keeps the wake task until the wake run starts and that the final roster is empty. The existing test was updated so it asserts the published roster keeps the task while the internal state is cleared. The reviewer-run sequencing is not a separate unit test. It relies on the same hold and on author-reported runtime transcripts, which I did not inspect. The issue workaround is not an acceptance criterion. No non-coding requirement applies.

Explanation

The whole-PR diff against merge base 27e0c684c5 changes only two files: apps/server/src/orchestration-v2/Adapters/ClaudeAdapterV2.ts and its existing test file ClaudeAdapterV2.test.ts. The adapter change adds the owedWakeTasks input and the idle-only collection that implements the #24297 hold. The test changes add regression coverage for the three orderings and adjust the existing empty-snapshot test to the new published-roster semantics. The renamed test title is a direct consequence of that semantic change. No unrelated files, refactors, or documentation changes appear in the diff.

Keep owed wake tasks published through an intervening user turn.

startTurn accepts a normal user prompt when the wake run is queued on the same query. When that user turn finishes, finalizeActiveTurn publishes the cleared internal roster and removes the published hold. The delegated child can then finalize with its pre-wake result.

Keep the published task until the wake run handles it or the process resets. Add a regression test with an intervening user turn.
