When a run fails and a queued run targets the same provider instance, the V2 orchestrator holds the thread's queue (queueHeld) until a user message from a web/mobile client or the UI's queue.resume command releases it. An MCP orchestrator could neither see that hold (t3_queue_list returned only queuedRunId/text/truncated) nor clear it (no resume tool), so a dispatcher thread driven only by scheduled tasks, webhooks and t3_thread_send sat with 24 queued runs stuck for 8 hours. t3_queue_list now returns a thread-level held flag plus held per item, with a description that says a held queue will not start on its own, and the new t3_queue_resume tool dispatches the existing queue.resume command through the same writesThread mode/permission checks as t3_queue_cancel, returning { released } and surfacing the orchestrator's "Continue the limited thread before resuming its queue." rejection as a tool error. The tool is registered in t3McpToolPresentation (new queue-resume summary action) so clients label it like the other queue tools. Tested with a new apps/server/src/mcp/toolkits/thread/handlers.test.ts against a real orchestrator: the list shows held: true after a hold, resume clears it and the next queued run moves to starting, and resume on a usage-limited thread returns the rejection; vp test run on that file plus core.test.ts and t3McpToolPresentation.test.ts passes (19 tests), and tsc --noEmit passes for apps/server, packages/shared and packages/client-runtime.

Approvability

Verdict: Approved at 4068bcd

Macroscope's review found this PR approvable — Adds a narrowly scoped, explicitly invoked MCP tool to expose and resume held queues, reusing existing orchestration checks and preserving existing behavior unless called. The accompanying client changes are presentation-only, and the new server path is covered by focused tests.

📝 Walkthrough

The thread MCP toolkit now reports held queue state and provides t3_queue_resume to dispatch queue resumption and return a released count. Tests cover successful release and usage-limit rejection. Client runtime and shared presentation code recognize the new action.

Changes: Thread Queue Resume

Layer / File(s)Summary
Queue tool contract
apps/server/src/mcp/toolkits/thread/tools.ts
The queue-list tool reports held state. The toolkit registers t3_queue_resume with an optional thread ID and a released-count response.
Queue state and resume handling
apps/server/src/mcp/toolkits/thread/handlers.ts, apps/server/src/mcp/toolkits/thread/handlers.test.ts
Queue entries and queue-list responses report held state. The resume handler counts held queued runs, dispatches queue.resume, and returns the count. Tests cover release behavior and usage-limit rejection.
Queue resume action presentation
packages/shared/src/t3McpToolPresentation.ts, packages/client-runtime/src/t3ToolSummary.ts, packages/client-runtime/src/work-log/presentation.ts
Shared presentation and client summaries recognize queue-resume, including its labels, summary text, and work-log priority.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Do not report a held queue as resumed when released is zero.

If an agent calls t3_queue_resume again after the hold is cleared, the command succeeds with { released: 0 }. This case still displays “Resumed 1 held queue.” Use the result count to give successful no-op calls an accurate label. As per coding guidelines, “Our users drive agents all day and notice a dropped frame, a lying spinner, and a stale label.”
