fix(server): show why Claude refuses full access as root#23804

Problem

On Linux, run the T3 server as root, create a Claude thread in Full access, and send a message. Claude Code refuses to start, but T3 reports only “Provider turn failed.” instead of the CLI’s explanation.

Change

Recognize Claude’s known root-refusal diagnostic in a bounded startup stderr tail and return a fixed message through the existing provider-failure path. Other stderr stays out of the user-facing failure. The turn now explains that --dangerously-skip-permissions cannot be used with root/sudo privileges for security reasons.

Scope and approval

Fixes https://github.com/pingdotgg/t3code/issues/15882

Triaged bug via the automated issue flow: #15882.

Verification

  • Regression test failed before the fix because the failure message omitted the CLI refusal; it passes after the fix through a real subprocess and query runner.
  • Before the review follow-up, after merging main at 6ecda6b491, the focused query-runner and Claude adapter suites passed 211 tests, and the provider-failure suite passed 10 tests.
  • Post-merge server test output and provider-failure test output.
  • The review follow-up adds a regression that first failed by exposing a private path, then passed after restricting diagnostics to the fixed root refusal. The error retains a required underlying cause.
  • At e1dfe9f9b3, the focused runner/adapter suites passed 212 tests and the provider-failure suite passed 10 tests. Both fresh full-diff reviewers returned MERGE.
  • Final test evidence and provenance · Final combined evidence. The real-app runtime transcript is tied to a1a19e01b9; the final root-refusal message is identical, with unrelated stderr now excluded.
  • Server and provider-core typechecks passed. Targeted lint reported one existing unused-variable warning.
  • Two independent reviewers returned MERGE; proof audit returned SUFFICIENT.

Approvability

Verdict: Approved at a480e86

Macroscope's review found this PR approvable — The change explains Claude’s existing refusal to run full access as root without relaxing permissions. Stderr capture is bounded, and only the recognized, fixed explanation is surfaced rather than arbitrary provider output.

Walkthrough

The Claude query runner captures up to 2,048 characters of CLI stderr before its first SDK message. If startup fails with the recognized root-permission refusal, it reports a typed error that provider failure handling preserves.

Changes: Claude startup diagnostics

The process-group spawner pipes stderr to an optional callback. The query runner retains a bounded stderr tail until the first SDK message.

Full details: Title check

Explanation

The title names the main change: Claude's root refusal for full access is now surfaced to the user. It is specific, follows conventional commit form, and matches the description and linked issue #15882. The change is a diagnostic message on the server side, so the title is accurate.

Full details: Description check

Explanation

The description follows the repository template. It has Problem, Change, Scope and approval, and Verification sections. The problem and reproduction steps are clear, and the change is explained. Scope links the triaged issue #15882. Verification gives concrete test counts, a before/after regression result, a runtime proof table, typecheck and lint results, and provenance for the models and harnesses. The description is long and includes some review-process detail, but every required section is filled in with specifics.

Full details: Linked Issues check

Explanation

[#15882] is the only active direct link. Its expected behavior allows either Full access shown as unavailable or the turn failing with the CLI's own explanation. The PR implements the second option. In ClaudeAdapterV2.ts, non-Windows spawns now pipe stderr into appendStderr, which keeps a 2,048-character tail. The query runner handles a stream failure: while no SDK message has arrived (answered is false), it waits up to 1 second for stderr to close, then checks the tail for CLAUDE_ROOT_REFUSAL. On a match it wraps the failure in ClaudeCodeRootRefusalError. Its message is Claude Code exited before it started: --dangerously-skip-permissions cannot be used with root/sudo privileges for security reasons. packages/provider-core/src/server/failure.ts lists this error tag in curatedCauseMessages, so the message reaches the user. The wait on stderr close resolves the ordering uncertainty noted in the earlier review. The regression tests in ClaudeAgentSdkQueryRunner.test.ts run a fake CLI through a real subprocess. One test asserts the refusal text appears in the failure. A second asserts that a token and file path in other stderr do not appear. The PR does not set IS_SANDBOX=1 and does not change permission modes, which the issue asks it to avoid. The retry symptom (No conversation found) is tracked in #15854 and is not an expected behavior of this issue. Residual notes: the Windows branch passes no stderr callbacks, so it does not surface the refusal. Root is a Unix concept, and the issue reports a Linux host. The PR's runtime transcript and test output are external links; this review did not run them.

Full details: Out of Scope Changes check

Explanation

The whole-PR diff against the merge base contains only changes that serve the root-refusal diagnostic. ClaudeAdapterV2.ts adds Duration import, the stderr tail constants, the onStderr and onStderrClose parameters to spawnClaudeProcessGroup (defaults keep existing callers working), the ClaudeCodeRootRefusalError class with a required cause, and the Stream.catch wrap that replaces the original fromAsyncIterable error mapping. The answered flag stops stderr capture after the first message. failure.ts adds one allowlist case. ClaudeAgentSdkQueryRunner.test.ts adds the stream import and two regression tests that cover the same behavior. No unrelated product, permission, or UI change appears in the diff. The Windows spawn path changes only in that it was not given the callbacks; its behavior is otherwise unchanged. The PR description's issue-flow and triage references are administrative and add no code.

Addressed both CodeRabbit findings in e1dfe9f9b3: only the known root refusal produces a fixed user-facing diagnostic, arbitrary stderr is excluded, and the underlying cause is required. The added regression failed on the prior head by exposing a private path; all 222 focused tests now pass. Both fresh full-diff reviewers returned MERGE.

Synchronize stderr before classifying the startup failure.

The SDK sets its exit error from the child exit event. Node allows child stdio streams to remain open after exit. The iterator can therefore fail while stderrTail is still empty, so the known refusal can become the generic provider failure. The real-subprocess test writes stderr before exit 1, but it does not enforce this event ordering.

Wait for stderr stream completion before checking stderrTail.

✅ Confirmed as addressed by @juliusmarminge

Fixed in a480e86c786: spawnClaudeProcessGroup now reports stderr close, and a stream failure before the first message waits for it (bounded at 1s; the group is SIGKILLed on exit so it closes promptly) before checking the tail for the root refusal.

@juliusmarminge The supplied code addresses this finding. Before the first SDK message, stream failures now wait for stderr close, with a 1-second timeout, before checking stderrTail. The answered guard and cause-preserving ClaudeCodeRootRefusalError wrapping remain intact.

Add coverage for SDK failure before stderr delivery.

Make the real child wait on its stdin before writing rootRefusal. In the mocked ClaudeSdk.query, start that child through the default options.spawnClaudeCodeProcess, schedule the stdin release after next() rejects, and assert that the final provider failure contains rootRefusal.

Keep the default spawn path. A custom spawn override bypasses the stderr capture and stderrClosed wait that this regression must cover.

9Extend the “reports the CLI's root refusal when it exits before answering” test
10to cover SDK failure before stderr delivery: have the child wait for stdin
11before writing rootRefusal, then release stdin after ClaudeSdk.query rejects and
12assert the final provider failure contains rootRefusal. Use realQueryRunner’s
13default options.spawnClaudeCodeProcess path so the test exercises stderr capture
14and closure handling.
