fix(server): queue explicit steer between provider turns#23559
Problem
An explicit Steer sent while a run is starting or between provider turns fails with “No running provider turn found” and leaves the follow-up undelivered. Reproduced on current main by starting a real Claude run and immediately dispatching an explicit Steer through the authenticated web connection.
Change
Apply the existing provider-turn gap check before resolving explicit Steer. Follow-ups in those gaps use the durable queue; steering a running provider turn and explicit Restart retain their existing behavior.
Scope and approval
Fixes #23271 GitCafe: https://git.cafe/pingdotgg/t3code/issues/23271 Triaged bug via the automated issue flow. The change is limited to server delivery policy and its existing tests.
Verification
- Red: six policy cases failed on main because explicit Steer returned
steer_activerather thanqueue_after_active. - Green:
vp test run apps/server/src/orchestration-v2/CommandPolicy.test.ts apps/server/src/orchestration-v2/runtimeLayer.test.ts apps/server/src/orchestration-v2/SteeringCompletion.integration.test.ts— 106 tests passed across three files on the final head. - Scoped lint and server typechecking passed. Two independent reviews returned
MERGE. - Runtime: Linux web dev server, Chrome headless, Claude Code 2.1.303 /
claude-opus-5-5. Before, explicit Steer was rejected; after, both requests succeeded, the follow-up queued, and Claude consumed it and returned the expected marker. Both runs completed.

Before: 8d3b061121; after: 2d0622b17b. Server-only change; no render paths changed and the capture matrix is empty. Tests cover preparing/starting, absent/pending/completed provider turns, and a running turn from a previous attempt. Controls preserve automatic delivery, Restart, and steering a matching running turn.
The reporter’s native Windows surface was not run; the reproduced failure and fix are in shared server policy. Runtime provider verification covers Claude; other adapters are covered by the shared policy tests and review.

Models: Claude Opus 5.5 and GPT-6.1-Sol. Harnesses: Claude Code and Codex through T3 Code.
Proof audit
Independent proof audit: SUFFICIENT (fixture matrix · audit pass 2). Runtime transcripts cover explicit Steer in the early gap (rows 1–3), no active run (row 12), and the new regression control: explicit Steer into a running provider turn still steers the live run on both builds (row 7: before · after). Driver: apps/server/scripts/verify-steer-running-turn.ts (c990ba1b46).
Verification log
- Audit pass 1 (claude-opus-5-5): SUFFICIENT; row 7, explicit Steer while the active attempt's provider turn is running, flagged as the uncovered runtime control.
- New captures (real server, isolated home, real Claude turns on claude-sonnet-5-5, no tools), same driver for before 8d3b061121 and after 2d0622b17b:
- Row 7: the provider turn was
runningat dispatch. Steer was accepted and delivered into run 1 (inputIntent: steer), with one run in total andROW7_STEEREDreturned. Identical on both builds. - Row 9: explicit Restart during a running turn was rejected on both builds with "Claude cannot redirect an active run". That is the Claude capability check, which this PR does not change (before · after).
- Row 7: the provider turn was
- Audit pass 2 (claude-opus-5-5): SUFFICIENT. All SHAs match, and c990ba1b46 changes only the driver script. The auditor rebuilt row 7 from the committed script and got a matching result (rebuild).
- Unit-test-only rows: 4, 6, and 8 (a pending provider turn or a previous attempt's running turn needs synthetic projection state or narrow races), 5 (completed turn while the run is still active; the timing window is tight), and 10–11 (auto-delivery controls, path unchanged). Rows 1–3 share one transcript that does not record which gap state the run was in; each row has its own unit test.
- Unverified: none. Not covered: a unit test for Restart during a running turn (this gap predates the PR), and providers other than Claude at runtime.
- Model/harness: Claude Opus 5.5 (lead, auditors, capturer) through Claude Code in T3 Code; captures ran Claude Sonnet 5.5 as the provider.

Verdict: Needs Human Approval
Macroscope's review found this PR requires human approval — The server policy change is a focused bug fix, but the PR adds a live verifier with file-level static-analysis suppressions and unresolved paths that can report success without proving delivery or leave a detached server running. These review-sensitive issues require human assessment before approval.

📝 Walkthrough
Explicit steering now queues after an active run when the run is preparing or starting, or when its active attempt has no running provider turn. Restart delivery remains unchanged. Tests cover these routing decisions. A host-side verifier exercises steer and restart delivery and records the resulting run projection.

Make delivery part of the verifier’s success condition.
The script documents that Steer or Restart must reach the active run. The current completion condition only checks that a dispatch was attempted and that all runs became terminal. A rejected dispatch is converted to { ok: false, error }, so it can still produce status: "finished" and exit zero. The final code also records the run association without validating it.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win
Wrap the transcript write in try/finally so stopServer() always runs.
The finally block runs git(...), uname, and writeFileSync before it calls stopServer(). Each of these calls can throw:
git rev-parsethrows when--server-rootis not a git checkout.execFileSync("uname")throws whenunameis missing.writeFileSyncthrows when the--outdirectory does not exist.
When one of them throws, stopServer() never runs. The server was started with detached: true and keeps running. It holds the port and the isolated home directory, so the next run can fail on the port or the home directory. The new exception also replaces the original failure, and no transcript is written.
Run stopServer() in a nested finally. Also make the environment probes non-fatal, so a failed probe does not discard the result.
