Problem

Settling a completed Codex parent unsubscribed an independently attached delegated child. Sending that child another message finished normally in Codex, but T3 never displayed the reply or completed its run.

Change

Check each projected native thread under its owning app thread’s attachment lock before unloading it. Attached children retain their subscriptions and loaded state; unattached children still unload. Regression tests cover both cases.

Scope and approval

Fixes #20348 GitCafe: https://git.cafe/pingdotgg/t3code/issues/20348 Triaged bug via the automated issue flow. Reproduces the failure reported and diagnosed by @jfet97 in #20348; related contributor fix #20361 remains separate.

Verification

  • Red: the attached-child regression fails without the fix: expected no native unloads, received native-child-thread.
  • Green: all 43 ProviderSessionManager tests pass, including unattached-child cleanup and unload/reattach serialization. Repeated after merging main.
  • Targeted lint and formatting pass; server typecheck passes.
  • Independent GPT-6.1-Sol and Claude Opus 5.5 reviews returned MERGE on the final head.
  • Real web dev app on Linux x86_64, Chromium, 1280×800, same configured Codex GPT-6.1-Sol instance: child READY, parent PARENT_DONE, settle parent, open that same child, send Reply exactly PONG. Before: native completion with T3 stuck running. After: PONG appears and T3 persists the child run as completed. No server restart between settlement and follow-up.

Evidence matrix: empty; server-only subscription lifecycle change with no changed render paths. Proof is the regression test and real runtime transcripts. Web exercises the shared server path used by desktop, mobile, local, remote/relay and tunnel connections. OpenCode shares the manager branch; provider-specific OpenCode runtime was not exercised.

The manager source on the before build is byte-identical to the PR merge-base 96bae5465b.

Models: GPT-6.1-Sol and Claude Opus 5.5. Harnesses: Codex and Claude Code via T3 Code.

Takeover

Credit: @juliusmarminge’s implementation and existing regression/runtime proof are preserved. No code changes; evidence and verification added below. Existing sufficient captures were retained.

Evidence

The earlier empty-matrix statement is superseded by this explicit server fixture matrix. No render paths changed; runtime transcripts apply, with no screenshot/theme/viewport cross-product. Final head: dca0af89a8; base: 96bae5465b.

Delivery index · Full matrix · Runtime transcripts · Reproduction scripts · Evidence archive.

Environment: Linux x86_64, real application server and Codex. U/C1/C2 are explicitly instrumented service-harness runtime captures; P observes modules without changing source. No visible changes introduced by this evidence-only pass.

Verification log
  • Independent pass 1 matrix/audit identified U/C1/C2/P gaps; existing A was accepted and retained.
  • Independent pass 2 audit: SUFFICIENT. Verified event ordering, supplied verifier, 971 module hashes and transformed manager digest. No fresh real-provider replay in pass 2; it independently inspected delivered artifacts.
  • Focused manager suite rerun on exact final head: 43/43 pass, one existing test file; no tests changed.
  • Cafe reports 34 successful, 0 pending, 0 failing checks on final head. Conflict readiness remains pending against a stale base snapshot; no merge-readiness claim.
  • Limits: historical A process provenance cannot be reconstructed; separate exact-head P supplements it. C2 proves serialization during the enclosing unload effect after native acknowledgment, not a still-pending native wire response or naturally occurring contention.
  • Unverified runtimes: OpenCode, desktop/mobile clients, relay and tunnel. These add no changed server branch or render path; no runtime capture claimed for them.
  • Evidence-only scope: no code changes, commits, branch synchronization, push, claim comment or merge. Reproduction scripts are public scratch artifacts, not committed fixtures.
  • Model/harness: GPT-6.1-Sol through Codex in T3 Code; uploads via ghdrop. Original GPT-6.1-Sol/Claude Opus reviews above preserved.

Approvability

Verdict: Approved at d111609

Macroscope's review found this PR approvable — Detach cleanup now respects each delegated child's attachment state, allowing attached children to keep receiving replies after their parent settles. Existing runtime checks, unload timeouts, and cleanup scheduling remain in place.

Walkthrough

When detaching a multi-thread session, the manager checks each native provider thread’s owner under that owner’s lock before unloading. Tests cover a resumed child that stays loaded and an unattached child that is unloaded.

Detach locks each native provider thread’s owner, falling back to the detaching thread when no owner ID is set. It skips unloading if the runtime changed or the owner is attached. Tests cover resumed and unresumed child threads.

The new test does not confirm that an attached child’s next reply arrives after its parent detaches. The timeout concern also remains conditional. These gaps warrant owner awareness or targeted verification before merging.

Full details: Description check

Explanation

The description includes all required sections: Problem, Change, Scope and approval, and Verification. It explains the bug, implementation, linked issue, test results, runtime evidence, and verification limits.

Full details: Linked Issues check

Explanation

The pull request meets the coding requirements in #20348. ProviderSessionManager.ts now locks each projected provider thread by its owning appThreadId before unload. The code skips unload when the owner remains attached and unloads unattached native threads. The existing runtime check and loaded-state handling remain inside the lock. ProviderSessionManager.test.ts adds coverage for attached-child retention and unattached-child cleanup.

Full details: Out of Scope Changes check

Explanation

The changes are limited to ProviderSessionManager.ts and its regression tests. The lock change and tests directly support #20348. No unrelated product behavior, render path, provider runner, or child creation logic is included.

Assert the child’s post-detach reply.

The fixture’s resumeThread only increments resumeCount and returns the thread. The test never emits or reads a provider event. A broken child subscription could therefore pass the unload, cache, and liveness assertions. Extend the fixture to emit a child reply after the post-detach turn, then assert that exact reply.

Verify native ordering after an unsubscribe timeout.

CodexAppServerProtocol.request queues thread/unsubscribe before it waits for the response. A timeout removes only the pending waiter, not the queued message. The owner lock can therefore release while the native unsubscribe is still in flight, allowing a concurrent resumeThread to proceed. If Codex completes the unsubscribe after the resume, the manager may record the child as loaded while it no longer receives replies. Keep the state unresolved until native completion or reconciliation.
