Problem

Each Stop while a Codex approval is pending leaves one request handler waiting forever. The Codex transport allows 32 active handlers per app-server, and all Codex threads on a provider instance share one app-server. After 32 such Stops, every later approval, question or elicitation from any thread on that instance is refused with -32001 "Too many Codex requests are already active." The user sees no approval card. Codex records the command as failed and the agent carries on as if it was denied.

The handler waits on a decision that only a user answer resolves. After Stop the request is cancelled and can't be answered, so the answer never comes. Codex itself settles the request when the turn ends and sends serverRequest/resolved {threadId, requestId}, but the transport ignored that notification.

On main, sending another message in the same thread declines the stale card and frees the slot. Stops spread across threads leak, which is what happens when a parent cancels delegated Codex tasks (#21935). Once #22023 withdraws stale cards on Stop, the same-thread path would leak too.

Change

packages/effect-codex-app-server/src/protocol.ts:

 • Request handler fibers now run in a FiberMap keyed by request id instead of a bare forkIn. The map lives in the same requestHandlerScope, so protocol termination still interrupts every handler. Finished handlers leave the map on their own.
 • On serverRequest/resolved, the matching handler is interrupted. No reply is written, because Codex already settled the request. The existing ensuring returns the slot, and the adapter's own ensuring drops its pending entry.

The notification is still forwarded to onNotification as before. Codex also sends it after a normal answer, by which point the handler has finished, so that case is a no-op. This covers every server request kind and every way Codex ends one, not only Stop.

One side effect: if a stale card is still on screen after Codex settled its request, answering it now fails with "No pending Codex runtime request". Before, the reply went to Codex, which ignored it.

Scope and approval

Fixes the adapter half of #21935, which @juliusmarminge accepted as a bug. #22023 by @fixfon fixes the projection half and lists this leak under Risks ("Codex's approval handlers wait without an abort"). That PR touches different files.

Verification

New test in protocol.test.ts: 32 approval requests whose handlers never finish, then a serverRequest/resolved for each, then request 33. It expects request 33 to be handled. It fails on main with the -32001 refusal and passes here, with the other 15 protocol tests.

The new protocol test fails on main with the -32001 refusal and passes on this branch

Real app: production builds of main and this branch, fresh state, real Codex CLI 0.163.0 pointed at a local Responses API stub, so no account was used. In 33 new Supervised threads on one Codex instance, each asks to run touch approved.txt. In threads 1 to 32 the approval card shows, and Stop is pressed without answering.

Main, thread 33: no approval card. The turn ends by itself with "Done."

Main: thread 33 shows no approval card and the turn ends with Done.

This branch, thread 33: the approval card shows.

This branch: thread 33 shows the command approval card

Pressing Approve runs the command, and the file is created.

This branch: after Approve, thread 33 finishes with approved.txt changed

Codex app-server traffic for the same runs: main answers request 32 with -32001, and this branch forwards the user's accept.

App-server traffic: main refuses request 32, this branch sends accept

A direct probe of codex app-server 0.163.0 (three turns, each interrupted with an approval pending) received serverRequest/resolved for each request.

Also run on this branch, in packages/effect-codex-app-server: protocol.test.ts and client.test.ts (18 passed), tsc-rs --noEmit, and vp lint and vp fmt --check on both changed files. All passed.

Not checked

 • Codex versions older than 0.163.0. The notification is in the generated protocol schema this package already uses.
 • A real model account. The stub only returns the tool call that triggers the approval.
 • Other request kinds (file change, permissions, MCP elicitation, user input) in the real app. They share the same transport path, which the protocol test covers.

Prior PRs

 • #22023 by @fixfon (open) withdraws a stopped turn's pending approvals in the projection and named this leak. Nothing reused; the two fixes are independent.
 • #22982 by @juliusmarminge (open) cancels approval-blocked tools when a run ends. It says the #21935 Stop path is not fixed there, and it touches different files.
 • #18764 by @SunkenInTime and #12777 by @VendettaGhub (both open) change protocol.ts termination handling in other hunks.
 • Issue #21935 reported by @eowca.

 • fix(codex): release an approval handler when Codex resolves its request47d82f5
Synced from GitHub

Approvability

Verdict: Approved at 47d82f5

Macroscope's review found this PR approvable — This focused Codex protocol fix releases handler fibers when Codex has already settled a request, preventing stale approvals from exhausting the 32-handler limit. It includes a regression test for the exhausted-handler scenario and leaves normal completion and termination behavior scoped as before.

Walkthrough

The protocol tracks incoming request-handler fibers in a FiberMap. A valid serverRequest/resolved notification removes the handler with the matching request ID. Tests cover handler release and distinguish numeric IDs from string IDs.

Changes: Request Handler Cleanup

Layer / File(s)Summary
Track and release request handler fibers
packages/effect-codex-app-server/src/protocol.ts, packages/effect-codex-app-server/src/protocol.test.ts
The protocol runs request handlers through a FiberMap and removes a matching handler when it receives a valid serverRequest/resolved notification. Tests cover resolving 32 pending requests and confirm that resolving string ID "1" does not interrupt the handler with numeric ID 1.

399
400## What version of Effect is running?
401
402`effect@4.0.0-rc.117` (the latest published v4 RC when checked). Also reproduced on `4.0.0-rc.112`.
403
404Runtimes tested on rc.117: Bun 1.2.21 and Node.js v24.7.0, macOS arm64.
405
406## What steps can reproduce the bug?
407
408In a new directory, install the exact version:
409
410 npm init -y
411 npm install --save-exact effect@4.0.0-rc.117
412
413Save the following as `repro.ts` and run `bun repro.ts` (or `node repro.ts` with Node.js v24.7.0):
414
415 import { Effect, FiberMap } from "effect"
416
417 const program = Effect.gen(function* () {
418 const map = yield* FiberMap.make<string>()
419 const log: Array<string> = []
420
421 yield* FiberMap.run(map, "a", Effect.sync(() => log.push("a started")), { startImmediately: false })
422 log.push("run(a) returned")
423
424 yield* FiberMap.run(map, "b", Effect.sync(() => log.push("b started")), { startImmediately: true })
425 log.push("run(b) returned")
426
427 // Control: forkChild honors startImmediately: false.
428 yield* Effect.forkChild(Effect.sync(() => log.push("child started")), { startImmediately: false })
429 log.push("forkChild returned")
430
431 yield* Effect.yieldNow
432 return log
433 })
434
435 Effect.runPromise(Effect.scoped(program)).then((log) => console.log(log.join("\n")))
436
437## What is the expected behavior?
438
439If `run` accepts `startImmediately: false`, it should defer starting the new fiber: `run(a) returned` should appear before `a started`, as with `Effect.forkChild`.
440
441Alternatively, if synchronous startup is intentional, remove the unsupported option from the public `run` types. The choice of API behavior is up to the maintainers.
442
443## What do you see instead?
444
445Both runtimes print:
446
447 `a started
448 run(a) returned
449 b started
450 run(b) returned
451 forkChild returned
452 child started
453 `
454
455The option does not change startup behavior. Whenever `run` launches a new fiber, it starts it synchronously, whether `startImmediately` is false, true, or omitted.
456
457## Additional information
458
459The same behavior was verified for `FiberSet.run` and `FiberHandle.run`, with both curried and uncurried calls. A separate matrix checked all four APIs (`FiberMap.run`, `FiberSet.run`, `FiberHandle.run`, and the `Effect.forkChild` control), both call forms, and false/true/omitted options: 24 cases per runtime.
460
461In the published rc.117 source, all three public APIs accept `startImmediately`, but their `runImpl` options types omit it. Each implementation calls `Effect.runForkWith(parent.context)(effect)` without forwarding startup options (omitting the source's context cast here for brevity).
462
463Locations relative to `packages/effect/src/` in rc.117:
464
465File | Public option declarations | runImpl starts | runForkWith call
466--- | --- | --- | ---
467FiberMap.ts | 1317, 1362 | 1375 | 1390

Preserve the request ID type in FiberMap keys.

JsonRpcId accepts both numbers and strings, and isIncomingRequest admits both types. Active requests with IDs 1 and "1" can therefore reach this map. String(request.id) maps both IDs to "1". FiberMap.run interrupts the existing handler, whose interruption does not enter either response branch. The first request may receive no response. The serverRequest/resolved lookup has the same collision.

7Update the request-handler key used by FiberMap.run to distinguish numeric IDs

Verified in a real dev environment

verified as-is at 82f1d04fa3fbcff0f3190bc89f7120e7b7fcadf4 on this Linux Boat VM. compared with github/main at 6575ce32508bd7908444241f3719aa33aaf77f90. installed both with vp i and ran one isolated dev server at a time. drove the actual web app using repo Playwright and /usr/bin/google-chrome, with real Codex CLI 0.164.0, model gpt-6.1-sol, through the local proxy. no model stub, replay provider, fixtures or database edits.

added a project for a real git repository through the app. in 32 separate supervised threads, sent the same prompt to run touch approved.txt, waited for the approval card, then clicked Stop without answering. provider logs confirmed serverRequest/resolved after every stop. main needed thread reloads to display cards.

main request 33 received -32001, showed no approval card, and did not create the file. on the exact pr head, request 33 displayed the card; clicking Approve sent accept, created approved.txt, and finished with the successful command and changed file visible.

neighboring checks also passed: declining request 34 sent decline and left declined.txt absent; approving request 35 sent accept, created approved-again.txt, and completed. the same shared codex app-server stayed running throughout each 33-thread run.

git.cafe renders the external evidence embeds as links. the uploaded originals were inspected directly, and the mp4 played through to completion with seeking/resuming checked in Chrome. inline playback inside the pr comment was not available.

native iOS/Android and Electron shell behavior, macOS/Windows, remote relay/tunnel connections, older Codex versions, and other server-request kinds such as file-change approval or elicitation were not exercised on this Linux web-client run. numeric/string request-id isolation and termination behavior were covered by the existing focused protocol tests, not separate real-provider scenarios.

stale approval-card withdrawal is outside this pr; stopped cards can remain visible. this run verified transport handler release.
