fix(server): ACP v2 idle errors fail the turn#23217
Problem
ACP v2 agents can accept a prompt and then report an idle error, but T3 silently records the turn as completed. Reproduced with real acplause v0.202610.0 and a deliberately invalid API key in an isolated Linux web dev environment, both in a fresh thread and after a provider handoff.
Change
Preserve the optional structured idle error in the prompt response. Finalize generic ACP idle errors as failed through the existing provider failure mapping, with a fallback message when the agent omits details. Reuse that mapping for the existing agent-work completion path.
Scope and approval
Fixes #21219 GitCafe: https://git.cafe/pingdotgg/t3code/issues/21219 Triaged bug via the automated issue flow: #21219. This is protocol error propagation; it changes no authentication, credentials, permissions, approvals, modes, sandboxing, or MCP capabilities.
Verification
Final-head CI: 30/30 checks successful, none pending or failing. CodeRabbit reports no actionable findings. Macroscope retains a human-approval note about failed-turn tool cleanup; two independent reviews confirmed that the existing integrated fixtures require running commands to remain stoppable.
Current head: f9c17f6f97, with a tree identical to the original verified revision below. The focused registry and replay-fixture run passes 31/31. Scoped server shard 7/8 passes 470/472; the two remaining local repository-identity failures are caused by this machine's global Git URL rewrite, and that file passes 11/11 with GIT_CONFIG_GLOBAL=/dev/null. No cleanup policy for ongoing tools or commands changes in this PR.
Verified equivalent tree: 9054687df3, including a clean merge of main ed9b7ca9c5. Post-merge focused rerun: 101/101 passed (output).
- Red on main: both regression cases failed with
expected completed to equal failed; red transcript. - Green: 101 distinct focused tests pass: registry 16, ACP adapter 59, ACP client 26. Registry output, adapter controls, client and registry output.
- Both failure cases preserve their failed terminal through completion of a subsequent successful prompt. Existing success, cancellation, interruption, and Claude following-work controls pass.
- Scoped
effect-acpandprovider-acptypechecks pass. Changed files formatted and linted; one existing untouchedno-inline-schema-compilewarning. Server-wide typecheck was stopped after hanging; no repo-wide checks run. - Fresh round-2 GPT-6.1-Sol and Claude Opus 5.5 reviews: MERGE; no sensitive surface changes. Proof audit pass 2: SUFFICIENT.
Final proof audit: SUFFICIENT. The final tree equals the original verified tree; the temporary tool-cleanup change has been reverted.
Evidence: server-only protocol change, visual matrix empty. No changed render paths require captures.

Runtime environment: Linux x86_64, isolated web dev app and server, real acplause v0.202610.0 with a deliberately invalid nonsecret key. Success and cancellation controls use deterministic fixtures. Desktop and native mobile were not run; the change is in shared server behavior.

Approvability
Verdict: Needs Human Approval
Macroscope's review found this PR requires human approval β This is a focused ACP server bug fix that propagates idle errors and marks affected turns as failed, with targeted regression coverage. The failed settlement path does not currently clean up open run-owned tools, leaving a material lifecycle/integration risk that requires review.

The ACP client now includes prompt errors in prompt responses. The provider adapter maps those errors to provider failures and finalizes affected turns as failed. Tests cover error payloads, a subsequent prompt, and failed-turn terminalization.

- fix(server): a failed ACP turn no longer leaves its foreground tools running3ae498c
Fixed the foreground-tool finding in 3ae498cbdd: failed ACP turns now close eligible unfinished foreground tools as failed, using the existing filter that preserves background tasks and subagents. The new replay regression failed with expected running to equal failed before the fix. All 92 focused adapter tests pass; the primary registry rerun passes 17/17.
The error.data finding needs no change. The extracted agent-work-state path already copied this field, matching AcpRequestError.fromProtocolError. Both registry and Grok failure mappers produce bounded makeProviderFailure fields; neither publishes arbitrary error data. Two fresh independent reviews verified this at the current head and returned MERGE.
Final proof audit: SUFFICIENT. The PR remains unmerged.
The human-approval concern refers to the already-reviewed error.data finding. The protocol object stays local: registry mapping and Grok mapping both return bounded failure fields, without arbitrary data. This also matches the pre-existing AcpRequestError.fromProtocolError behavior. Two fresh independent reviews confirmed that data flow at this head; no additional data-flow change was made.
Thread transfer impact is within every enforced ceiling. The separate server shard 7 failure is under investigation.

The omitted-stop-reason suggestion is outside this fix's scope. The decoder preserves optional error details without assigning them completion semantics. The pinned ACP contract says omitted/null stopReason means no reason was reported. This fix handles the explicit stopReason: "error" case requested by #21219, with or without error details. Inferring a reason from the payload would change the existing client default. A fresh reviewer checked the contract and found no supported error-only idle failure signal, so no further change was made.

- revert(server): keep commands from a failed ACP turn stoppablef9c17f6
Reverted the broad failed-turn tool cleanup in f9c17f6f97. Existing integrated replay fixtures require a generic running command to remain stoppable after a rejected prompt, including after releasing the thread. Blanket foreground cleanup removed that command and made the subsequent Stop fail with Run is not interruptible. The final tree now exactly matches the original verified idle-error fix.
Both affected replay fixtures pass again; registry plus fixture tests pass 31/31. Server shard 7/8 passes 470/472, with only two local Git URL-rewrite failures; their file passes 11/11 with global Git config disabled. Both fresh round-2 reviews returned MERGE and the updated proof audit is SUFFICIENT.
The tool-cleanup finding needs no further change within this issue: ongoing commands cannot be assumed finished when their turn fails. The original idle-error propagation fix is preserved. The earlier cleanup-specific proof is superseded by the updated audit.
Closing: main already fixes this via https://github.com/pingdotgg/t3code/pull/24615.
