Problem

A new run can stay starting after all five start attempts fail to read its thread's recovery projection. On Linux at a12d6ed942, a normal Codex turn completed; injecting a malformed prior-run command_execution item when the next message committed left that follow-up starting with a failed outbox effect and no error item.

Change

Handle a typed recovery-projection read failure through the existing start-failure settlement path on the final attempt. Earlier attempts still retry. The existing current-run guard writes the failed run, attempt, root node, and history-read error together before any provider execution starts.

Scope and approval

Fixes #16441 GitCafe: https://git.cafe/pingdotgg/t3code/issues/16441 Triaged bug via the automated issue flow. This focused server fix reuses existing failure behavior; it changes no contracts, clients, provider adapters, or access controls. See the reproduced report on #16441.

Verification

  • Focused command, from apps/server: ../../node_modules/.bin/vp test run src/orchestration-v2/ProviderTurnStartService.test.ts.
  • Red regression output: actual rerun at 61081da02d, one regression failed and 22 tests passed.
  • Green output: all 24 tests passed at 016889ea43, including final settlement and retry preservation.
  • Final integration at aef3779e8f after merging main ed9b7ca9c5: all 24 focused tests passed; server vp run typecheck passed; targeted lint and formatting passed (three pre-existing unused-variable warnings). The diff remains the same two reviewed server files.
  • Final proof audit: SUFFICIENT; independently verified durable settlement events 58–61 against their projections and the unchanged malformed row. The evidence script now reads the active event table.
  • Two isolated reviewers (GPT-6.1-Sol and Claude Opus 5.5) returned MERGE with no confirmed findings.

Evidence: Linux x86_64, Node 24.21.0, real web dev server on port 8492 with isolated worktree state. The browser submitted both follow-ups. This is a server-only change; the proof matrix has zero visual rows.

BeforeAfter
Runtime transcript, a12d6ed942: run starting, outbox failed after five attempts, no error item.Runtime transcript, 016889ea43: run, attempt, and root node failed with null start times; “Thread history failed to load” persisted; outbox succeeded on attempt five; no provider turn or native execution item.

The malformed prior-run row remained unchanged across every column. Read-only verification script and isolated fault fixture are available to repeat the check. No other visible change is claimed. The malformed history still prevents the browser's bounded-history read; this fix does not repair corrupt stored data. Desktop and native mobile were not run; the changed path is shared server logic.

Models and harnesses: Claude Opus 5.5 via Claude Code for tests and implementation; GPT-6.1-Sol via Codex for triage and proof; both via T3 Code delegation for review.

Approvability

Verdict: Needs Human Approval

Macroscope's review found this PR requires human approval — History-read failures now settle non-retrying runs as failed rather than leaving startup unfinished. An unresolved privacy concern remains: the persisted failure message can include underlying error text containing stored command data.

Walkthrough

The provider turn start flow handles runtime-recovery projection read failures based on retry state. It returns retryable failures to the worker. When no retry is planned, it settles the run as failed with a thread-history error.

Changes: Runtime recovery read failure handling

Layer / File(s)Summary
Handle and test recovery projection read failures
apps/server/src/orchestration-v2/ProviderTurnStartService.ts, apps/server/src/orchestration-v2/ProviderTurnStartService.test.ts
The service propagates projection read failures when willRetry is true. Otherwise, it settles the run as failed with the title “Thread history failed to load.” Tests cover both outcomes and configure the local-command harness to simulate projection read failures.

Full details: Linked Issues check

Explanation

Issue #16441 requires final-attempt settlement when getRuntimeRecoveryProjection fails, while earlier attempts must remain retryable. ProviderTurnStartService.ts now captures the typed read failure, rethrows it when willRetry is true, and otherwise calls settleStartFailure with signal runtime-recovery-read-failure and title Thread history failed to load. The focused tests verify failed run, attempt, root node, error item, no provider session, and retry preservation.

Full details: Out of Scope Changes check

Explanation

The diff is limited to the recovery-read handling and its focused test harness and tests in ProviderTurnStartService.ts and ProviderTurnStartService.test.ts. The test changes directly verify issue #16441 behavior. No unrelated contract, client, provider, or access-control changes appear in the reviewed diff.

Checked the stored-command-data claim against the production read path at aef3779e8f. decodeRows wraps turn-item decode errors in ProjectionStoreReadError, and getRuntimeRecoveryProjection wraps that error again. The immediate cause consumed by settleStartFailure therefore has the fixed projection-read message, not the malformed command payload. The raw message in the unit test comes from its synthetic Error cause. Single-wrapped thread-schema and SQL failures carry schema expectation/path or SQLite diagnostic text; no stored command payload was found there. A fixed-message override would be additional hardening rather than a fix for the reported exposure. Leaving the existing start-failure message behavior unchanged and resolving this finding.
