Problem

A delegated task cancelled while waiting for command approval can finish as interrupted while its approval remains pending. This occurs when the provider's terminal event commits before interruption cleanup, leaving the old approval actionable and the thread needing attention.

Change

Withdraw the stopped turn's pending native approvals alongside questions, marking approvals cancelled and non-resumable and cancelling their associated nodes and timeline items. Preserve requests belonging to other turns and approvals on completed or failed root turns, whose background subagents may still need them. Recheck pending state inside the write transaction so cleanup cannot overwrite a concurrent answer.

Scope and approval

Fixes #21935 GitCafe: https://git.cafe/pingdotgg/t3code/issues/21935 Triaged bug via the automated issue flow. Existing contributor implementation and author credit retained. Approval lifecycle changes require Julius's review. The PR has bugfix; it has not been merged or labeled land.

Verification

  • Main snapshot 3b95e7831b: minimal ingestor regression test fails in both interrupted terminal paths with expected 'pending' to equal 'cancelled'; completed-turn control passes.
  • Candidate b3dbe20998, after merging that main snapshot: ingestor, SQL/in-memory projection store, and turn-control suites pass, 63/63 tests. Includes concurrent-answer protection.
  • Targeted lint on the six changed files passes. vp run typecheck in apps/server exits 0 with no TypeScript errors. git diff --check passes.
  • Historical real Codex cancellation reproduction demonstrates the terminal-event ordering behind the bug.
  • Seven preserved local provider attempts cleared approvals normally; they do not reproduce the race. A new real-app attempt on the main snapshot was blocked by VM event-loop stalls, disconnects, and EnvironmentNotRegisteredError after pairing.

Sanitized red/green server evidence.

Evidence: server tests prove the state transition; fresh before/after interaction recordings are missing. Proof audit is incomplete. Pending approval → parent cancellation → cleared child/parent UI, live child approval after normal root completion, and reconnect after cancellation remain unverified in a real client. Web, desktop/Electron, mobile, OpenCode v2 and Muse runtime behavior remain unverified. No UI capture or complete end-to-end verification is claimed.

Original implementation: Claude Sonnet 5.5 via T3 Code. Integration and verification: GPT-6.1-Sol and Claude Opus 5.5 through Codex and Claude harnesses in T3 Code.

The projection store now retrieves pending native requests with optional approval inclusion. Provider-turn cleanup dismisses approvals for interrupted or cancelled turns. Cancelled non-user requests receive non-resumable response capabilities.

Possibly related issues

  • pingdotgg/t3code#22405 — The issue concerns stopped-turn cleanup for native user-input requests, which shares the cleanup path extended here to approvals.

Suggested reviewers: t3dotgg, juliusmarminge

Merge Risk: 🟡 Moderate · up to b3dbe

A stopped turn can regain a resolved approval after its approval was cancelled. Validate the approval within the response write before merging.

Security architecture risk: 🔵 Low · up to 90f78

Cleanup remains restricted to the stopped turn and does not add approval authority. However, cancellation can race an approval response and leave the recorded outcome inconsistent with the accepted decision. The identified impact is bounded to the affected requests; unauthorized execution has not been established.

Retained concerns

  • Medium · reliability · inferred: Stopped-turn cleanup adds approval cancellation without extending the existing write-time stale-state guard. If an approval response commits after cleanup reads pending state but before cleanup writes, cancellation replaces the resolved decision and cancels its node and item; the queued response may then fail its execution-time check. The reverse interleaving can replace cancellation with a stale resolved payload retaining live capability. This makes the persisted approval outcome depend on scheduling rather than one authoritative terminal transition. Earlier decision events remain in history, and unauthorized provider execution is not established.
Security review details

Security Blast Radius

  • observed — A cleanup invocation selects pending requests from one thread and one provider turn. Other-turn and null-turn session requests are excluded, and message-mode user inputs remain outside native cleanup. Approval inclusion broadens eligible request kinds, not thread or session authority.

Security Findings and Attack Paths

  • inferred — The supported adverse path requires an approval response and stopped-turn cleanup to overlap for the same request. It can corrupt the current decision projection or disrupt delivery of an accepted response. It does not establish cross-thread access, privilege escalation, or execution without a user decision; earlier decision events are not deleted.

Trust Boundaries and Controls

  • observed — Response dispatch rejects requests already observed as non-pending or not_resumable. Before invoking the provider callback, execution independently requires a resolved request, a live capability naming the expected provider session, and an active session. These existing checks limit stale-response exposure but do not provide a write-time compare-and-set for the approval transition.

Hardening Proposals

  • proposed — Make approval resolution and stopped-turn cancellation contend on one authoritative transition check. Preserve request, node, and turn-item consistency when either transition loses, and validate both commit orderings using the existing gated-write concurrency pattern.

Explanation

Issue #21935 requires unresolved approvals to be withdrawn after an interrupted or cancelled run and excluded from pending state. ProviderEventIngestor now includes approvals for interrupted and cancelled turns. Withdrawn approvals receive cancelled status and a not_resumable capability. The projection-store tests cover turn scoping, approval inclusion, and pending-request selection. The ingestor tests cover terminal cleanup and concurrent answers. Completed turns remain excluded, which preserves approvals for running descendants.

Full details: Out of Scope Changes check

Explanation

The changed production files support issue #21935. The projection-store API and implementations select pending approvals. ProviderEventIngestor applies approval withdrawal to stopped turns. EventSink prevents stale cancellation from overwriting a concurrent approval answer. The related tests and mock update support these changes. No unrelated change is shown in the supplied scope summary.

Full details: Description check

Explanation

The description includes all required sections. It clearly explains the problem, the implementation, issue and approval scope, focused verification results, limitations, and tooling used. The stated lack of live UI and provider verification is explicit and appropriate for this server-side change.

Written by Claude Opus 5.5 on behalf of @ScottN-PV.

Real-run data for "What I couldn't prove", in case it helps. We did not run this patch.

On main b34501a, a Codex parent used delegate_task to start an approval-required child, which blocked on a shell-command approval. The parent's task_cancel returned cancel_requested, then interrupted. In both of two runs, the approval went from pending to cancelled/not_resumable with the reason "The run was interrupted." That reason comes from the request loop in Orchestrator.settleInterruptedRun (#15442). The reporter's v0.0.46-nightly.20261008.2819 already contains that loop, so it was present when their Codex approval stayed pending.

Not covered: Claude tasks, native subagents, Stop from the UI, timing variations.

Synced from GitHub

Thanks @ScottN-PV, this is useful. I agree: for a Codex parent's task_cancel, settleInterruptedRun already withdraws the approval, so that isn't the gap.

This PR covers a different path. settleInterruptedRun returns early unless the run and its attempt are still running. When a turn ends as interrupted or cancelled through ProviderEventIngestor, main only withdraws user-input requests there, so approvals stay pending. The regression test shows this on main.

I haven't confirmed in code which real scenario reaches that path, so I'm not claiming it explains the reporter's case.

Synced from GitHub

Live data for the "What I couldn't prove" section. On main (7208335), a Codex task_cancel left the child's command approval pending.

Setup: a Supervised Codex parent used delegate_task to start a child, the child blocked on touch approved.txt, and the parent called task_cancel. The child run went to interrupted. The approval stayed pending, with a live Approve/Decline card, until I stopped the server.

Doesn't settleInterruptedRun already handle this, as @ScottN-PV saw? Sometimes. It only runs while the run is still running, and in my run Codex's turn/completed got there first. The provider log shows turn/interrupt, then turn/completed (interrupted), then serverRequest/resolved for the approval, within 19 ms.

A second run backs this up. With a turn-end withdrawal in the ingestor (close to this PR), the same cancel left the approval cancelled with its capability still live. That means the turn-end path withdrew it, not settleInterruptedRun, which writes not_resumable. In other words, a real cancel does reach the path this PR fixes.

Two smaller things from testing:

  • Leaving completed turns out is right. I first tried withdrawing approvals on every turn end. A Claude background subagent asked for approval while the main turn ran, and when the main turn completed, its still-live callback was cancelled. The subagent hung with no card until Stop.
  • The Codex handler leak you list is easy to close in the adapter. If the adapter settles the pending approval's Deferred with cancel when the turn ends interrupted, Codex gets a reply and the handler fiber finishes. The true result from Deferred.succeed also guards the approve-while-interrupting race. I have this with a failing-first adapter test and can send it as a separate PR once this one lands.

Thanks for writing up the turn-ownership cases so clearly.

Synced from GitHub

Thanks @SudheerNaraharisetty, this is really helpful. The provider-log ordering and your second run show that a real Codex cancel reaches this path. I've linked your comment in the "What I couldn't prove" section of the PR description. Thanks as well for confirming that completed turns should stay out.

Protect approval cancellations against a resolution race.

guardPendingUserInputCancellations rechecks only user-input requests. An approval can resolve after selection and before the cancellation write, allowing stale cancellation events to overwrite the resolution.

Extend the guard to approval requests while continuing to exclude message-mode questions.

@coderabbitai Good catch, fixed. The guard now rechecks approval requests too (still pending, same kind and turn, message-mode left out), and there's a test for an approval answered after the pending requests were read.

Validate the approval inside the serialized response write.

For an interrupted or cancelled turn, approval cleanup can cancel the request after Orchestrator.getRuntimeResponseContext(...) reads it as pending but before EventSink.commitCommand(...) appends the already-built resolved response. commitCommand serializes writes but does not revalidate the request. This can persist a resolved approval after cleanup marked it cancelled and not_resumable.

Validate pending, kind, and providerTurnId inside the serialized write transaction immediately before appending the response. A standalone read immediately before commitCommand does not close this race.

On CodeRabbit's outside-diff note about the respond/cancel race: it's real, and it's the reverse of the case I covered. A late cancel can't overwrite an answer anymore, but a respond that read the request as pending before the interrupt can still commit "resolved" after the cancellation and queue a response effect for a turn that's gone. Approvals only hit this since this PR started cancelling them; questions have the same gap on main.

The fix I have in mind is small: commitCommand re-reads the request inside the transaction and rejects the respond if it's no longer pending or the kind or provider turn changed, and dispatchRuntimeRequestRespond maps that to the existing "is cancelled" error, with a latch test for the interleaving. @juliusmarminge since you're driving this branch now, want me to push that here, or keep it for a follow-up PR?
