Problem

On Claude threads, any MCP tool that asks the user to confirm something gets decline back right away, and the user never sees a prompt. The same tool on Codex shows the app-access card. The Claude adapter never registered the Agent SDK's onElicitation callback, so the SDK declines on its own.

Change

The adapter now registers onElicitation, opens the existing mcp-elicitation card with Cancel, Decline and Approve, and waits for the user to pick one.

A few calls I made:

  • Claude uses Codex's toMcpElicitationResponse to decide what counts as a plain approval, so the two providers agree. That also handles JonasFocus's point on #14895: allow_once, Allow and accept_once now work.
  • A form with no fields is a plain confirm. Approve answers it with { action: "accept", content: {} }.
  • The card can only say yes or no, so it never sends a value the user didn't see. Schema defaults are dropped, since one could be always or a refusal. CodeRabbit caught this.
  • URL mode, a missing schema, and forms that need real input are declined without showing a card.
  • Approve is always one-time. No session or always grants, and no Claude permission rules get written.
  • If the session stops while the card is open, the request is cancelled. A response that arrives after that gets the usual unknown-request error.
  • MCP servers that return input_required from a tool call go through the same callback. I checked this in the Claude Code 2.1.304 bundle.

Not covered: URL-mode auth, forms with real inputs, and session or always grants. Also, the shared helper picks an option like "disallow" if it comes before the allow option. That fails safe, since the server gets a refusal, and JonasFocus suggested fixing it for both providers in a separate PR.

Scope and approval

Fixes #14878, following the scope set in the triage comment: one-time accept only, reuse the existing card, and decline anything we can't handle. This redoes #14895, which was closed in the October cleanup, on current main.

Verification

  • Focused adapter tests cover: the card waiting and accepting only after Approve, Decline, Cancel, URL and value forms declined with no card, schema defaults never sent, abort before an answer, session stop with the card open, and a late response. The Claude, Codex and elicitation-helper test files pass, and the server typecheck is clean.
  • In a local web dev build, a Claude Haiku 5.5 thread called a test MCP tool that asks for a plain confirm ({ type: "object", properties: {} }). On main (f740a97947) the tool got {"action":"decline"} back after 7.6s and no card showed up. With this branch the card showed up, and Approve sent {"action":"accept","content":{}} back to the server.
  • Not checked by hand: Decline and Cancel (the tests cover both), desktop, and mobile. Desktop and mobile show the same card Codex already uses.

📝 Walkthrough

ClaudeAdapterV2 now handles supported MCP form elicitation requests with approval prompts. It returns accepted form content after approval and preserves decline and cancellation outcomes. URL-mode and unsupported requests are declined.

Merge Risk: 🔵 Low · up to cac64

An unusual form can send a refusal when the user clicks Approve. The previously reported default-backed grant risk is fixed; the remaining issue is narrow but worth correcting.

Security architecture risk: 🔵 Low · up to cac64

The new flow waits for an explicit decision, restricts approval to one-time responses, and declines forms the approval card cannot answer. No unauthorized action was established. Remaining uncertainty concerns cancellation during shutdown and how external servers interpret automatically selected form values.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The added exposure is an MCP server obtaining a user-mediated elicitation response during an active Claude turn. The inspected response path remains scoped by thread, runtime request, and live provider-session identity; it does not introduce a demonstrated cross-session authority path.

Trust Boundaries and Controls

  • observed — MCP-supplied schemas influence response content through the existing converter, but do not by themselves authorize acceptance. The callback waits for a decision, while runtime response execution checks that the projected request is resolved, has a live capability for the selected provider session, and has an active session.

Resilience and Maintainability Implications

  • observed — Cancellation and cleanup are implemented through the SDK callback signal. Session/query shutdown calls query.close without directly settling elicitation deferreds. The session-stop test explicitly makes its fake close abort that signal, so it establishes adapter behavior under abortion, not the exact production SDK shutdown guarantee.

Hardening Proposals

  • proposed — Define a documented provider-neutral contract for approval-only enum values, declining ambiguous values rather than interpreting approval substrings. This would strengthen consent semantics without treating an unverified downstream interpretation as a vulnerability.
  • proposed — Make session-owned settlement and cleanup explicit across publication, waiting, and shutdown, or document a version-applicable SDK cancellation guarantee. This would make stopped-session approval invalidation independent of an implicit external lifecycle assumption.

Explanation

The PR satisfies #14878. ClaudeAdapterV2 registers onElicitation and routes supported approval-only form requests to the existing mcp-elicitation card. The flow waits for Cancel, Decline, or Approve. Approve returns a one-time accept. URL mode, missing schemas, and unsupported value requests decline without a card. Tests cover decisions, cancellation, session stop, aborts, and late responses. The supplied PR summary and change summary establish this behavior.

Full details: Out of Scope Changes check

Explanation

The changes remain within #14878. The CodexMcpElicitation helper change generalizes the existing response builder for Claude's MCP request envelope. The Claude adapter changes implement the requested callback and card flow. The added adapter tests cover that behavior. No unrelated product behavior is identified in the supplied change summary.

Fixed in cac644c5d7. The Claude path now drops default from every field before filling the form, so Approve only sends a one-time choice or a false persistence toggle. A required field that only a default could fill is declined with no card, and an optional one is left out.

Match one-time approval values exactly.

When a required enum is ["disallow", "allow"], toMcpElicitationResponse selects "disallow" because /allow/i matches its substring. claudeMcpElicitationAcceptance then returns action: "accept" with the refusal value, so an Approve click can send a refusal to the MCP server. Use an anchored matcher and add this ordering as a regression case.

Leaving this one for a separate PR. The substring match lives in the shared toMcpElicitationResponse, so fixing it changes Codex too. On #14895 the reviewer asked for it to be fixed for both providers in its own PR, and an exact-match list is the stricter approach they pushed back on, since it would stop matching values like allow_this_time. It also fails safe: the server gets a refusal, never a grant. The PR description lists it under "Not covered".

@chrischriscris Thanks for the clarification. Handling the shared matcher in a separate PR makes sense because it affects both Claude and Codex.

My proposed anchored matcher is too restrictive: it excludes allow_this_time. The reported ["disallow", "allow"] case sends a refusal value, not an unintended grant. The correctness issue remains, but I accept deferring it from this PR.

Would you like me to open a follow-up issue for both providers?

5Learning: In pingdotgg/t3code, apps/server/src/provider/CodexMcpElicitation.ts contains the shared toMcpElicitationResponse helper used by Claude and Codex. Approval-value matching must preserve one-time variants such as allow_this_time; a narrow exact-match allowlist can reject supported server values. Matcher changes must account for both providers.
