Settling a parent thread can unload an independently running delegated child that shares its provider runtime. The child then loses replies and completion events. Detach now preserves subscriptions owned by attached child threads while continuing to unload unattached mirrors.

Each provider thread is checked under its owner's attachment lock, one lock at a time. An attachment that arrives first is preserved; a resume during unload waits and reloads afterward. Cleanup clears only the matching cached load and idle timer. This replaces the original owner-grouping map with one loop. Provider adapters, contracts and clients are unchanged.

Retains @jfet97's diagnosis and original commits from #20361. Main is merged; no history was rewritten. Fixes #20348.

Verification on headless Linux:

  • Four original added tests → two focused manager races in the existing manager test file, removing 190 test lines. Both fail against main; all 67 manager tests pass with the fix.
  • 190 scoped tests pass: 67 manager, 100 unchanged OpenCode-adapter and 23 orchestration/session/cancellation integration cases. Eight Codex/OpenCode replay cases pass. Server and OpenCode typechecks, targeted lint and formatting pass.
  • A running service harness uses the actual orchestrator, manager, event ingestor, effect worker and isolated in-memory SQLite with a subscription-filtered adapter fixture. It creates an app-owned child through delegated_task.request, starts a follow-up through message.dispatch, and settles the parent through thread.settle. Only the parent's native thread unloads. The child's later AFTER_PARENT_SETTLED reply persists and its run completes. This exercises supported commands and production detach effects without paid provider calls or live userdata. The assertions pass; fixture teardown can log a detached completion-handler read after its scoped database closes.

This is a server-only change; screenshots do not demonstrate subscription ownership. Actual provider CLI delivery, browser presentation, macOS, Windows, iOS, Android, Electron desktop and relay/tunnel routes were not exercised. Native-client access is disabled on this Linux host.

Three independent full-diff review rounds by Claude Opus 5.5 and GPT-6-Sol; both final reviews found no code blockers. An earlier adapter expansion was reverted after review confirmed provider-created native children are read-only and its lifetime changes could retain unused sessions. The final change stays in the manager.

Implementation: GPT-6.1-Sol; tests: GPT-6-Astra; independent review: Claude Opus 5.5 and GPT-6-Sol, through T3 Code delegation using the Codex and Claude harnesses.

📝 Walkthrough

Parent detach cleanup now processes provider threads under each owning thread’s attachment lock. It updates matching loaded and idle-unload state, and skips unloading after runtime replacement or reattachment. Tests cover native-child attachment, projection-read races, and unload races.

Changes: Native child cleanup

Merge Risk: ⚪ Minimal · up to d645c

The server-side child-subscription fix is mergeable after normal checks; no actionable merge-blocking risk is established.

Security architecture risk: 🔵 Low · up to d645c

Cleanup now preserves independently attached child work without adding public operations or privileges. The examined changes do not establish a new security defect, but recovery after failed, timed-out, or interrupted cleanup remains only partly established.

Retained concerns No architecture-level concerns identified.

Security review details

Security Blast Radius

  • inferred — The directly changed decisions affect native-thread subscriptions and cached lifecycle state within a shared provider session, including child owners referenced by its projection. The inspected change does not add an attacker-facing input or grant new tool authority. Repository-wide or tenant-wide exposure is not established by the available coverage.

Trust Boundaries and Controls

  • observed — Cleanup retains the existing provider-session filter and adds an owner-level attachment check under a lock keyed by provider session and application thread. A runtime-instance check prevents the stale cleanup snapshot from modifying a replacement runtime. These are lifecycle ownership controls, not a new caller-authentication boundary.

Resilience and Maintainability Implications

  • observed — Unload remains timeout-bounded and warning-only on failure; matching cached state is cleared before the call. A later resume reloads when the cache is absent. Codex unload sends an external unsubscribe request, whereas OpenCode removes eligible local registrations and then MCP configuration. The interface does not specify partial-failure or cancellation semantics, so recovery after delayed completion or interruption remains a coverage gap rather than an established PR regression.

Hardening Proposals

  • proposed — Define adapter unload guarantees for retries, cancellation, and late completion, including whether an earlier unsubscribe can take effect after a subsequent resume. Validate those guarantees with partial-failure and interrupted-cleanup scenarios before relying on them as a recovery contract.

Explanation

The description explains the problem, the implementation, scope, linked issue, verification results, limitations, and test evidence. Although it does not use the template headings explicitly, it provides the required information and is sufficiently complete.

Full details: Linked Issues check

Explanation

#20348 is an active direct issue. ProviderSessionManager.ts now processes each native provider thread under its owner attachment lock. It preserves an attached child, waits for a concurrent resume during unload, clears only matching loaded and idle state, and still unloads unattached native mirrors. The two added manager tests cover child attachment during detach and resume during unload. These changes meet the issue's coding and regression-test requirements.

Full details: Out of Scope Changes check

Explanation

The diff changes only ProviderSessionManager.ts and its manager test file. The new load identity, lock handling, cleanup changes, and test fixtures support the ownership and unload requirements in #20348. No unrelated product area or provider adapter is changed.

Match idle-unload state by runtime identity.

detach snapshots provider threads before it removes the attachment. During that window, the resume fallback can replace the native runtime while preserving the same provider-thread ID. If the replacement becomes idle before cleanup takes the owner lock, clearIdleUnload matches the old snapshot by ID, but clearLoaded rejects it by runtime identity. Cleanup then cancels and removes the replacement's idle unload while leaving its loaded marker intact. The replacement native thread can remain loaded without another idle unload scheduled.

@coderabbitai Could you re-evaluate this finding against ec23c247?

I traced the normal path and could not establish the described failure. If the replacement child is still attached, cleanup skips it under its own attachment lock. If the child detaches, that operation removes its attachment, loaded marker and idle entry together. The child's ordinary turn-start and detach effects run sequentially through the per-thread outbox; terminal scheduling cannot recreate a deleted idle entry.

There is a cancellation caveat: an effect can be marked cancelled before its fiber stops, and tracking writes occur after the attachment lock. That gap already exists on the base commit, so serialization alone is not an unconditional proof. I have not found a complete production sequence showing an additional failure caused by this PR's ID comparison.

Matching native identity is stronger consistency protection. Keeping that timer does not unload an unattached owner, although it could become useful after a later bare open() reattaches the owner. That makes the suggestion plausible hardening, rather than an established fix for the reported symptom.

Could you show a concrete sequence through the current callers that produces the claimed failure, including the cancellation path if necessary? I have left the code unchanged while this remains unverified; a demonstrated defect will get a focused regression and correction.

@jfet97 I re-evaluated ec23c247 and compared the relevant paths with the base commit. I withdraw this finding. Thanks for the correction.

My original sequence omitted two checks:

  • If the replacement child remains attached, unloadForOwner skips cleanup under the child’s attachment lock.
  • If the child detaches, detach removes its attachment, loaded marker, and idle entry in one Ref.modify.

scheduleThreadUnload cannot recreate that deleted idle entry. It requires an existing entry and preserves a missing entry during its update.

The cancellation caveat is real. cancelUnsettled can make the next effect claimable before the previous execution fiber stops. Also, tracking writes occur after ensureThreadAttached releases the attachment lock. Those conditions already exist on the base commit, however. I could not establish a complete production sequence where this PR’s ID comparison causes the additional failure I claimed.

Matching native identity is reasonable consistency hardening, but I do not have evidence that it is a required defect fix here. A retained timer also skips unloading while its owner is unattached. No code change is required for this finding.

5Learning: In apps/server/src/orchestration-v2/ProviderSessionManager.ts, parent detach cleanup checks each owner's attachment under that owner's attachment lock. An attached child is skipped. A child's detach atomically removes its attachment, loaded marker, and idle-unload entry; scheduleThreadUnload cannot recreate a missing idle entry. When reviewing this TypeScript Effect lifecycle, do not infer a replacement-runtime timer-loss defect from the provider-thread ID comparison alone. Establish a reachable caller sequence. In apps/server/src/orchestration-v2/EffectOutbox.ts and EffectWorker.ts, durable cancellation can permit another same-thread effect before the previous execution fiber stops, so outbox ordering is not an unconditional fiber-serialization guarantee.

Sorry for the extra merge, @juliusmarminge. I had set up an automated job to keep this PR aligned with upstream, and it ran after you took over.

I've disabled it now and will leave the branch to you.

To clarify: it was only a merge from upstream main. Your implementation and tests are unchanged; nothing was overwritten or restored.
