Managed ChatGPT token renewal updates saved credentials, but a running Codex app-server keeps the ACCESS_TOKEN it received at launch. Reusing that process after expiry can make every later turn fail with 401 token_expired until the server restarts. This addresses #16132.

Managed Codex now supplies an optional stale-credential probe. It reads current auth through the existing access gate, allowing renewal and persistence, and compares the credential revision with the launch revision. Before reusing an idle session, the manager uses its normal release path to close a stale runtime and then starts a replacement. The existing native conversation identity and resume flow are preserved. Any busy turn in the shared process prevents replacement; the release path rechecks runtime identity and busy turns atomically after the probe yields. Probe failure or sign-in activity preserves the current runtime. Other providers do not supply this hook.

Human approval is still required for the background-work tradeoff: replacing an idle stale process can stop background commands that are still running. This change deliberately does not wait for that work, since it could delay recovery indefinitely. It does not restart a busy turn, which may still fail on expiry; the next open can recover. The takeover does not settle the prior maintainer approval request. Existing forced session release can also leave saved command/subagent items marked pending after the process stops; this change does not add general release-item terminalization. A sibling open can replace the runtime in the gap before another thread marks its turn busy. These limitations need maintainer consideration alongside the interruption policy.

The takeover preserves @voltcrash’s commits and merges main. It removes an unnecessary runtime revision property, uses the driver’s existing auth service directly, and reuses session-release logging. The original two tests were replaced with two behavior tests in existing files: managed-driver auth renewal and launch credentials, and shared-session replacement after a sibling turn settles.

Verification:

  • Both behavior tests fail on main. Final focused suites: 361 passed across seven files.
  • The managed-driver test exercises real auth refresh/persistence with dummy credentials, mocked HTTP and TestClock, then observes the renewed credential on the next adapter launch. It also covers temporary refresh failure and missing auth.
  • The manager test verifies busy-sibling protection, terminal-event settlement, an activity event arriving during the credential probe, replacement despite pending background work, one process close, subsequent reuse, and persisted native conversation resume.
  • Targeted lint, formatting, server and provider-core typechecks and diff whitespace check passed. Two existing adapter lint warnings remain.
  • Parent replay: 2 passed. Two independent full-diff review rounds with Opus and Sol; both final reviewers found no code blocker. The activity-during-probe race failed before its fix and passes after it.
  • Before/after regression and current-head verification proof.

The driver/auth and manager lifecycle are verified in separate tests; this is not a full orchestration-worker run against live Codex. Real ChatGPT expiry/renewal, real native process recovery, browser clients, macOS, Windows, Electron desktop, iOS and Android were not verified. No screenshot claim is made.

📝 Walkthrough

Managed Codex sessions now expose a check for credential revisions. Before reusing a live session, the session manager checks whether its launch-time credentials are stale and releases eligible sessions so the existing open path can create a replacement.

Explanation

Issue #16132 requires managed ChatGPT Codex conversations to recover after credential renewal without an application restart or another sign-in. The PR adds a revision-based stale-credential probe, supplies the managed token revision, and releases an idle stale session before reuse. The next turn opens Codex with renewed credentials and resumes the native thread. Tests cover revision detection and stale-session replacement. A turn that is already running can still fail if expiry occurs during that turn, but the next turn recovers as required by the implemented approach.

Full details: Out of Scope Changes check

Explanation

The changes remain within issue #16132. They add optional session-runtime stale-credential detection, connect managed Codex authentication to the revision, replace stale sessions, and add focused tests. The optional hook leaves other providers unchanged. No unrelated client, contract, or provider behavior changes are identified.

Full details: Description check

Explanation

The description clearly covers the problem, implementation, scope, tradeoffs, linked issue, verification, limitations, and agent details. It does not repeat the template headings verbatim, and maintainer approval for the background-work tradeoff remains pending, but the content is sufficiently complete for review.

Full details: Approvability

Explanation

Rule: changes authentication or credentials. The pull request changes managed Codex credential handling in apps/server/src/provider/CodexManagedRuntime.ts and apps/server/src/provider/Drivers/CodexManagedProvider.ts by reading the access token revision and using it to detect renewed credentials. It also changes apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts and apps/server/src/orchestration-v2/ProviderSessionManager.ts to replace sessions based on credential state. The pull request needs a maintainer's review.

Resolution

A maintainer must review the managed authentication and credential-renewal changes before approval.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Do not reuse a stale session while background work delays release.

If a Codex turn settles with a background command still running, this branch keeps the stale process. open then returns that process for the next turn, which can fail authentication with its launch token despite successful renewal. Preserve the background work, but defer the new turn or use a fresh session instead of returning the stale runtime.

✅ Confirmed as addressed by @voltcrash

Valid, fixed in 33fcaf494. Deferring the turn could wait indefinitely on a long-lived background command, and sessions are keyed by providerSessionId, so a fresh process can't run alongside the stale one. A stale process fails auth on the next turn anyway, so releaseIfStaleCredentials no longer checks pending background work. Only a running turn keeps the process now. The test now opens with background work pending and confirms the session is replaced.

@voltcrash Thanks for the explanation and fix. releaseIfStaleCredentials now replaces an idle stale session even when background work is pending. The running-turn guard remains in place. This addresses the stale-runtime reuse concern.

5Learning: In apps/server/src/orchestration-v2/ProviderSessionManager.ts, live sessions are keyed by providerSessionId, so a replacement process cannot run alongside the stale process under the same session ID. Pending background commands can be long-lived, so waiting for them can defer credential recovery indefinitely. For stale-credential replacement, pending background work must not prevent release; running turns remain protected.

apps/server/src/orchestration-v2/Adapters/CodexAdapterV2.ts:6044-6047 🩺 Stability & Availability | 🟠 Major | ⚡ Quick win

Propagate failed revision probes instead of treating them as current.

Effect.orElseSucceed(() => false) classifies a failed currentRuntimeRevision probe as non-stale. ProviderSessionManager.open can then return the existing runtime, whose launch-time token may be invalid after credential renewal. The next turn can fail authentication.

Let the probe failure propagate. open will fail before releaseEntry, retain the session, and allow a later probe to determine whether replacement is required.

@coderabbitai Re: the outside-diff finding on CodexAdapterV2.ts (hasStaleCredentials falling back to false). I'm leaving this as is, deliberately.

  • The suggested change wouldn't change behavior. releaseIfStaleCredentials in ProviderSessionManager.ts catches every cause and only logs a warning, so open would not fail before releaseEntry. It would reuse the existing session as it does now, just with an extra warning.
  • Making open fail whenever the probe fails would be worse. The probe fails when a sign-in is in progress (withAccess rejects with "sign-in is changing") or when the token can't be renewed. During a sign-in, that would fail every turn, even though the sign-in releases the sessions itself when it finishes. If renewal fails, the running process may still hold an unexpired token, so failing the turn early would break a session that still works. A fresh process wouldn't have a valid token either.

Treating an unreadable revision as "not stale" keeps the current behavior in both cases. The adapter test covers this ("A failed renewal keeps the session rather than replacing it blindly").

@voltcrash, you’re right. I withdraw the finding. releaseIfStaleCredentials catches probe failures and then lets open reuse the existing session. Propagating the failure from hasStaleCredentials would only add a warning under the current manager behavior. Returning false also avoids replacing a session when the current revision cannot be read. The adapter test covers that choice.

Thanks for explaining the intended behavior.

This replaces a stale Codex session even when it still has a background command running, so token renewal can stop that command before the next turn. Do maintainers want that tradeoff, or should recovery preserve the background work? The linked issue offers recovery directions without settling this choice; please confirm it under the scope approval rule.

Taking over this one as maintainer. I’ll preserve the original contribution, merge current main, rebuild the credential-recovery path and focused tests, and run independent reviews. The background-command interruption tradeoff still needs human approval; this takeover does not settle that decision.

Takeover verification is complete at 594c4b9e70. All running CI jobs passed, all inline review threads are resolved, and the final Opus and Sol full-diff reviews found no code blocker. Focused verification proof.

The older CodeRabbit change request concerned reusing a stale runtime when background work was pending; that was fixed by the contributor and acknowledged in its resolved thread. The current manager regression verifies replacement despite background work and activity during the credential probe. I am dismissing that superseded bot change request with this explanation.

The older out-of-diff suggestion to propagate failed auth probes is intentionally declined: a failed renewal or sign-in transition does not establish that the running launch credential is invalid. The managed-driver regression verifies preservation on temporary refresh failure and missing saved auth.

Human approval remains required for interrupting background commands on renewal, including the existing stale pending-item display limitation described in the body. Real ChatGPT expiry and a full live worker-to-provider recovery flow remain unverified. Nothing is merged.
