🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Describe the project override.

The same-repository project is the default. If a caller supplies target.projectId, the task runs in that project instead. State the default and the override so users can choose the intended project. As per coding guidelines, user documentation should explain “how to start, and anything unintuitive.”

7Update the remote-access documentation to state that the same-repository project

Fixed. The docs now say the task runs in the project with the same repository, or the one target.projectId names.

C

@juliusmarminge Thanks. The updated text covers both the same-repository default and the target.projectId override.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

A transient failure on the first completion permanently blocks the task.

complete always uses the same command ID: command:remote-task-complete:${parentThreadId}:${taskId}. The first dispatch can fail for a transient reason, such as a SQL read error in planDelegatedCompletionDelivery or a projection read error. In that case, dispatchWithReceiptEffect stores a rejected receipt for that command ID. Each later dispatch then fails with OrchestratorCommandPreviouslyRejectedError.

followOnce still reads task.result === null. As a result, follow retries forever at the 5-minute backoff cap. The task never completes, and the parent never wakes. The same loop starts again at every restart through start().

A fixed command ID is not needed for idempotency. dispatchRemoteDelegatedTaskComplete already ignores a repeat report because it returns early when task.result !== null. To fix this, give each attempt its own command ID.

When the cancel completion and the follower completion race, the later dispatch is refused. Handle that refusal by re-reading the task.

8Update the complete helper to generate a fresh command ID for each dispatch
9attempt instead of reusing one derived only from parentThreadId and taskId. When
10a completion dispatch is refused because another completion won the race,
11re-read the task so the follower can observe its completed state and stop

Fixed. Each completion attempt has its own command id, and a report refused because the task already ended counts as done.

C

@juliusmarminge Thanks for fixing this. A fresh command ID lets each completion attempt retry after a transient failure. Treating a refused report as done when the task already ended handles the completion race.

🟡 Medium peer/RemoteDelegation.ts:269

Remote tasks with a final answer longer than 20,000 characters are completed locally with only the answer’s prefix, so the parent receives an incomplete result. lastReply keeps item.text without checking textTruncated or fetching the rest; use the selected item’s itemId and nextTextOffset to retrieve the remaining text before completing the task.

No longer relevant as of 8f7d28c5dcf8affa53f40bbbc4bddcc98f6691c9

J

Fixed. A truncated reply is read to the end through itemId and textOffset. Tested with a 45,000-character reply.

🟡 Medium peer/RemoteDelegation.ts:184

Remote delegated tasks ignore input.role, so a child launched with a specialized role receives only input.task and is not instructed to act as that sub-agent. Unlike local delegateTask, this path does not apply taskPrompt(input), and t3_thread_launch has no role parameter; apply the same prompt transformation to the launch message.

No longer relevant as of 8f7d28c5dcf8affa53f40bbbc4bddcc98f6691c9

J

Fixed. The remote launch message now gets taskPrompt(input), like a local child.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

A deleted or unknown remote thread makes the follower retry forever.

follow handles only capability_denied as a final failure. The remote thread can be deleted or archived, or the far side can reply thread_not_found or run_not_found. In those cases, waitForThread or t3_thread_read returns an OrchestratorMcpFailure that is not capability_denied. Effect.retry then retries the same call every 5 minutes with no limit. The task stays running and the parent never wakes. getOpenRemoteDelegatedTasks returns the task again at each boot.

Treat these failure codes as final and complete the task as failed with the message from the far side.

Fixed. thread_not_found, run_not_found, and a link forgotten here are now final: the task fails with the reason. Tested.

🟠 High peer/RemoteDelegation.ts:240

A delegation with input.isolation: "worktree" launches in the remote project's shared checkout, so the requested isolation is not honored and the task can mix with or overwrite other work. This launch omits workspaceStrategy, which defaults to { type: "root" }; pass the corresponding strategy or reject unsupported isolation before launching.

Fixed in 1d2557f9e2 by refusing it: delegate_task with environmentId and isolation "worktree" now fails with invalid_request before anything launches. A worktree there would branch from the parent's local commit, which the linked checkout may not have; the message points at t3_thread_launch with environmentId and a worktree workspaceStrategy for that case. The test checks the refusal and that nothing launched; it fails with the guard removed.

This new service translates a dispatch failure into a caller-visible message built from error.message and drops the underlying error. Could RemoteDelegation expose a structured domain error with the dispatch error as cause, then map it to the MCP failure at the tool boundary using a fixed, safe message? The cancellation dispatch at line 735 needs the same treatment.

Gate remote-thread actions on the connected phase.

useEnvironment returns a presentation for catalog entries even when its connection phase is not "connected". Check that phase before exposing each remote-thread action.

🟡 Medium peer/RemoteDelegation.ts:185

A failed remote run is recorded with a stale assistant message or the generic noReply text instead of its provider error, so the parent task status and wake omit the failure reason. lastReply only accepts assistant_message items; include the failed run’s error item when collecting its result.

Fixed in c154634b48, Lenori's commit (thanks @Lenoridev). A failed run now reads the activity view and reports that run's own error item, never another run's. Five tests cover it; the error cases fail without the fix.

🟠 High peer/RemoteDelegation.ts:395

A cancelled remote task can keep running and editing files indefinitely after its peer is unreachable. stopThere gives up after five retries, and Effect.ignoreCause lets followOnce finish; because the local task already has a result, start will not recover it after a restart. Persist pending remote stops and retry them until they succeed.

apps/server/src/orchestration-v2/ThreadManagementService.ts:783

Recording delegated_task.remote.complete here durably sets a non-null result before the remote child is stopped; the actual interrupt is left to the in-memory follower. If the server restarts after this commit (especially while the remote environment is offline), that stop is permanently lost: RemoteDelegation.start only follows getOpenRemoteDelegatedTasks, whose query in ProjectionStore.ts requires result to be null. Retrying stopDelegatedTasks also skips this row at line 782. The task is displayed as interrupted while its remote run can continue executing and editing files. Persist pending remote stops separately or include unacknowledged interrupted tasks in restart recovery.

No longer relevant as of 48cf33b9e2c15eabd7a3de0059f9f3c000614497

J

Fixed. stopThere no longer gives up: remoteChild.stoppedThere records whether the stop landed, getOpenRemoteDelegatedTasks returns cancelled or stopped tasks without it, and the follower keeps sending the stop, across restarts, until the other side acknowledges it. A refusal on mode grounds ends it. Tested with the box unreachable and across a restart.

🟠 High orchestration-v2/Orchestrator.ts:7178

A delayed acknowledgement for stop-1 marks the current remoteChild stopped even if stopCount has since advanced for stop-2, so the follow-up and recovery paths skip the outstanding stop while remote work continues. This handler applies stoppedThere without checking which stop generation the response acknowledges; carry that generation in the completion command and update the state only if it matches the current stopCount.

Fixed in 3c1af63346, Lenori's commit (thanks @Lenoridev). A stopped report carries the stop generation it acknowledges, and the handler only clears an owed stop when that matches the task's current stopCount, so a late stop-1 acknowledgement leaves stop 2 owed and the follower sends it. Two tests cover it, and both fail with the guard removed.

🟠 High peer/RemoteDelegation.ts:214

A successful remote launch keeps running untracked when the local threads.dispatch fails, so the caller gets a delegation failure while the remote agent continues executing. This mapError only converts the recording error and never interrupts launched.threadId; interrupt the launched child before returning the failure.

No longer relevant as of 8f7d28c5dcf8affa53f40bbbc4bddcc98f6691c9

J

Fixed. If recording the task here fails after the launch, the launched thread is interrupted before the failure returns.

Interrupt remote delegated tasks when the parent thread is stopped.

thread.stop dispatches run.interrupt with holdQueue: true, which enqueues delegated-tasks.stop. That effect only stops delegated tasks with a non-null childThreadId. Remote tasks store their child in remoteChild and have no local child thread, so the loop skips them. holdStoppedThread only disposes completion delivery. The remote thread therefore continues running after the parent Stop, despite the Stop contract stating that delegated tasks are stopped.

Handle remoteChild tasks in delegated-tasks.stop by forwarding t3_thread_interrupt through RemoteDelegation.cancel, while preserving the existing recursive handling for local child threads.

Fixed. stopDelegatedTasks completes open remote tasks as interrupted, and their followers stop the thread there with t3_thread_interrupt stop: true (the whole thread, its queue and its own delegated tasks). The stop owed is kept on remoteChild.stoppedThere, so it survives a peer that is down and a restart here. Tested.

Record the local task before launching remote work.

delegate launches the remote thread before it dispatches the local task record. If that dispatch fails, the method returns an error without starting follow. The remote thread can continue, but start() only resumes tasks returned by getOpenRemoteDelegatedTasks, so the child has no local owner or completion path.

The request key only helps when the caller supplies and reuses it. When omitted, line 194 generates a key that is not returned to the caller. Create a durable pending local record before launching, then attach the returned remote thread ID to that record, or add an equivalent reconciliation path for failed dispatches.

8In the `delegate` flow, dispatch the durable local task record before launching

10alternatively, ensure failed dispatches are reconciled so every launched child
11has a local owner and completion path. Preserve replay behavior for requests

@juliusmarminge Thanks for clarifying. Interrupting the launched thread handles a reported record failure, and retries with the same clientRequestId can recover the local record.

The crash or lost-response window remains unresolved in this finding. #17051 tracks that remaining work; no duplicate issue is needed.

🟠 High peer/RemoteDelegation.ts:192

When a later provider session on the same parent reuses a clientRequestId, delegate launches a new remote thread but replays the prior local receipt, returning the old task ID and never recording or following the new child. The local commandId omits scope.requestNamespace, which scopes the remote launch; include that namespace in the local idempotency key too.

A follow-up run in the remote thread can be mistaken for this delegated task: waitForThread receives no runId, so it waits on the latest run, and lastReply can return that run’s reply. Persist launched.runId and scope both waiting and result selection to it.

When a remote child has already produced a result but started follow-up work, stopDelegatedTasks skips it, leaving that work running after the parent is stopped. The task.result !== null guard causes this skip, and RemoteDelegation.followOnce exits for completed tasks with a result; use the remote stop path that RemoteDelegation.cancel uses for this case.

🟠 High peer/RemoteDelegation.ts:569

When the launched remote run becomes terminal, this code completes the delegated task with that run’s reply even if the remote thread still has an async child or completion delivery in progress. The local parent therefore settles before the remote agent resumes and produces its final result; waitForThread only reports the selected run’s terminal status. Follow the remote thread’s delegated-work progress and capture its eventual result run before calling complete.

🎯 Functional Correctness | 🟠 Major | ⚡ Quick win

Reject a local project target that delegation cannot honor.

If target.environmentId names this environment and target.projectId names another project, remoteTarget returns undefined. This branch then calls service.delegateTask, which does not pass projectId to the local task command. The task runs in the parent project instead of the requested project. Reject an unsupported local projectId, or implement local project selection before accepting it.
