fix(ui): keep incident markers stable across refresh and fallbacks

Review fixes for the incident-attribution change:

- In a vertical panel with hidden meters, an incident on a provider other
than the selected one kept no marker: the identity badge now anchors to
the primary incident's provider via identityCarriesIncidentBadge, and
the standalone status element stays available whenever that icon cannot
carry the badge, so every incident stays visible and attributed.
- The meter incident badge no longer hides while a refresh runs; the
refresh keeps rendered providers, so the badge must not flicker away
  with loading.
- The provider account label now fills the meta row capped at its own
text width, so Text.ElideMiddle actually applies on narrow popups
while the plan stays directly beside the email.
- Tests: decouple the overview detail predicates (nonempty unknown
status falls through to source; source/title/provider repetitions
matched independently), reset the incident fixture state in init(),
and cover the vertical fallback rows, the refresh badge, and the
header elision; surface contracts updated to the new bindings.
