The panel service-status dot was a single floating element, so with a Codex outage it sat beside whichever provider came first and read as that provider's outage. The badge now lives on the incident provider's own meter icon (horizontal and vertical) and follows provider reordering; the standalone status element stays only as a fallback for incidents no meter can badge, and the vertical identity icon badges only its own provider. The provider header stretches the account label across the row, pushing the subscription label toward the popup's middle; the plan now sits directly beside the email with a trailing filler, eliding the account first. The overview detail line falls back to any status text, so Claude showed 'All Systems Operational' where other providers show their account email. Only an active incident may now stand in for identity, and a source that repeats the provider's own name is suppressed. Claude has no account identity on CLI 0.56.2; pinned in TODO. Verification: make check (876 QtTests, 75 Python tests, lint/hardening pass; 13 optional-module skips locally, CI rejects skips). New test_overview_detail.py covers the fallback chain; tst_visual_layout covers badge attribution and the standalone fallback; surface checks updated.
