Fix new-thread shortcuts when terminal is focused

- move chat-wide key handling into `_chat` route-level shortcut handler
- extract reusable `useHandleNewThread` hook and `isTerminalFocused` helper
- update browser WS fixture to support `terminalOpen` RPC shape
