## Message

```
Bind provider sessions to thread IDs and preserve resume cursors
- Require `threadId` when starting provider sessions and route recovery via persisted bindings
- Persist and propagate provider `resumeCursor`/`providerThreadId` instead of synthesizing values
- Expose full session bindings in `ProviderSessionDirectory` and update tests/integration fixtures
- Rename sqlite state file from `orchestration.sqlite` to `state.sqlite`
```
